Repair of circumcision and inguinal pathologies with single incision

Aim:

In this study, we retrospectively analyzed the inguinal pathologies we performed from the circumcision incision and wanted to show the applicability of the method.

Method:

Circumcision incision was used for the surgeries of 50 patients with inguinal pathology. In all cases, after the excision of the foreskin and mucosa, the inguinal region was reached by dissecting to the root of the penis in the direction of pathology. The spermatic cord and its vessels were easily removed from the incision and dissection was made till the preperitoneal fat tissue. Inguinal hernia, hydrocele and cord cyst surgeries were performed from this area. For the undescended testes, dissection after the root of the penis continued to the external ring and the testis was found and released and the testis was fixed from the inside.

Results:

The mean age of 50 patients who were retrospectively screened between the dates 01.10.2018-01.07.2019 and included in the study was 18,2±1.2 (1-60) months. The diagnoses of the patients were undescended testis (4 patients), cord cyst (5 patients), hydrocele (10 patients), inguinal hernia (31 patients) and all of them also had circumcision requests. Palpable testes in undescended testes are included in the study. Only two cases (undescended testis and inguinal hernia) developed no complications other than hematoma.

Conclusion:

Inguinal pathologies from the circumcision incision can be considered as an alternative method both in terms of aesthetics and ease of application.

Tek insizyonla sünnet ve inguinal patolojilerin onarımı

Amaç:

Bu çalışmada sünnet insizyonundan yaptığımız inguinal patolojileri geriye dönük olarak analiz ettik ve yöntemin uygulanabilirliğini göstermek istedik.

Yöntem:

Elli adet inguinal patolojili hastanın cerrahileri sünnet insizyonundan gerçekleştirilmiştir. Vakaların tümünde sünnet derisi ve mukozanın fazlası eksize edildikten sonra patolojinin olduğu tarafa doğru penis köküne kadar disseksiyon yapılarak inguinal bölgeye ulaşıldı. Kord ve elemanları rahatlıkla insizyon dışına alınarak preperitoneal yağ dokusuna kadar disseksiyon yapıldı. İnguinal herni, hidrosel ve kordon kisti cerrahileri bu alandan gerçekleştirildi. İnmemiş testislerde ise penis kökünden sonra disseksiyona external ringe kadar devam edilerek testis bulunup serbestleştilerek testis içerden tespit edildi.

Bulgular:

01.10.2018-01.07.2019 tarihleri arasında geriye dönük olarak taranıp çalışmaya alınan 50 hastanın yaş ortalaması 18,2±1.2 (1-60) aydı. Hastaların tanıları inmemiş testis (4 hasta), kordon kisti (5 hasta), hidrosel (10 hasta), inguinal herni (31 hasta) olup hepsinin aynı zamanda sünnet istemleri de vardı. İnmemiş testislerde ele gelen testisler çalışmaya alındı. Yalnızca 2 vakada (inmemiş testis ve inguinal herni) gelişen hematom dışında komplikasyon olmadı.

Sonuç:

İnguinal patolojilerin sünnet insizyonundan yapılması hem estetik açıdan hem de uygulanım kolaylığı yönüyle alternatif bir yöntem olarak düşünülebilir kanaatindeyiz.
