SOLUTION TO FUNCTIONAL OBSTRUCTION AND SHORT BOWEL SYNDROME: LATERAL T-STOMA IN HIGH DIAMETER DIFFERENCE OF JEJUNOILEAL ATRESIA

Introduction/Aim: This study is a preliminary report of the efficacy of lateral T-stoma, which developed as a solution for anastomosis dysfunction and SBS were evaluated.

Material and Methods: The primary pathologies and results of cases who underwent lateral T-stoma between July 2017-2019 were evaluated retrospectively. The lateral T-stoma was created similarly in all cases by tapering with a 3-4 cm trouser leg incision on the antimesenteric edge proximal to anastomosis.

Results: 1.case with meconium pseudocysts secondary to intrauterine volvulus had Type-3aJIA at 50.cm to treitz, and end-oblique anastomosis was performed. In the 15thday due to abdominal distension, lateral T-stoma was created due to 5/1 difference in anastomosis diameter. Patient was discharged on the 13thday. The stoma was closed at 10-month.

End-to-end anastomosis and primary lateral T-stoma were created with a diameter difference of 5/1 at 60.cm from Treitz in 2.case with Type-2JIA . The stoma was closed at 10-month. In the 3.case with antenatal diagnosis, end-to-side anastomosis was performed due to Type2JIA at 70.cm from Treitz. Closed perforation was detected in the anastomosis line at 36thday; the diameter difference was 6/1 in anastomosis, lateral T-stoma was created. In 5thmonth stoma was closed. Fourth case with antenatal diagnosis was intubated due to low APGAR and surfactant was administered. Intrauterine volvulus, meconium pseudocyst and intestinal perforation were detected 50.cm from Treitz, anastomosis and lateral T-stoma with 5/1 diameter were performed; patient by nasogastric feeding was lost with sepsis on 44thday without extubation.

Primary repair and end-stoma were performed with gastroschisis/intestinal perforation in Fifth case. In exploration due to the high flow stomal discharge/weight loss, ileo-colic anastomosis (in 60.cm distal to Treitz) between end-stoma and 30cm unused blind left colon were detected on 68th day, and end-to-end with lateral T-stoma due to diameter difference of 3/1; patient was discharged on 90thday.

Conclusion: Although the number of cases is not enough, according to our preliminary results creation of lateral T-stoma proximal to the anastomosis in JIA with a large diameter difference, is an effective treatment for functional obstruction/SBS.

FONKSİYONEL OBSTRUKSİYON VE KISA BARSAK SENDROMUNA ÇÖZÜM: ÇAP FARKI YÜKSEK JEJUNOİLEAL ATREZİLERDE LATERAL T-STOMA

Giriş/Amaç: Jejunoileal atrezi(JIA)’lerde çap farkı, adinamik ansın fonksiyonel obstrüksiyonu, anastomoz kaçağı veya kısa barsak sendromu(KBS) en önemli komplikasyonlardır. Bu çalışma, anastomoz disfonksiyonu ve KBS’na çözüm olarak geliştirdiğimiz lateral T-stomanın ilk sonuçlarıdır.

Materyal Metod: Temmuz 2017-2019 arasında lateral T-stoma yapılan olgular retrospektif olarak değerlendirildi. (Anastomozun proksimalinde antimezenterik kenardan 3-4cm’lik pantolon bacağı kesisi ile tapering yapılarak lateral T-stoma, tüm olgularda benzer şekilde oluşturuldu).

Bulgular: İntrauterin volvulusa sekonder mekonyum pseudokistili 1.olguda operasyonda Treitz’den 50.cm’deTip3a JIA, distalde 35cm ileum mevcuttu, 5/1 çap farkıyla uç-oblik anastomoz yapıldı. Batın distansiyonu, pasaj olmaması nedeniyle 15.gün yapılan reoperasyonda intakt olan anastomozun çap farkından dolayı çalışmadığı görüldü; proksimalden lateral T-stoma oluşturuldu, 13.gün taburcu edildi. 10.ay stoma kapatıldı, iki yaşında halen sorunsuzdur.

Tip1 JIA’lı 2.olguda Treitzden 60.cm’de 5/1 çap farkıyla uç-uç anastomozla birlikte primer lateral T-stoma oluşturuldu, 14. gün taburcu edildi. 10.ay stoma kapatıldı, iki yaşında halen sorunsuzdur.

Antenatal tanılı 3.olguda Treitzden 70.cm’deki Tip2 JIA nedeniyle uç-yan anastomoz yapıldı. 36.gün beslenememe, batın distansiyonu sebebiyle reoperasyonda anastomoz hattında kapalı perforasyon saptandı; çap farkı 6/1 olduğundan lateral T-stoma açıldı, 5.ay stoma kapatıldı, halen sorunsuzdur.

Antenatal tanılı 4.olgu düşük APGAR/pulmoner hipertansiyon nedeniyle entübe edildi, surfaktan uygulandı. Intrauterin volvulus, mekonyum psödokisti, Treitz’den 50.cm’de perforasyon nedeniyle 5/1 çap farkıyla uç-uç anastomoz, primer lateral T-stoma yapıldı. Beslenmeyi tolere eden, stoma/rektal yoldan gaita yapan olgu ekstübe edilemeden 44.gün sepsis/pulmoner hipertansiyon/kalp yetmezliğinden kaybedildi.

Gastroşizisli 5.olguda, barsak perforasyon yerinden uç stoma ile primer onarım yapıldı. Yüksek debili stomadan kilo kaybı nedeniyle 68.gün reoperasyonda 60.cm’deki uç ileum ve distalinde 30cm’lik kullanılmamış sol kolon saptandı; 3/1 çap farkıyla uç-uç anastomoz, lateral T-stoma yapıldı, 10.gün pasaj başladı, sepsis tedavisi bitiminde 90.gün taburcu edildi, takipleri sorunsuzdur.

Sonuç: Olgu sayısı yeterli olmasa da ilk sonuçlarımıza göre, çap farkının yüksek olduğu atrezilerde anastomozla birlikte lateral T-stoma oluşturulması, fonksiyonel obstrüksiyon/KBS için etkili bir tedavi yöntemi olabilir; primer yapılabildiği gibi fonksiyonel obstruksiyonlarda sekonder de uygulanabilir.
