Ultrathin ureterorenoscopy (Mini-URS) treatment of urinary tract stones of two years old and younger children

Aim: In this study, we aimed to present two years old and younger patients who were treated with urinary stones by mini-URS.

Method: We reviewed the data of 43 patients who underwent URS and laser lithotripsy for ureter and pelvis stone between January 2017 and March 2018. The results of 19 patients who were 2 years old were included in the study. A mini URS (Richard Wolf GmBH, Knittlingen, Germany) of 4.5F / 6F for ureteroscopy, a Holmium: YAG laser device (Litho Quanta System, Italy) for laser lithotripsy and a 150μ thick probe were used. The data were compared in terms of procedure characteristics, stone-freeness, and complication rates.

Results: Nine of the cases were male (47%) and 10 were female (44%). The mean age of the patients was 1.92 ± 0.53 years (0.5-2.5 years). The mean stone size was 8.34 ± 2.06 mm (5.5-14 mm). Twelve of the stones were in the pelvis (63%) and 7 were in the distal ureter (37%). Eighteen (95%) patients had different grades of hydronephrosis. Six patients with distal ureteral stones and one patient with pelvic stones did not require pre-stenting before the procedure, whereas 12 (63%) patients had pre-stenting because we could not reach the stone. We performed lithotripsy after an average of 21.33 ± 15.13 days (7-58 days). In ten (52%) patients, calcified residual stones were detected with an average size of 5.22 ± 2.27 mm (2-8 mm). When five millimeters of large stones were accepted as residues, 14 patients (74%) were completely free of stone. Hydronephrosis regressed in all patients at follow up. Twelve (57%) patients required post-stenting. Complete stone removal was obtained after the first operation in 14 (74%) patients and after 2 operations in 4 (26%) patients. The mean duration of operation was 78.15 ± 39.48 min (20-150 min). No complication developed during the procedure. The mean duration of hospitalization was 6.36 ± 3.02 days (2-14 days).

Conclusion: We believe that we can treat ureter and pelvic stones effectively and safely in patients who do not have a high stone burden in 2 years old and younger with the mini URS-LL once the double j catheter has been installed if necessary.

İki yaş ve altında üriner sistem taşlarının ultrathin üreterorenoskopi (Mini-URS) ile tedavisi

Giriş ve Amaç:Bu çalışmada mini-URS kullanarak tedavi ettiğimiz 2 yaş ve altındaki üriner sistem taşı olan hastaların sonuçlarını değerlendirmek amaçlanmıştır.

Gereç-Yöntem:Ocak 2017-Mart 2018 tarihleri arasında kliniğimizde üreter ve pelvis taşı nedeni ile URS ve laser litotripsi uygulanan 43 hastanın verileri geriye dönük olarak incelendi. Çalışmaya 2 yaş ve altı 19 hastanın sonuçları dahil edildi. Üreteroskopi için 4.5F/6F’lik mini URS (Richard Wolf GmBH, Knittlingen, Germany), lazer litotripsi için Holmium:YAG lazer cihazı (Litho Quanta System, VA, Italy) ve 150μ kalınlığında prob kullanıldı. Veriler, işlem özellikleri, taşsızlık ve komplikasyon oranları açısından karşılaştırıldı.

Bulgular:Olguların 9u erkek (%47), 10u kız çocuktu (%44). Yaş ortalamaları 1.92±0.53 yıl (0.5-2.5 yıl) olarak bulundu. Ortalama taş boyutu 8.34±2.06 mm (5.5-14 mm) idi. Taşların 12 tanesi pelviste (%63), 7 tanesi distal üreterdeydi (%37). Onsekiz (%95) hastada değişik derecelerde hidronefroz mevcuttu. Distal üreter taşı olan 6 hastaya ve pelvis taşı olan 1 hastaya işlem öncesi çift j kateter takılma ihtiyacı olmazken, 12 (%63) hastada taşa ulaşılamadığı için çift j kateteri kondu. Bu hastalara ortalama 21.33±15.13 gün (7-58 gün) sonra litotripsi uygulandı. On (%52) hastada ortalama boyutları 5.22±2.27 mm (2-8 mm) olan kalisiyel rezidü taşlar tespit edildi. Beş milimetreden büyük taşlar rezidü olarak kabul edildiğinde, 14 hastada (%74) tam taşsızlık sağlandı. Takiplerde tüm hastaların hidronefrozları geriledi. Onbir hastaya (57) işlem sonrası çift j kateteri kondu. İlk seansta tam taşsızlık hastaların 14ünde (%74) sağlanırken, 4 hastada (%26) ikinci seans sonucunda sağlanabildi. Ortalama ameliyat süresi 78.15±39.48 dk (20-150 dk) idi. Hiç bir vakada işlem esnasında komplikasyon gelişmedi. Ortalama hastanede kalma süresi 6.36±3.02 gün (2-14 gün) idi.

Sonuç:Gerektiği takdirde işlem öncesi çift j kateteri ile sağlanan pasif dilatasyon sonrası mini URS-LL ile 2 yaş ve altında taş yükü çok olmayan hastaların üreter ve pelvis taşlarının etkin ve güvenli bir şekilde tedavi edilebileceğini düşünmekteyiz.
