A rare case of trichobezoar: Rapunzel syndrome

The masses that are formed in the gastrointestinal tract by foreign substances such as plant fibers, hair, drugs are taken orally are called bezoars. The clinical condition caused by a bezoar located in the stomach extending to the duodenum and jejunum is dedicated to Rapunzel's long-haired tale of the Grimm brothers and is called Rapunzel syndrome. In this article, we aimed to discuss the rare case of Rapunzel syndrome with two case report.

Case1: A 16-year-old female patient presented with complaints of abdominal pain and weakening. The patient had a history of chewing hair and there was no known disease or surgical history. On physical examination, a palpated mass extending from the left upper quadrant to the right lower quadrant was observed. Direct radiographs revealed no fundus air and the shadow of the transverse colon gas spreading to the pelvis. Endoscopy showed trichobezoar mass starting from the cardia region and not allowing the endoscope to pass through, and computed tomography showed that the mass filled the entire stomach and duodenum.

Case 2: A 14-year-old female patient presented with abdominal pain and abdominal mass. Computed tomography showed a mass filling the entire stomach and duodenum. Laparotomy was planned due to the mass filling the endoscopy.

As a result, trichobezoars are treated more surgically because the hairs they contain are not suitable for disintegration or enzymatic dissolution and cannot usually be treated by endoscopic methods. It is important to obtain an anamnesis for bezoar in the masses of the stomach and duodenum.

Nadir rastlanan bir trikobezoar olgusu: Rapunzel sendromu

Ağızdan alınan bitki lifleri, kıl, ilaçlar gibi yabancı maddelerin gastrointestinal sistemde oluşturduğu kitlelere bezoar denir. Midede yerleşen bir bezoarın, duodenum ve jejunuma kadar uzayarak neden olduğu klinik durum, Grimm kardeşlerin masalındaki uzun saçlı Rapunzel’e ithaf edilerek, Rapunzel sendromu olarak adlandırılır.

Bu yazıda, nadir rastlanan Rapunzel sendromunu bir olgu eşliğinde tartışmayı amaçladık.

Olgu1: 16 bir yaşında kadın hasta karın bölgesinde yaygın ağrı ve zayıflama şikayeti ile başvurdu. Anamnezinde saç çiğneme alışkanlığı olan hastanın bilinen bir hastalık veya cerrahi öyküsü yoktu. Fizik muayenede sol üst kadrandan sağ alt kadrana doğru uzanan ve palpasyonda hareketli olan bir kitle gözlendi. Direkt grafilerde mide fundus havasının olmadığı ve transvers kolon gazına ait gölgenin pelvise doğru yaylandığı dikkati çekti . Endoskopide kardiya bölgesinden başlayıp endoskopun geçişine izin vermeyen trikobezoar kitlesi ve bilgisayarlı tomografide bu kitlenin tüm mide ve duodenumu doldurduğu görüldü.

Olgu 2: 14 yaş kadın hasta karın ağrısı ve abdominal kitle ile başvurdu. Bilgisayarlı tomografide bu kitlenin tüm mide ve duodenumu dolduran kitle görüldü. Endoskipide ideyi dolduran kitle nedeniyle laparatomi planlandı.

Sonuç olarak trikobezoarlar, içerdikleri kıllar parçalanmaya veya enzimatik yolla çözülmeye uygun olmadıkları ve genellikle endoskopik yöntemlerle tedavi edilemedikleri için daha çok cerrahi yolla tedavi edilirler. Mide ve duedonumu dolduran kitlelerde bezoara yönelik anamnezin alınması önemlidir.
