Controlled Elastic Loop for Intestinal Kinking (CELIK): A novel surgical technique to improve intestinal adaptation in short bowel syndrome

Autologus gastrointestinal reconstruction(AGIR) procedures for short bowel syndrome require adequate intestinal width.There are some techniques identified in order to contribute dilatation but none of them are adjustable so far.In this study,we aimed to share the results of the case which Controlled Elastic Loop for Intestinal Kinking(CELIK) technique was applied.

A 3 month-old boy(6.1kg) referred to our intestinal failure and rehabilitation center with ultra short bowel syndrome after radical intestinal resection and proximal jejunostomy due to midgut volvulus.Residual intestine was dudodenum,8cm proxial jejunum and distal half colon.Firstly,controlled jejunostomy obstruction with foley cathetery balloon for proximal jejunal segment dilatation was tested but could not succeed while he was 3 months old.After 2 months,jejunocolic tapering anastomosis was performed but intestinal diameter was still insufficient.During 10 months pediatric gastroenterology follow-up high calory TPN and partial enteral feeding kept on and he reached 8.1kg.Before AGIR procedures experimental CELIK procedure was planned.The mesentery of colonic segment which is 4 cm distal of jejunocolic anastomosis was hung on the anterior abdominal wall by an elastic loop for controlled intestinal dilation.In early postoperative period the tension of the elastic loop was increased step by step.Competent proximal dilation was showed with intermittent abdominal x-rays.After about a 4 weeks observation period, explorative laparotomy repeated. Proximal jejunum diameter which was approximattely 2.5 cm before CELIK procedure enlarged to 4 cm;previous 8 cm jejunum and 4 cm colonic segment were lenghtened together up to 24 cm by Bianchi technique.Postoperative enteral feeding tolerance progressed and the patient gained weight reaching 9.2 kg. With cyclic TPN he discharged and attending the clinic as an outpatient with a plan of second AGIR procedure following repeating CELIK procedure.

CELIK technique,which is different from its equivalents in that it easy to apply,physiological and reversible,is a safe and effective method of providing intestinal adaptation before AGIR procedures.


Çelik Prosedürü (Elastik halka ile kontrollü barsak daraltılması): Kısa bağırsak sendromunda bağırsak adaptasyonunun iyileştirilmesinde yeni bir cerrahi teknik

Kısa barsak sendromu (KBS) olgularında otolog barsak rekonstrüksiyonu prosedürleri (OBRP) uygulanabilmesi için yeterli barsak genişliği gerekmektedir. Bu amaçla dilatasyonu sağlamak için çeşitli yöntemler tanımlanmıştır. Ancak mevcut yöntemler ayarlanabilir değildir. Bu çalışmada Çelik prosedürü uyguladığımız bir olgunun sonuçlarının sunulması amaçlanmıştır.

Midgut volvulusa bağlı geniş barsak rezeksiyonu ve proksimal jejunostomi uygulanması sonrası KBS tanısı ile takip edilen 3 aylık (6,1 kg) olgu barsak yetmezliği ve rehabilitasyonu merkezimize yönlendirildi. Kalan barsak miktarı duedonum, 8cm’lik proksimal jejunum ve kolonun distal yarısından oluşmaktaydı. Olguda öncelikle proksimal jejunal segmentin dilatasyonu amacıyla 3 aylıkken foley kateter balonu ile kontrollü jejunostomi obstrüksiyon denendi ancak başarılı olunamadı. Olgu 5 aylıkken denenen dar jejunokolonik anastomoz uygulanması sonrası jejunum çapı yeterli genişliğe ulaşmadı. Pediatrik gastroenteroloji birimince 10 ay süreyle yüksek kalorili TPN ve kısmi enteral beslenme ile 8,1kg’a ulaşan olguya OBRP öncesi jejunum dilatasyonu amacıyla deneysel Çelik prosedürü uygulanması planlandı. Kontrollü barsak dilatasyonu sağlamak amacıyla jejunokolik anastomozun 4 cm distalindeki kolon segmentinin mezenterinden geçirilen elastik bir halka batın ön duvarından dışarı alınarak asıldı. Postoperatif dönemde elastik halkanın gerginliği kademeli olarak artırıldı. Aralıklı karın grafileriyle yeterli proksimal jejunum dilatasyonu oluştuğu görüldü. Yaklaşık 4 haftalık bir takip süresi ardından tekrarlanan laparotomide Çelik prosedürü girişimi öncesi 2,5 cm olan jejunum çapının 4cme genişlediği görüldü ve 8 cm jejunum ile 4cmlik kolon segmenti Bianchi tekniği uygulanarak 24cm’e uzatıldı. Ameliyat sonrası hastanın enteral beslenme toleransında tedrici gelişme görüldü, kilo alımı sağlandı ve TPN bağımlılığı %20’e gerileyerek ayaktan takibi mümkün oldu. Olgu halihazırda 2. Çelik prosedürü denemesiyle duodenuma OBRP planıyla izlenmektedir.

Uygulaması kolay, fizyolojik ve geri çevrilebilir olması ile eşdeğerlerinden farklı olan Çelik prosedürü OBRP öncesi barsak adaptasyonunun sağlanmasında güvenli ve etkili bir yöntemdir.
