A RARE CASE OF FAMILIAL SUPERIOR MESENTERIC ARTERY SYNDROME

Introduction: Superior mesenteric artery (SMA) syndrome is a rare cause of epigastric discomfort and vomiting, caused by the narrowed angle between the superior mesenteric artery and the aorta. SMAS is more frequent in female and often diagnosed during late teenagers and early adulthood.

Case Report: A 16-year-old female patient presented with abdominal distension, vomiting and weight loss. She had lost 12 kg in 2 months. She was cachectic and suffering from extreme malnutrition with a body mass index of 8.6 kg/m2. The patient’s sister had also undergone surgery for SMA Syndrome 2 years ago. Abdominal CT and CT angiography showed dilated stomach and narrowed aortomesenteric angle. Duodenum was compressed between SMA and aorta. The patient was kept under parenteral nutrition for a week before surgery in order to optimise her nutritional status. Intraoperatively, it was observed that both the first and second parts of the duodenum were dilated and the third part was clamped by the superior mesenteric artery. A side-to-side duodenojejunostomy was performed and a nasojejunal feeding tube was placed. The patient was discharged a week after the surgery and the post-operative follow up was uneventful.

Conclusion: Patients with SMA syndrome often present with non-specific complaints and thus, diagnosis is challenging. In most cases, SMA syndrome is considered only after evaluating and excluding various gastrointestinal diseases. In case of suspicion, diagnosis can easily be confirmed by a CT scan with contrast. Family history may be a risk factor as it was in our case. Duodenojejunostomy should be considered the treatment of choice for these patients as it offers effective outcome on treatment of the syndrome, based on recent studies.

NADİR BİR AİLEVİ SUPERIOR MEZENTERİK ARTER SENDROMU OLGUSU

Giriş: Superior mezenterik arter (SMA) sendromu, epigastrik hassasiyet ve kusmanın superior mezenterik arter ve aort arasındaki açının daralmasından kaynaklanan, nadir görülen bir sebebidir. SMA sendromu kadınlarda daha sık görülüp sıklıkla geç adölesan veya erken erişkin dönemde tanı alır.

Olgu Sunumu: 16 yaşında kadın hasta abdominal distansiyon, kusma ve kilo kaybı yakınmalarıyla başvurdu. Son 2 ayda 12 kilo kaybetmişti. Kaşektik görünümde ve 8.6 kg/m2 beden kitle endeksiyle ileri derece malnütrisyondan muzdaripti. Hastanın kız kardeşi de 2 sene önce SMA sendromu nedeniyle opere olmuştu. Batın BT ve BT anjiyografi incelemeleri dilate halde mideyi ve daralmış aortomezenterik açıyı gösteriyordu. Duodenum SMA ve aorta arasında sıkışmıştı. Hasta, operasyondan önce nütrisyonel dengesini optimize edebilmek için parenteral nütrisyonda kaldı. Operasyon sırasında duodenumun birinci ve ikinci kıtalarının dilate olduğu; üçüncü kıtanın ise superior mezenterik arter tarafından sıkıştırıldığı izlendi. Uç uca duodenojejunostomi yapıldı ve nazojejunal beslenme sondası yerleştirildi. Hasta operasyondan bir hafta sonra taburcu edildi. Takiplerinde sıkıntı gözlenmedi.

Sonuç: SMA sendromu genellikle nonspesifik yakınmalarla başvurur; bu sebepten tanı koymak zordur. Çoğu durumda SMA sendromu ihtimali ancak diğer çeşitli gastrointestinal hastalıkların değerlendirilmesi ve elenmesinden sonra akla gelir. Şüphe varlığında tanı kontrastlı BT incelemeyle kolayca konabilir. Vakamızda da olduğu gibi aile öyküsü bir risk faktörü olabilir. Güncel çalışmalarda etkili sonuç verdiği gözlenen duodenojejunostomi bu hastalarda tedavi yöntemi olarak düşünülmelidir.
