Coexistence of Hypertrophic Pyloric Stenosis and Long-gap Isolated Esophageal Atresia

Introduction: Esophageal atresia is a congenital abnormality which may be seen together with additional systemic pathologies. Therefore, possible additional anomalies should be revealed and preparations should be made for the sufficient therapy before the surgical operation. The coexistense of hypertrophic pyloric stenosis and esophageal atresia is a rare condition. In the literature, also, there are only a few publications written about this rare coexistential condition. In this study, an incidental hypertrophic pyloric stenosis case that was found out during the gastric pull-up surgery for long gap isolated esophageal atresia , is aimed to be presented.

Case Report: 50 day-old girl with isolated esophageal atresia was applied to our clinic for gastric pull-up surgery, which was planned to be replaced with her currently existing gastrostomy that was done in the newborn period. In the operation, it was seen that the stomach is larger than expected because of hypertrophic pyloric stenosis. Pyloromyotomy was performed before the gastric pull-up. Then, the stomach was pulled upwards from the mediastinum and anastomosed with the proximal esophagus. No complication was developed during or after the surgery.

Conclusion: In case, patients with esophageal atresia and nutrition problems, when the stricture of the anastomosis line or gastrooesophageal reflux are excluded, it should be kept in mind that the reason may be hypertrophic pyloric stenosis.

Hipertrofik Pilor Stenozu ve Uzun Aralıklı Özofagus Atrezisinin Birlikteliği

Giriş : Özofagus atrezisi (ÖA), ek sistemik patolojilerle birlikte olabilen doğumsal bir anomalidir. Ameliyat öncesi dönemde olabilecek ek anomaliler araştırılmalı ve uygun tedavisi için hazırlık yapılmalıdır. Hipertrofik pilorik stenozu(HPS)’nun özofagus atrezisi ile birlikteği nadir görülmektedir. Literatürde, bu birlikteliğinin sunulduğu az sayıda yayın bulunmaktadır. Bu olguda, uzun aralıklı izole ÖA nedeniyle gastrik pull-up cerrahisi sırasında rastlantısal olarak görülen HPS olgusu sunulması amaçlanmaktadır.

Olgu Raporu : İzoleözofagus atrezisi nedeniyle yenidoğan döneminde gastrostomi uygulanan 50 günlük kız hasta gastrik pull-up ameliyatı için başvurdu. Ameliyat sırasında HPS nedeni ile midenin beklenenden daha büyük olduğu görüldü. Gastrik pull-up öncesi piloromiyotomi yapıldı. Daha sonra mide mediastenden yukarı çekilerek proksimal özofagusa anastomoz yapıldı. Ameliyat sırasında ve sonrasında herhangi bir komplikasyon gelişmedi.

Sonuç : Özofagus atrezisi nedeniyle takip edilen hastalarda beslenme bozukluğu görülmesi halindeanastomoz hattındaki darlık ve gastroözofageal reflü mevcut değilse bu hastalarda HPS olabileceği akılda tutulmalıdır.
