"Comparison of sleeve method and guillotine method in circumcision"

Circumcision is the surgical removal of skin that covers the glans. It is the oldest known in the history and still the most common operative in the world. Circumcision is made for medical, religious and traditional reasons. Circumcision with surgical methods (Sleeve method, Dorsal Slit and Excision, Guillotine method). The world health organization suggested that if the circumcision provided that it is done under favorable conditions. With the sleeve method preferred by most surgeons, the skin and mucosa are marked and incised to determine the incisional limitations, as can be done using special clamps (Mogen clamp, Gomco clamp and Plastibel device). The mucosa and skin between the incisions are excited all around. The remaining mucosa and skin are sutured together. In the guillotine method, the flat clamp is prepunched. In the meantime, you should be sure that the glans is at the bottom of the clamp. Otherwise serious injuries may occur.

We aimed to compare the two most widely used circumcision techniques in terms of the duration and complications of the operation.

Materials and Methods:

Mustafa Kemal University The records of 148 patients circumcised between December 2016 and January 2018 in the pediatric surgery clinic were retrospectively reviewed. Patients with hemophilia or any other bleeding diathesis were not included in the study. 73 of the patients were circumcised by the same surgeon using the guillotine method. Seventy-five patients were surgically circumcised by another surgeon. All of the patients were circumcised under local anesthesia and sedation. patients' operation time, complications after circumcision were compared. Advantages and disadvantages of the methods were evaluated according to each other.

Result:

The techniques used in circumcision may vary from surgery to surgery. Eventually they all reach the same result. The important thing is to show the necessary care to protect the complications.

Giriş:

Sünnet glansı kaplayan derinin cerrahi olarak çıkarılması işlemidir. Tarihte bilinen en eski ve dünyada halen en yaygın yapılan operasyondur. Sünnet, medikal, dini ve geleneksel nedenlerle yapılmaktadır. Dünya sağlık örgütü uygun şartlarda yapılması koşuluyla sünneti onaylamaktadır. Sünnet cerrahi yöntemlerle(Sleeve yöntemi, Dorsal Slit ve eksizyon, Giyotin yöntemi ) yapılabildiği gibi, özel klempler kullanılarak(Mogen klemp, Gomco klemp ve Plastibel cihazı)ta yapılabilir. Pek çok cerrah tarafından tercih edilen sleeve yönteminde cilt ve mukoza insizyon sınırlarını belirlemek için işaretlenir ve insize edilir. İnsizyonlar arasındaki mukoza ve cilt çepeçevre eksize edilir. Kalan mukoza ve cilt birbirine sütüre edilir. Giyotin yönteminde düz klemp prepisyuma yerleştirilir. Bu sırada glansın klempin alt seviyesinde kaldığından emin olunmalıdır. Yoksa ciddi yaralanmalar görülebilir.

Biz bu çalışmada en çok kullanılan iki farklı sünnet tekniğini operasyonun süresi ve komplikasyonları açısından karşılaştımayı amaçladık.

Materyal ve Metod:

Mustafa Kemal Üniversitesi Çocuk cerrahisi kliniğinde aralık 2016-ocak 2018 arasında sünnet edilen 148 hastanın kayıtları retrospektif olarak incelendi. Hemofili veya başka herhangi kanama diyatezi olan hastalar çalışmaya dahil edilmedi. Hastaların 73'ü aynı cerrah tarafından giyotin yöntemi ile sünnet edildi. 75 hasta başka bir cerrah tarafından sleeve cerrahi yöntemi ile sünnet edildi. Hastaların tamamı lokal anestezi ve sedasyon altında sünnet oldu.  hastalar operasyon zamanı, sünnet sonrası komplikasyonlar açısından karşılaştırıldı. Yöntemlerin birbirlerine göre avantaj ve dezavantajları değerlendirildi.

Sonuç: 

Sünnette kullanılan teknikler cerrahtan cerraha değişebilir. Neticede hepsi aynı sonuca ulaşır. Önemli olan komplikasyonlardan korunmak için gereken özenin gösterilmesidir.
