Method of introducing the intestinal contents from the proximal bowel extension to distal bowel extension in 2 patients experiencing anastomosis transition problem with Intestinal atresia;

Introduction/Objective: The aim of this study was to share developments during the jejunostomy follow-up of the 2 patients operated with the diagnosis of intestinal atresia and whose reanastomosis was impossible due to proximal-distal diameter difference.

Materials/Method: Two cases administered enterostomy are reported.The primary repair postoperative (PO) care after of the patients were total parenteral nutrition (TPN), nasogastric (NG) decompression, warm serum physiological/day rectal lavation. jejunostomy PO follow-up consisted of NG decompression, TPN support, oral feeding, starting proximal jejunum 5cc/h from the distal jejunum via closed system and increasing.Administration duration of the proximal contents from the distal extension was 5 weeks in case 1 and 6 weeks in case 2. At the end of this period, distal stomagraphy with contrast was viewed and stoma closure surgery was planned.

Findings: Patient 1: It was 35 weeks/2790 grams male, type 3B Jejunal atresia. While 4 days old, PO was performed; reanastomosis on the PO day 33, dejejunostomy on the day 90 at the end of anastomosis transition realization for the second time.Oral feeding was started on the day 5 of the jejunostomy and deproximal bowel contents were administrated to the distal bowel extension on the day 7 day of the PO. On the day 130. the jejunostomy of the patient was closed.

Patient 2: It was 33-week male twin, 2040 grams, type 3B jejunal atresia. While 2 days old, primary repair was performed and jejunostomy on PO day 41 due to an anastomosis transition problem. On PO day 7, jejunostomy, oral feeding was started and proximal bowel contents were administrated to the distal bowel extension. On the day 93. the jejunostomy of the patient was closed.

Conclusion: It was observed that the intestinal contents taken from the proximal bowel through a closed system to the distal bowel length shortened the PO follow-up period, saved patients from long-term TPN dependence and improved TPN complications.

Anastomoz geçiş problemi yaşayan intestinal atrezili 2 olguda; proksimal barsak uzantısından gelen intestinal içeriğin distal barsak uzantısına verilmesi yöntemi

Giriş/Amaç: İntestinal atrezi tanısıyla opere edilmiş, proksimal-distal çap farkı nedeniyle reanastomozu mümkün olmayan 2 olgunun, jejunostomili takip sürecindeki gelişmelerin paylaşılması amaçlandı.

Gereç/Yöntem: Enterostomi yapılmış iki olgu sunulmaktadır. Hastaların, primer onarım postoperatif (PO) bakımı; total parenteral beslenme (TPN), nazogastrik (NG) dekompresyon, ılık serum fizyolojik / gün rektal lavman, jejunostomi PO takipleri ise, NG dekompresyon, TPN desteği, oral beslenme, kapalı sistem ile proksimal jejunum içeriğinin distal jejunumdan 5cc/h başlanıp arttırılarak verilmesi şeklindeydi. Proksimal içeriğin distal uzantıdan verilme süresi 1.olguda 5, 2. olguda 6 hafta idi. Bu süre sonunda, kontrastlı distal stoma grafisi çekildi ve stoma kapatılma cerrahisi planlandı.

Bulgular: 1. Olgu: 35 hafta / 2790 gr, erkek, tip 3B Jejunal atreziydi. Hastaya 4 günlük iken, primer onarım, PO 33. gün reanastomoz, ikinci kez anastomoz geçişinin gerçekleşmemesi sonucu, bebeğin 90. gününde jejunostomi yapıldı. Jejunostominin PO 5. günü, oral beslenmeye, PO 7. günü proksimal barsak içeriği distal barsak uzantısına verilmeye başlandı. Olgunun 130. gününde jejunostomisi kapatıldı.

2. Olgu: 33 haftalık, ikiz eşi 2040 gr, erkek, tip 3B jejunal atreziydi. Hastaya 2 günlük iken primer onarım, PO 41. gün anastomoz geçiş problemi nedeniyle jejunostomi yapıldı. Jejunostominin PO 7. günü oral beslenmeye ve proksimal barsak içeriği distal barsak uzantısına verilmeye başlandı. Olgunun 93. gününde jejunostomisi kapatıldı.

Sonuç: Proksimal ve distal çap farkı nedeniyle, reanostomozu mümkün olmayan olgularda, proksimal barsaktan alınan intestinal içeriğin, distal barsak uzantısına kapalı bir sistem ile verilmesinin, PO takip sürecini kısalttığı, uzun dönem TPN bağımlılığından kurtardığı ve TPN komplikasyonlarında iyileşme sağladığı görüldü.
