Robotic excision in thoracic outlet mass

Aim: Paragangliomas that are derived from adrenal medulla and extra-renal chromaffin cells are rarely seen in thorax. Since they release catecholamine, they may cause some serious problems such as tachycardia, tachypnea, hyperhidrosis and paleness. The aim of this study is to report a case that was operated due to thoracic outlet mass.

Case: The child followed up for the thalassemia minor consulted to other center with complaints of excessive sweating and swelling in the right arm. In the physical examination, swelling in the right arm, anisocoria, miosis, sweating deficiency in the right half of the forehead (anhidrosis) was observed. Radiological examination, revealed opacity in C8-T2 level. It was thought to be neurogenic origin. Robotic surgery was planned with the referral diagnosis of Stellat ganglioneuroma, Schwannomm. Before operation, no problem was observed in the vital signs of the child; but during the operation arterial blood pressure went up to 280/200. After mass excision, hemodynamics of the case returned to a stable situation. In histopathological examination, it was reported as paraganglioma. After surgery, the patient who has just minimal ptosis, was discharged on the forth postoperative day. In the four months follow-up, no problem was detected.

Discussion: Among the masses in the thorax, paragangilomas were observed rarely. For that reason, preoperative algorithms are not clearly defined. However, during operation, serious problems could be observed. Robotic surgery is advantageous, especially for the masses that are hard to reach, owing to the localization.

Torasik outlet yerleşimli kitlede robotik eksizyon

Amaç: Adrenal medulla ve sürrenal dışı kromaffin hücrelerden köken alan paragangliomalara toraksta ender olarak rastlanır. Katekolamin salgılamaları nedeni ile taşikardi, takipne hiperhidrozis, solukluk gibi çeşitli sıkıntılara yol açabilirler. Torasik outlet yerleşimli kitle nedeni ile ameliyat edilen on yaşındaki olgunun sunulması amaçlandı.

Olgu: Talasemi minör nedeni ile takipli çocuk aşırı terleme ve sağ kolda şişlik yakınması ile dış merkeze başvurmuş. Yapılan fizik incelemede sağ kolda şişlik, anizokori, miyozis, alnın sağ yarısında terleyememe (anhidrozis) görüldü. Radyolojik incelemede C8-T2 seviyelerinde nörojenik kökenli olduğu düşünülen kitle, (Stellat ganglionörom, Schwannom?) ön tanısıyla robotik cerrahi planlandı. İşlem öncesi vital bulgularında problem saptanmayan çocuğun işlem sırasında tansiyon arteriyel 280/200’e kadar yükseldi. Kitle eksizyonu sonrası olgunun hemodinamisi stabil hale geldi. Histopatolojik inceleme paraganglioma olarak raporlandı. Ameliyat sonrası sadece minimal pitozisi olan hasta postoperatif dördüncü gün taburcu edildi. Dört aylık takibinde problem saptanmadı.

Yorum: Toraksda yer alan kitleler arasında paragangliomalar daha az akla gelir bu nedenle peroperatif algoritmalar net oturmamıştır. Ancak işlem sırasında ciddi problemler gözlenebilir. Özellikle ulaşılması zor bölgelerdeki kitlelerde robotik cerrahi avantajlıdır.
