Stereomicroscopic Stone Analysis: Study of 334 Patients Data

Background:

Stone analysis has great importance in the management of the urinary stone diseases in children, since metabolic disorders in children with stone disease are more prominent than adults. Methods such as X-ray diffraction, spectroscopy and steromicroscopy are known to be reliable for stone analysis.

Method:

The results of cases, whose urinary stone analysis were made in our clinic between the years 2010-2017, were evaluated. Stone analysis was performed by stereomicroscopic method as well as chemical evaluation. Hydrochloric acid for oxalate and carbonate, ammonium molybdate for ammonia and phosphate, murexite for urate were used to determinate compounds of the stones. The findings were also assessed by the Kruskal-Wallis test for changes over the years.

Results:

495 stone samples belonging to 334 patients were analyzed in the eight-year period. In accordance with the literature, the most frequently detected stone is isolated oxalate stone in 125 cases (37%). This is followed by multicomponent stones (13%) containing oxalate, urate and phosphate, and cystine stones (10%). There was no difference between the groups according to years (p = 0,147). When the stones were regrouped for urate composition in order to evaluate the grain-weighted diet, which is one of the important reasons for stone disease in our country, the difference was found significant (p = 0,048), but it was observed by posthoc tests that the only difference is the change in 2011, not a significant trend compared to the years.

Conclusion:

When the results of the stone analysis made in our clinic were evaluated, it was found to be consistent with the literature and no change was found in stone components according to years.

Giriş

Çocuklarda taş hastalığında metabolik bozukluklar erişkine kıyasla daha ön planda olduğundan taş analizi hastalığın yönetiminde büyük önem taşımaktadır. Taş analizi için X ışını difraksiyon, spektroskopi ve steromikroskopi gibi yöntemlerin güvenilir olduğu bilinmektedir.

Hastalar-Yöntem

Kliniğimizde 2010-2017 yılları arasında üriner taş analizi yapılan olguların sonuçları taranmıştır. Taş analizi stereomikroskopik yöntemin yanı sıra kimyasal değerlendirme ile yapılmıştır. Kimyasal analizde oksalat ve karbonat bileşenlerinin tayini için hidroklorik asit, amonyak ve fosfat için amonyum moliptat, ürat için müreksit testleri kullanılmıştır. Bulgular ayrıca yıllara göre değişim açısından Kruskal-Wallis testiyle değerlendirilmiştir.

Bulgular

Sekiz yıllık dönemde 334 hastaya ait 495 taş örneğine analiz yapılmıştır. Literatürle uyumlu olarak en sık saptanan taş 125 olguda izole oksalat taşıdır (%37). Bunu oksalat, ürat ve fosfat içeren çok bileşenli taşlar (%13) ve sistin taşları (%10) izlemektedir. Yıllara göre yapılan değerlendirmede gruplar arasında fark saptanmamıştır (p=0,147). Ülkemizde taş hastalığı için önemli sebeplerden biri olan tahıl ağırlıklı beslenmeyi değerlendirmek için taşlar ürat bileşeni içermesi açısından yeniden gruplandırılarak değerlendirildiğinde fark saptanmış (p=0,048) ancak posthoc testleriyle farkı yaratanın yalnız 2011 yılındaki değişiklik olduğu görülmüş, yıllara göre belirgin bir eğilim gözlenmemiştir.

Sonuç

Kliniğimizde yapılan taş analizi sonuçları değerlendirildiğinde literatürle uyumlu bulunmuş, taş bileşenlerinde yıllara göre bir değişim saptanmamıştır.
