Perinatal testicular torsions

Introduction: Diagnosis and treatment management of perinatal testicular torsion (PTT) are difficult due to its rare occurrence and uncertain etiology. In this study, our aim was to present 21 years of experience on PTT.

Methods: The records of patients between the ages of 0-16 who had been treated for TT between 1998-2019 were retrospectively examined. The demographic data, complaints at admission, side and degree of torsion, radiologic characteristics, diagnosis and treatment methods of patients who underwent follow-up and treatment for PTT were recorded.

Results: Ten of the 46 patients who were operated for testicular torsion had PTT. Median age of diagnosis was 1 day (min:1, max:12). Of the patients, 50% had PTT on the right testicle, 30% had PTT on left testicle, and 20% had bilateral PTT. Median week of birth was 38 (min:35, max:40). It was found that 60% were born through caesarean section, while 40% were born through vaginal delivery. Median birth weight was 3300 (min:1730, max:4000) grams. All patients except for one had discoloration in the scrotum. During the examinations of the patients, twisted testicles were palpated as being harder than normal. In 70% of the cases, blood flow could not be visualised in colour doppler ultrasonography (CDUS). Hydrocele was present on the opposite side of the affected testicle in 60% of the cases. Orchiectomy+fixation on the opposite testicle were performed on 10 of the 12 twisted testicles, while detorsion+fixation were performed on one of the testicles. One case with intrauterine TT had physical examination and radiologic findings that suggested a necrotic testicle, and this case was followed up. The median degrees of torsion was 540 (min:360, max:720).

Conclusion: Scrotal discoloration and hardened testicles are important findings for the diagnosis of PTT. Physical examination and CDUS are the primary methods used for diagnosis. Most of the testicles had developed necrosis by the time of diagnosis and were eventually lost. The presence of hydrocele in the opposite scrotum can be a warning with regards to TT. Due to the potential of contralateral torsion, fixation must be performed on the opposite testicle during the same session/intervention.

Giriş: Perinatal testis torsiyonu (PTT) doğumdan önce veya hemen sonrasında görülen torsiyondur. Nadir görülmesi ve etiyolojisindeki belirsizlik nedeniyle teşhisi ve tedavi yönetimi zordur. Bu çalışmada 21 yıllık PTT deneyimlerinin sunulması amaçlandı.

Materyal ve Metod: 1998–2019 yılları arasında testis torsiyonu nedeniyle tedavi edilen, 0–16 yaş aralığındaki hastaların kayıtları geriye dönük olarak incelendi. PTT nedeniyle takip ve tedavisi yapılanların; demografik özellikleri, başvuru şikayetleri, torsiyon tarafı ve derecesi, radyolojik özellikleri, tanı ve tedavi yöntemleri ile histopatolojik bulguları kayıt edildi.

Bulgular: Testis torsiyonu nedeniyle ameliyat edilen 46 hastanın 10’u (%21.7) PTT idi. Median tanı yaşı 1 (min:1, max:12) gündü. Hastaların %50’si sağ, %30’u sol ve %20’si bilateral PTT idi. Median doğum haftası 38 (min:35, max:40) idi. Hastaların %60’ı sezaryen, %40’ı vajinal yol ile doğurtulmuştu. Median doğum ağırlığı 3300 (min:1730, max:4000) gramdı. Biri hariç hepsinde skrotumda renk değişikliği mevcuttu. Muayenelerinde torsiyone testisler normalden sert olarak palpe edilmişti. Olguların %70’inde renkli doppler ultrasonografilerinde (RDUS) kan akımının alınmadığı raporlandı. Olguların %60’ında karşı tarafta hidrosel mevcuttu. Torsiyone olan 12 testisin 10’una orşiektomi + karşı testise fiksasyon, birine ise detorsiyon + fiksasyon işlemi yapıldı. İntrauterin TT olan, fizik muayene ve radyolojik bulguları nekrotik testis olduğunu düşündüren bir olgu ise aile ile görüşülerek takip edildi. Median torsiyon derecesi 540 (min:360, max:720) idi. Hastaların %70’inde saat yönünde, %30’unda ise saat yönünün tersine torsiyon mevcuttu. Hastaların %90’ında ekstravajinal, %10’unda ise intravajinal torsiyon mevcuttu.

Sonuç: Skrotal renk değişimi ve sert testis, PTT tanısında önemli bulgulardır. Fizik muayene ve RDUS tanı koydurucudur. Testislerin çoğunda tanı konulduğunda nekroz oluşmuş ve kaybedilmişlerdir. Karşı taraf skrotumda hidrosel varlığı TT açısından uyarıcı olabilir. Kontralateral torsiyon potansiyeli nedeni ile karşı taraf testise aynı seansta fiksasyon yapılmalıdır.
