A rare complication of ventriculoperitoneal shunt: Shunt migration from the appendiceal stump

Aim: To present a rare complication, a case with ventriculoperitoneal (VP) shunt entering the colon from the appendiceal stump.

Case: A 15-year-old male patient with VP shunt, who has been followed up by the neurosurgery team due to hydrocephalus, presented with the complaints of fever, vomiting and headache for a week. The patient had a history of appendectomy in our clinic a year ago due to perforated appendicitis. During the appendectomy, there was no problem with the VP shunt, the appendical stump has been embedded during the operation. Shunt scintigraphy was performed on the patient who was evaluated by neurosurgery, considering the lower end dysfunction of the shunt, and the shunt was visualized to be in the transverse colon. During the laparatomy, it was observed that a capsule has formed around the shunt and the shunt is advancing from appendiceal stump into the colon, but that there was no evidence of peritonitis in the abdomen. The shunt was removed from the cecum and the wall of the cecum was repaired and the shunt end was taken out of the abdomen. The patient, who postoperatively progressed without problem, underwent shunt revision on the 5th day and a ventriculopleural shunt was inserted. The patient is still being followed without any problem.

Conclusion: Intestinal perforation due to VP shunt is rarely reported in the literature. However, our case is the first case in which the shunt entered the colon from the stump after appendectomy. The treatment was successfully performed with the neurosurgery team.

Nadir bir ventriküloperitoneal şant komplikasyonu: Apendiks güdüğünden şant migrasyonu

Amaç: Nadir bir komplikasyon olan apendiks güdüğünden kolon içine giren ventriküloperitoneal (VP) şantlı bir olgunun sunulmasıdır.

Olgu: Hidrosefali nedeniyle beyin cerrahisi ekibi tarafından takip edilen ve VP şantı olan 15 yaşında erkek olgu bir haftadır süren ateş, kusma ve başağrısı şikayeti ile başvurdu. Olgunun 1 yıl önce kliniğimizde perfore apendisit nedeniyle apendektomi öyküsü mevcuttu. Apendektomi sırasında VP şanta ait bir sorun yoktu, apendiks güdüğü operasyon sırasında gömülmüş idi. Beyin cerrahisi tarafından değerlendirilen olguda şant alt uç disfonksiyonu düşünülerek şant sintigrafisi yapıldı ve şantın transvers kolon içinde olduğu görüldü. Laparatomide şantın etrafında bir kapsül oluşturarak apendiks güdüğünden kolon içine ilerlediği, ancak batında peritonit bulgusu olmadığı görüldü. Şant çekumdan çıkarılarak çekum duvarı tamir edildi ve şant ucu batın dışına alındı. Postoperatif sorunsuz seyreden olguda 5.gün şant revizyonu yapılarak ventriküloplevral şant takıldı. Olgu halen sorunsuz takip edilmektedir.

Sonuç: Literatürde VP şanta bağlı intestinal perforasyon nadir de olsa bildirilmektedir. Ancak olgumuz apendektomi sonrası güdükten şantın kolona girdiği ilk olgudur. Tedavisi beyin cerrahisi ekibi ile başarılı şekilde gerçekleştirilmiştir.
