New danger for children: neodymium magnet

Aim: Foreign body ingestion is one of the common causes of emergency department admissions. Swallowing more than one magnet or magnet together with a metal object can cause intestinal compression necrosis, obstruction and perforation. In this study, two cases with multiple magnet ingestion history are presented.


Case 1: A 5-year-old male patient was admitted to the emergency department with a history of swallowing two magnets and abdominal pain. Abdominal x-ray showed air-fluid levels and two spherical magnets in jejunal loops. When laparotomy was performed in the patient with acute abdominal findings, compression-related perforations were observed in two jejunal segments at a distance of 30 cm and primary repaired. Oral feeding was started on the 4th postoperative day and the patient was discharged on the 6th day.

Case 2: A 18 months old female patient presented to the outpatient clinic with multiple magnet swallowing. Plain abdominal X-ray of the patient who had no
active complaints showed six spherical magnets adhered to each other at epigastric region and one at lower right quadrant. Due to the
immobility of the magnets in the epigastric region in direct graphical series laparotomy was performed. In surgical exploration,
perforations were observed over the transverse colon and duodenum, then magnets were removed and primary repair was performed.
Oral feeding was started on the 5th postoperative day and the patient was discharged on the 7th day.


Conclusion: In recent years, the use of high-powered neodymium-iron-boron magnets in toys production has led to the frequent occurrence of complications
associated with magnet ingestion. Even if the patient is asymptomatic, close clinical follow-up and early intervention may prevent complications.

Amaç: Yabancı cisim yutma, acil servis başvurularının yaygın nedenlerinden biridir. Birden fazla mıknatıs ya da mıknatıs ile metal bir cismin beraber yutulması intestinal bası nekrozu, obstruksiyon ve perforasyona neden olabilir. Bu çalışmada çoklu mıknatıs yutma öyküsü olan iki olgu sunulmuştur.

Olgu 1: 5 yaşında erkek hasta acil servise 2 adet mıknatıs yutma öyküsü ve karın ağrısı şikayeti ile başvurdu. Çekilen ayakta batın grafisinde jejunal anslarda hava-sıvı seviyeleri ve iki adet küre şeklinde mıknatıs izlendi. Akut karın bulguları olan hastaya laparotomi yapıldığında 30 cm mesafedeki iki jejunal segmentte basıya bağlı perforasyonlar izlenip primer onarıldı. Postoperatif 4. gün oral gıda başlanan hasta 6. gün taburcu edildi.

Olgu 2: 18 aylık kız hasta çoklu mıknatıs yutma nedeniyle polikliniğe başvurdu. Aktif şikayeti olmayan hastanın direk grafisinde epigastrik bölgede birbirine yapışık 6 ve sağ alt kadranda 1 adet küre şeklinde mıknatıs izlendi. Direk grafi serilerinde epigastrik bölgedeki mıknatısların hareketsiz kalmaları üzerine yapılan laparotomide duodenum 1. kıtası ile transvers kolonda perforasyon izlenip, mıknatıslar çıkarılıp primer onarım yapıldı. Postoperatif 5. gün oral gıda başlanan hasta 7. gün taburcu edildi.

Sonuç: Son yıllarda yüksek güçlü neodmiyum-demir-bor mıknatısların oyuncak üretiminde kullanılmaya başlanması, mıknatıs yutmaya bağlı komplikasyonların sık görülmesine neden olmaktadır. Hasta asemptomatik olsa dahi yakın klinik izlem ve erken girişimin komplikasyonları önleyeceğini düşünmekteyiz.
