Neonatal gastrointestinal perforations: Ten years experience

Amaç: In this study it is aimed to present surgical outcomes of last 10 years’ gastrointestinal (GIS) perforations in newborns in a tertiary center.

Materyal-Metod: The charts of neonates who underwent surgery due to GIS perforation between 2008-2018 were retrospectively analyzed. Birth weights and gestational ages, symptoms, intraoperative findings and surgical outcomes were evaluated.

Results: Among 20 patients in total, there were 11 girls and 9 boys. Mean age at surgery was 6days (1-21days). Fifteen of the babies were prematüre and 5 were term babies. Diagnosis’ were necrotizing enterocolitis in 18 patients, spontaneous intestinal perforation in 1 and gastric perforation in 1 patient. Mean birth weight was 1776 grams (620-3910 grams). The most common symptom was abdominal distention (n=15) which was followed by vomiting and worsening in general condition. The perforation sites were; terminal ileum (n=8), appendix vermiformis (n=3), colon (n=3), caecum (n=2), jejunum (n=2), duodenum (n=1) and stomach (n=1). There were no intraoperative complications. There was a ileostomy revision need in a patient who had undergone surgery due to caecum perforation. In the second operation, a new perforation site at 0.5 cm proximal to the ileostomy was seen. In total, 16 patients treated via enterostomies, duodenum and gastric perforations were repaired primarily and two patients underwent resection and anastomosis. Among 20 patients, eight of them were lost due to non surgical complications (45%). Only one of these babies was term. Among these, four patients had suffered from terminal ileum, two from appendix, one from jejunum and one from duodenum perforation.

Conclusion: GIS perforations may be seen in neonatal period and be fatal. In spite of early and correct intervention, mortality rates stay high.

Yenidoğan Gastrointestinal Perforasyonları: On yıllık klinik deneyim

Amaç: Bu çalışmada son 10 yılda 3. basamak üniversite hastanesinde yenidoğan döneminde gastrointestinal perforasyon nedeni ile ameliyata alınan hastaların sunulması amaçlandı.

Materyal-Metod: 2008-2018 yılları arasında merkezimizde gastrointestinal perforasyon nedeni ile ameliyata alınan hastaların dosyaları geriye dönük olarak tarandı. Hastaların doğum kiloları ve haftaları, semptomları, ameliyat bulguları ve ameliyat sonrası dönemdeki komplikasyonları ile sağ kalımları değerlendirildi.

Bulgular: Çalışmadaki toplam 20 hastanın 11’i kız dokuzu erkekti. Ameliyat günü ortalama postnatal 6. gündü (1-21 gün). Hastaların 15 ‘i prematüre iken beş çocuk miyadında doğmuştu. 18 hasta nekrotizan enterokolit tanısı alırken 1 hastada spontan intestinal perforasyon düşünüldü, 1 hasta ise spontan gastrik perforasyon nedeni ile ameliyata alındı. Ortalama doğum ağırlıkları 1776 gram (620-3910 gram) idi. En sık semptom karın distansiyonu (n=15) idi. Karın distansiyonunu kusma ve genel durumda kötüleşme izlemekteydi. Ameliyata alınan hastalarda perforasyon yerleri şöyle idi: Terminal ileum (n=8), appendiks (n=3), kolon (n=3), çekum (n=2), jejenum (n=2), duodenum (n=1) ve mide (n=1). Hastaların hiçbirinde intraoperatif komplikasyon saptanmazken çekum perforasyonu nedeni ile ameliyat edilen ve ileostomi açılan bir hastada ileostominin çalışmaması üzerine tekrar ameliyat ihtiyacı oldu. Ameliyatta ileostominin 0.5 cm prokeimalinde perforasyon izlendi. Toplamda 16 hastaya enterostomi açılırken, duodenum ve mide perforasyonları primer onarıldı, 2 hastaya da rezeksiyon anastomoz yapıldı. Hastaların 8’i ameliyat sonrası dönemde ameliyat ile ilişkili olmayan sebeplerle kaybedildi (%45). Kaybedilen bebeklerden sadece 1’i term iken, 4 hastada terminal ileum, 2 hastada appendiks, 1 hastada jejunum ve 1 hastada da duodenum perforasyonu vardı. Duodenum perforasyonu izlenen hasta dışında tüm hastalara enterostomi açılmıştı

Sonuç: Gastrointestinal perforasyonlar yenidoğan döneminde görülebilen ve hayatı tehdit eden durumlardır. Erken ve doğru müdahaleye rağmen mortalitesi yüksektir.
