Obstruction due to intussusception in a patient being followed-up for benign systemic hemangiomatosis

Aim: Hemangiomas rarely involve the digestive system. A patient applying with obstruction and known multiple hemangiomas is presented.

Case Report: An 8.5-year-old male patient was consulted for abdominal pain. The case story revealed that the patient was born with a giant mass on his back and multiple hemangiomas in his body. It was found that he was unresponsive to treatment consisting of propranolol and steroids, underwent surgery at the age of 16 months and 5 years due to a mass on his back and histopathology was compatible with cavernous hemangioma. Transfusion was required many times because of low hemoglobin. Recent endoscopy of upper and lower digestive system revealed multiple hemangiomas on the mucosal face. Systemic sirolimus treatment was recommended to the patient, but the family refused treatment because of possible side effects. The patient applying with fatigue, paleness, abdominal pain and vomiting at his last admission and was hospitalized for transfusion because his hemoglobin value was 3.4 g / dl. Upon continuous abdominal pain, the patient was consulted with anamnesis and intussusception was contemplated. On ultrasonography, ileoileal intussusception was detected in the 6 cm segment of the left middle quadrant and hydrostatic reduction was not successful. During surgery, while ileoileal intussusception was reducted, resection and anastamosis were performed to the 25 cm necrotic looking segment. In all small and large intestines, diffuse masses compatible with hemangioma involving serosa and mesentery were observed. In the histopathological examination of the resection material, thrombosed vascular structures developing in a 1 cm diameter to the lumen in one area and a hemangioma was seen positive immunoreactivity with CD 31 was detected. The patients’ early post-operative period was uneventful and was reevaluated for further medical treatment.

Conclusion: Digestive system hemangiomas are very rare. Disseminated intestinal involvement is more rare. In particular, management of patients who remain unresponsive to standard and relatively innocent medical treatment options is difficult to conduct in both medical and surgical aspects. In addition to gastrointestinal bleeding and frequent transfusion requirements, recurrent surgical interventions may be required for reasons such as intussusception and perforation. Ultimately, short bowel syndrome may develop. Considering these possibilities, close monitoring of patients is extremely important.

Benign Sistemik Hemanjiomatozis İle İzlenen Hastada İnvajinasyona Bağlı Obstrüksiyon

Amaç: Hemanjiomalar nadiren sindirim sistemini tutabilirler. Bilinen çoklu hemanjiomları olan ve obstrüksiyonla başvuran bir hasta sunuldu.

Olgu Sunumu: 8,5 yaşında erkek hasta karın ağrısı nedeniyle danışıldı. Öyküde, hastanın sırtında dev bir kitle ve vücudunda çok sayıda hemanjiomla doğduğu öğrenildi. Propranolol ve steroidlerden oluşan tedaviye yanıtsız kaldığı, sırtındaki kitle nedeniyle 16 aylık ve 5 yaşındayken ameliyat olduğu ve histopatolojinin kavernöz hemanjiomla uyumlu olduğu anlaşıldı. Hemoglobin düşüklüğü nedeniyle çok kere transfüzyon gereksinimi olmuştu. Yakın tarihte yapılan üst ve alt sindirim sistemi endoskopisinde mukozal yüzde çok sayıda hemanjiom saptanmıştı. Hastaya sistemik sirolimus tedavisi önerilmiş, ancak olası yan etkileri nedeniyle aile tedaviyi kabul etmemişti. Son başvurusunda, halsizlik, solukluk, karın ağrısı ve kusma ile getirilen hastanın, hemoglobin değeri 3,4 g/dl olduğu için transfüzyon amacıyla yatırılmıştı. Karın ağrısının devam etmesi üzerine, danışılan hastada anamnez eşliğinde, invajinasyon düşünüldü. Ultrasonografide batın sol orta kadranda 6 cm’lik segmentte ileoileal invajinasyon saptandı ve hidrostatik redüksiyon başarılı olmadı. Ameliyatta ileoileal invajinasyon redükte edilerek 25 cm’lik nekrotik görünümlü segmente rezeksiyon ve anastamoz yapıldı. Tüm ince ve kalın barsakta, seroza ve mezenteri tutan hemanjiomla uyumlu yaygın kitleler görüldü. Rezeksiyon materyalinin histopatolojik incelemesinde bir alanda 1cm çapında lümene doğru gelişim gösteren tromboze vasküler yapıların oluşturduğu, CD 31 ile pozitif immunreaktivite görülen hemanjiom saptandı. Erken ameliyat sonrası dönemi sorunsuz olan hasta ileri medikal tedavi için tekrar değerlendirmeye alındı.

Sonuç: Sindirim sistemi hemanjiomları çok nadir görülür. Yaygın barsak tutulumu ise çok daha nadirdir. Özellikle, standart ve nispeten masum sayılabilecek medikal tedavi seçeneklerine yanıtsız kalan olguların yönetilmesi hem tıbbi hem de cerrahi açıdan güçtür. Sindirim sistemi kanaması ve sık transfüzyon gereksinimine ek olarak, invajinasyon ve perforasyon gibi nedenlerle tekrarlayan cerrahi girişimler gerekebilir. En nihayetinde kısa barsak sendromu gelişimi söz konusu olabilir. Bu olasılıklar göz önüne alınarak, hastaların yakın takibi son derece önemlidir.
