Endoscopy-assisted treatment of a tubular esophageal duplication and bronchoesophageal fistula

Aim: The objective of this study was to demonstrate the efficacy of the endoscopic method in the treatment of a patient with tubular esophageal duplication and bronchoesophageal fistula.

Case:A 5-year-old male patient applied to our clinic with cough and a lung infection for 6 months. In the thoracic CT examination, a spherical hyper-dense foreign body (approx. 20x20 mm), which was located in the posterior of the trachea at the level of carina and atelectasis in the middle lobe of the right lung were observed. The graphical examination of the upper GIS passage showed a stricture, GER and a fistula between the esophagus and right main bronchus. We determined a fistula at the beginning of the right main bronchus and a septum (tubular duplication) with a length of 15-20 cm in the initial diagnostic bronchoscopy and endoscopy. The stomach was accessed with the endoscope through the right lumen but, the left one had a stricture at a distance of 20 cm.

Steps of the therapeutic process:

1) Cutting the septum endoscopically with the Olympus DualKnife™, formation only one lumen and removal of the plastic foreign body from the last segment of the septum.

2) Performing an endoscopic examination for the control of the outcome (one lumen) and Nissen antireflux gastrostomy due to the severe GER and malnutrition.

3) Repair of the bronchoesophageal fistula with thoracotomy.

No pathological finding was observed in the upper GIS passage imaging carried out two months after the surgery. After 4 months, the patient is still under follow-up without any complication and with normal oral nutrition.

Conclusion: Tubuler esophageal duplication and bronchoesophageal fistula are very rare. With the correction of the duplication, endoscopic surgery has been helpful and ensured that the integrity of the native esophagus is preserved without complications.

Tubuler özofageal duplikasyon ve bronkoözofageal fistülün endoskopi yardımlı tedavisi

Amaç: Tubuler özofageal duplikasyonu ve bronkoözofageal fistüllü olgunun tedavisinde endoskopik yöntemin etkinliğinin gösterilmesidir.

Olgu: Beş yaşında erkek olgu 6 aydır olan öksürük ve akciğer enfeksiyonu nedeni ile başvurdu. Toraks BT’de karina seviyesinde trakea posteriorunda, özofagus lümenine uzanımı bulunan, yaklaşık 20x20 mm boyutlarında sferik hiperdens yabancı cisim ve sağ akciğer orta lobda atelektazi izlendi. Üst GİS pasaj grafisinde torasik özofagus orta bölümünde darlık, GER ve özefagus ile sağ ana bronş arasında fistül mevcuttu. İlk yapılan tanısal bronkoskopi ve endoskopide; sağ ana bronş ağzında fistül, özofagus 15-20 cm arası septum(tubuler duplikasyon) saptandı. Sağ lümenden endoskop ile mideye geçilebildi, ancak solda 20. cm.de darlık vardı.

Tedavi basamakları;

1) Endoskopik olarak Olympus DualKnife™ kullanılarak septumun kesilerek özofagusun tek lümen haline getirilmesi ve septumun son kısmından plastik yabancı cisim çıkarılması;

2) Kontrol endoskopi(özofagus tek lümen halinde), şiddetli GER ve beslenememe nedeniyle Nissen antireflü, gastrostomi açılması;

3) Torakotomi ile bronkoözofageal fistülün onarımı şeklinde gerçekleştirildi.

Postoperatif 2.ayda yapılan üst gis pasaj grafisinde patoloji saptanmadı. Halen postoperatif 4.ayda tam oral beslenir şekilde sorunsuz olarak takip edilmektedir.

Sonuç: Tubuler özofageal duplikasyon ve bronkoözofageal fistül birlikteliği çok nadir bir durumdur. Duplikasyonun düzeltilmesinde endoskopik cerrahi yararlı olmuş ve nativ özofagus bütünlüğünün komplikasyon gelişmeden korunmasını sağlamıştır.
