Atrophy in testicular torsions: relation to torsion grade and duration of symptoms

Introduction and Aim: Testicular torsion is an emergency situation, with the spermatic cord and surrounding structures rotating around themselves, resulting in loss of testicular blood flow. Testicular detorsion is important for the protection of the testis and for future fertility. In this study, we aimed to determine the relationship between testicular atrophy and torsion degree and symptom duration in cases with detorsion of testicular torsion in our clinic.

Findings: Between 2008 and 1818, 42 boys who were diagnosed with testicular torsion and treated with detorsion in our clinic, were included in the study. The mean age of reference was 13.46 years (7-17 years), torsion were in the right in 17 and left in 25. Testicular atrophy was detected in 16 cases (38.8%). Testicular volume in the preop ultrasound of atrophic testis was 12,6 cc, whereas testis volume in the control ultrasound was 7.74 cc. Testicular volume was 13.40 cc in the preop ultrasound of the non-atrophy testis, whereas testis volume was 11.91 cc in the control ultrasound.

Regarding the degree of torsion independent of time, testicular atrophy was detected in all cases with testicular torsion in two rounds (720 degrees) without any atrophy in cases with testicular torsion in half round (180 degrees). Testicular atrophy was detected in 25.4% of cases with testicular torsion between 360 and 720 degrees. While atrophy was detected in 24% of the cases who applied to the first 6 hours in this group, the atrophy rate was 33.3% in cases who applied after 6 hours.

Conclusion: In testicular torsion, regardless the degree of torsion independent of time, as testicular atrophy was detected in all cases with testicular torsion with greater than 720 degrees and above, atrophy was not seen in any of the cases with torsion degree less than 360 degrees.
Torsion degree was statistically significant on testicular detorsions (p <0.05). In testicular torsion of 360-720 degrees,although the testis atrophy rate was found to be less in cases who is undergoing surgery in the first 6 hours ,it is not statistically significant. In all cases with testicular torsion in two rounds (720 degrees), all cases have testicular atrophy regardless of the duration despite the detorsion.

Testis torsiyonlarında atrofi: torsiyon derecesi ve semptom süresi ile ilişkisi

Giriş ve Amaç: Testis torsiyonu, spermatik kord ve etraf yapıların kendi etrafında dönmesi ve bunun sonucunda testis kan akımının kaybolması olup acil durumdur. Testis detorsiyonu, testisin korunması ve gelecekteki fertilite açısından önemlidir. Bu çalışmada, kliniğimizde testis torsiyonu tanısı ile detorsiyon yapılan olgularda testis atrofisi ile torsiyon derecesi ve semptom süresi arasındaki ilişkiyi ortaya koymayı amaçladık.

Bulgular: Çalışmaya 2008-2018 yılları arasında kliniğimizde testis torsiyonu tanısı alan ve detorsiyon yapılan 42 erkek olgu dâhil edildi. Ortalama başvuru yaşı 13.46(7-17 yıl) yıl olup, torsiyon 17 sinde sağ, 25‘inde sol taraftaydı. Olguların takiplerinde 16 (%38,8) olguda testis atrofisi tespit edildi. Olguların ortalama takip süresi 14,17 ay (3-48 ay idi. Atrofi olan testislerin preop ultrasonunda testis volümü ortalama 12.6 cc iken, detorsiyon sonrası kontrol ultrason da testis volümü 7.74 cc idi. Atrofiye olmayan testislerin preop ultrasonunda testis volümü ortalama 13.40 cc iken, detorsiyon sonrası kontrol ultrason da testis volümü 11.91 cc idi.

Süreden bağımsız olarak torsiyon derecesi açısından değerlendirildiğinde, yarım tur(180 derece) testis torsiyonu olan olguların hiçbirinde testisin atrofiye olmadığı, iki tur (720 derece) testis torsiyonu olan olguların tümünde testis atrofisi olarak tespit edildi. 360 ile 720 derece arasında testis torsiyonu olan olguların %25,4 inde testis atrofisi tespit edildi. Bu grupta ilk 6 saate başvuranların %24’ünde atrofi tespit edilirken, 6 saat sonrası başvuran olguların ise atrofi oranı %33,3 idi.

Sonuç: Testis torsiyonlarında, semptom süresinden bağımsız olarak torsiyon derecesi 720 derece ve üzeri olguların tümünde testis atrofiye iken, torsiyon derecesi 360 dereceden az olan olguların hiçbirinde testis atrofisi görülmedi. Testis detorsiyonlarının atrofik olması üzerinde torsiyon derecesi istatiksel olarak anlamlı bulundu(p < 0.05). 360-720 derece arası testis torsiyonlarında ilk 6 saatte ameliyata alınan olgularda testisin atrofiye gitme oranı daha düşük bulunmakla birlikte istatiksel olarak anlamlı bulunmadı. İki tur (720 derece) testis torsiyonu olan tüm olgularda detorsiyona rağmen süreden bağımsız olarak tüm olgular atrofiye gitmektedir.
