Percutan Endoscopic gastrostomy tube placement in children: Is preoperative pH-metry necessary?

Purpose: Percutaneous endoscopic gastrostomy (PEG) feeding is a frequently preferred method with short surgical time and rare complication rate. The aim of study is to evaluate the cases who underwent PEG in terms of preoperative gastroesophageal reflux and to compare the results after PEG.

Material and methods: We reviewed 78 children who underwent PEG catheterization between January 2008 and January 2018 retrospectively. Complications such as symptoms and aspiration pneumonia were questioned to cases for the excretion of gastroesophageal reflux prior to the PEG decision. Suspected cases were evaluated by preoperative pH-meter. Group which PEG performed after symptom questionnaire was compared with group which PEG performed after reflux negative pH-meter in terms of postoperative follow-up and complications.

Results: 45 of the cases were male (57.7%) and 33 were female (42.3%). The average age was 5.8 years. The mean time of feeding initiation was 28 ± 8 hours. PEG was performed with the cause of swelling dysfunction due to pathologies of the central nervous system in all cases. PEG was performed in 27 cases, in 51 cases PEG was performed after 24-hour pH meter and gastroesophageal reflux study. Complications of 24-hour pH meter group are stoma revision in nine cases, stoma granulation in five cases, colonic fistulization in one case and in one case,on the basis of the symptoms of GER, a 24-hour ph meter was performed and GER is detected. This case underwent nissen fundoplication. The complications of PEG group after symptom questioning are stoma revision in four cases, stoma granulation in three cases. There was no statistical difference between the two groups in terms of age, sex, postoperative follow-up and complication.

Discussion: Before the PEG procedure, reflux questionnaire must be done. PH-meter examination should be kept in cases with an additional anomaly or anatomical disorder with a high risk of surgical correction. Although it has been suggested in the literature that PEG may increase the incidence of reflux, our study group was noticed that the incidence of reflux after PEG was low. As a conclusion, PEG is a reliable and effective method that can be applied to children who need enteral nutrition due to nutritional difficulty.

Amaç: Perkütan endoskopik gastrostomi (PEG) ile besleme kısa cerrahi süresi ve nadir komplikasyon oranı ile sık tercih edilen bir yöntemdir. Çalışmamızda PEG yapılan olguların preoperatif gastroözofageal reflü yönünden değerlendirilmesi ve PEG sonrası sonuçlarının karşılaştırılması amaçlandı.

Gereç ve yöntem: Ocak 2008 – Ocak 2018 tarihleri arasında hastanemizde perkutan endoskopik gastrostomi kateteri takılan 78 çocuk olgu geriye dönük olarak incelendi. Olgulara PEG kararı öncesinde gastroözofageal reflünün ekartasyonu için semptom ve aspirasyon pnömonisi gibi komplikasyonlar yönünden sorgulama yapıldı. Şüpheli olgular preoperatif pH-metre ile değerlendirildi. Semptom sorgulaması sonrası PEG yapılan grup ile reflü yönünden negatif pH-metre sonrası PEG yapılan grup postoperatif izlem ve komplikasyonlar yönünden karşılaştırıldı.

Sonuçlar: Olguların 45’ i erkek (% 57,7), 33’ ü kız (% 42,3) idi. Yaş ortalaması 5,8 yıl idi. Hastaların beslemeye başlama süreleri ortalama 28 ± 8 saat idi. Tüm olgulara santral sinir sistemini patolojilerine bağlı yutma disfonksiyonu nedeni ile PEG işlemi uygulandı. Olguların 27’ sine PEG, 51’ine 24 saatlik pH metre ile gastroözofageal reflü araştırması sonrası PEG yapıldı. 24 saatlik ph metre yapılan grubun komplikasyonları; dokuz olguda stoma revizyonu, beş olguda stoma çevresinde granülasyon, bir olguda kolona fistülizasyonu, bir olguda ise GÖR semptomları olması üzerine 24 saatlik Ph metre yapılıp GÖR tespit edilmesidir. Bu olguya nissen fundoplikasyonu yapıldı. Semptom sorgulaması sonrası PEG yapılan grubun komplikasyonları ise dört olguda stoma revizyonu,üç olguda stoma çevresinde granülasyon oluşumudur. İki grup arasında yaş, cinsiyet, postoperatif izlem ve komplikasyon açısından istatiksel fark yoktu.

Tartışma: PEG öncesi reflü sorgulaması mutlaka yapılmalıdır. PH-metre incelemesi ise cerrahi düzeltim riski yüksek ek anomali ya da anatomik bozukluğu olan olgulara saklanmalıdır. Literatürde PEG’in reflü insidansını arttırabileceği ön görülse de çalışma grubumuzda PEG sonrası reflü insidansının beklenenden düşük bulunması dikkati çekmiştir. Sonuç olarak PEG, beslenme güçlüğü nedeni ile enteral nütrisyona ihtiyaç duyan çocuklarda uygulanabilecek güvenilir ve etkin yöntemdir.
