Is the Position of the Arcus Aorta Effective in Choosing the Thoracotomy Direction in Esophageal Atresia Repair?

AİM: Esophageal atresia(EA) is a congenital anomaly seen in 3,000-4,500 live births. The most concomitant anomaly is cardiac. It is important to perform the echocardiography (ECO) before the operation. Cardiac anomalies are the most important prognostic factors. The direction of Arcus aorta gives an idea in choosing the direction of thoracotomy. In this study, 3 patients with left arcus aorta were presented to emphasize the importance of preoperative ECO and the comparison of thoracotomy side preference.

MATERİAL AND METHODS: The clinical characteristics of 3 patients with EA were retrospectively analyzed.

RESULTS: Patients who were followed up with EA were followed up in our clinic. Preoperative ECO images showed left atrial aorta in 3 patients. Patients' definitive surgeries were planned with right thoracotomy. The 3 patients surgery was started with right thoracotomy. In one case, the operation was continued with left thoracotomy, in 2 cases operation was continued with right thoracotomy and definitive operation was completed. All of the patients were discharged from our clinic with healing.

CONCLUSION: Cardiac anomalies associated with EA are important in the clinical follow-up and the position of arcus aorta is important in surgical technique. Despite the operation plan according to preoperative ECO, unexpected situations may be encountered. Although the operation is technically challenged by the surgeon in cases where the right arcus aorta is incidentally detected, the operation can be successfully completed in the experienced hands. On these tables, definitive repair can be tried before deciding to continue the operation with left thoracotomy. If the position of aorta cause technical difficulty, support from the cardiovascular surgeon can be obtained. The purpose of presenting these case is to emphasize the importance of careful examination in the preoperative period and to emphasize that the surgical intervention should be continued with the same incision in these incidental tables.

Özofagus Atrezisi Onarımında Arkus Aortanın Pozisyonu Torakotomi Yönü Seçiminde Etkili mi?

AMAÇ: Özofagus atrezisi (ÖA), 3,000-4,500 canlı doğumda bir görülen konjenital bir anomalidir. ÖA’lı bebeklerde en sık görülen kardiyak anomalilerdir. Hastalara ekokardiyografinin (EKO) operasyon öncesi nde yapılması önemlidir. Kardiyak anomaliler en önemli prognostik faktörlerdendir. Arkus aortanın yönü operasyona torakotomi yönü seçiminde fikir vermektedir. Bu çalışmada kliniğimizde sol arkus aorta saptanan 3 hasta incelenmiş olup ; preoperatif yapılan EKO'nun önemi vurgulanması ve torakotomi yön tercihinin karşılaştırılması amacıyla sunulmuştur.

GEREÇ YÖNTEM : ÖA tanılı 3 hastanın klinik özellikleri geriye dönük incelendi.

BULGULAR : ÖA tanısı ile takip edilen hastalar kliniğimizde izleme alındı. 3 hastada preoperatif yapılan EKO'da sol arkus aorta saptandı. Hastaların definitif cerrahileri sağ torakotomi ile planlandı. 3 hastaya sağ torakotomi ile operasyona başlandı. Eksplorasyonda sağ dönüşlü arkus aorta saptanan 1 olguda sol torakotomi ile operasyona devam edilerek definitif cerrrahi tamamlandı. 2 olguda operasyona sağ torakotomi ile devam edilerek operasyon tamamlandı. Hastaların hepsi kliniğimizden şifa ile taburcu edildi.

SONUÇ: ÖA'da eşlik eden kardiyak anomaliler hastanın klinik izleminde prognostik açıdan önemli olup, arkus aortanın dönüş yönü ise cerrahi teknik açısından önemlidir. Preoperatif EKO'ya göre operasyon planı yapılmasına rağmen, beklenmedik durumlar ile karşılaşılabilir. İnsidental olarak sağ arkus aorta saptanan hastalarda operasyon teknik açıdan cerrahı zorlasa da, deneyimli ellerde operasyon başarı ile tamamlanabilmektedir. Peroperatif karşılaşılan bu tablolarda operasyona sol torakotomi ile devam etmeye karar vermeden önce definitif onarım denenebilir. Aortanın pozisyonu nedeni ile teknik açıdan güçlük yaşanması durumunda kalp damar cerrahisi ekibinden destek alınabilir. Bu olguları sunmamızdaki amaç, hastanın preoperatif dönemde kardiyak açıdan dikkatli bir şekilde tetkik edilmesinin hem klinik seyir hem de cerrahi seyir açısından önemini vurgulamak ve insidental olarak yaşanan bu tablolarda cerrahi işleme öncelikli aynı kesi ile devam edilmesi gerektiğini vurgulamaktır.
