Full Thickness Skin Graft of Inguinal Region for Repairing Penile Skin Loses

Introduction: Penile skin loss is a common complication after circumcision or hypospadias surgery. Different methods have been described for the repair of this complication. In this presentation, we present the results and complications of 3 cases with penile skin loss in our clinic after full repair with a full-thickness skin graft harvested from the inguinal area.

Case 1: One-year-old boy was circumcised at home by a Syrian surgeon.The skin of the penis is completely excised.The patient had been referred to us after he received local treatment cause of infection after circumcision. Penile skin was repaired with full-thickness skin graft harvested from both inguinal areas. There were no complications.

Case 2: A 17-year-old boy was operated three times due to hypospadias.Coronal hypospadias, penile skin loss and penil curvature secondary to penil skin loss were present. The patient’s hypospadias were repaired. The penis was repaired by taking a full-thickness skin graft from the right inguinal area. In the early period after the surgery light apertures were seen in the wound due to the sportive activity. A straight penis was achieved at the erection.

Case 3: A 23-year-old male patient was operated three times due to hypospadias.There were an external meathus at subcoronal level and loss of ventral penile skin which almost combine scrotum and glans. A full-thickness skin graft from the right inguinal area was placed on the ventral side of the penis. Penile skin was recovered without any complication. Hypertrophic scar occured at the site of inguinal graft. The patient is receiving local treatment.

Result: Inguinal area is a good source for full-thickness skin grafts for treatment of penile skin lesions.There may be tension and development of hypertrophic scar after extensive graft harvest. In such cases, it should be considered that grafts may be harvested from both inguinal areas.

Penil Cİlt Kayıplarında Kasıktan Alınan Tam Kat Cilt Grefti İle Onarım

Giriş/Amaç: Sünnet veya hipospadias cerrahileri sonrası komplikasyonlarda penis cildi kaybı sık görülen bir komplikasyondur. Bu kompikasyonun onarımı için farklı metodlar tarif edilmiştir. Kliniğimizde bu rahatsızlığa sahip 3 olgunun kasıktan alınan tam kat cilt grefti ile tamiri sonrası sonuçları ve komplikasyonları sunmayı amaçladık

Olgular: 1. 1 yaşında olgu evde suriyeli bir hekim tarafından sünnet edilmiş. Penis cildi tamamen eksize edilmiş. sonrasında gelişen enfeksiyon dış merkezde lokal olarak tedavi eidlmiş halde başvurdu. Her iki kasıktan alınan tam kat cil grefti ile penil cilt onarıldı. işlem sonrası komplikasyon olmadı.

2. 17 yaşında olgu hipospadias nedeni ile 3 kez opere edilmiş. Koronal hipospadias, peniste cilt kaybı ve buna bağlı eğrilik mevcuttu. Olgunun hipospadiası onarıldı. Cilt kaybı için sağ kasıktan tam kat cilt grefti alınarak penis distaline sarıldı. İşlem sonrası erken dönemde (10. gün) uyarılara rağmen spor yaptığı için kasıktaki yara yerinde hafif açılmalar görüldü. Sekonder iyileşme sağlandı. Mea olağan yerleşimde idi, ereksiyonda düz bir penis elde edildi.

3. 23 yaşında olgu hipospadias nedeni ile 2 kez opere edilmiş. subkoronal düzeyde mea ve neredeyse skrotuma birleşik bir glansı olacak şekilde ventral penil cilt kaybı mevcuttu. Penis deglove edildikten sonra kordi onarıldı, sağ kasıktan alınan tam kat cilt grefti penis ventraline yerleştirildi. Penil cilt sorunsuz iyileşti. Kasık greftinin alındığı yerde hipertrofik skar gelişti. Bu durum için lokal tedavi uygulanıyor.

Sonuç: Penil cilt kayıplarının tedavisinde tam kat cilt grefti için kasık iyi bir kaynaktır. Geniş greft alımında gerginlik oluşabileceği ve hipertrofik skar gelişebileceği bu tip ihtiyaçlarda her iki kasıktan da greft alınabileceği akılda tutulmalıdır.
