Gastrostomy site problems in ichthyosis patient: a case report

Introduction: Enteral nutrition is provided through gastrostomy in many patients with insufficient or impossible oral intake. In this study, the problems were evaluated in the gastrostomy site care in a patient diagnosed with ichthyosis.

Case: A 5-year-old girl was diagnosed with motor mental retardation, epilepsy and ichthyosis vulgaris. The patient had tracheostomy three years ago due to respiratory problems. Laparoscopic nissen fundoplication and gastrostomy were planned for gastroesophageal reflux disease and swallowing problem. Postoperatively, there was skin and gastric mucosa dissociation at the site of gastrostomy. It was thought that the problems of wound healing were due to the patient's ichthyosis. Intravenous antiacid therapy was given. Wound dressings were performed daily with magistral cream as recommended by Department of dermatology. The patient was fed with a nasojejunal tube which was placed endoscopically until the gastrostomy site healed. Wound healing was closely followed. Since gastrostomy site healed with care and dressings, patient was fed via gastrostomy, by withdrawing the nasojejunal tube.

Conclusion: Gastrostomy site care requires careful nursing care. Proper dressing, close follow-up and care are very important for patients fed by gastrostomy with wound healing problems due to skin disease.     

İktiyozisli hastada gastrostomi giriş yeri sorunları: bir olgu sunumu

Giriş: Oral alımı yetersiz ya da mümkün olmayan birçok hastada enteral beslenme gastrostomi aracılığıyla sağlanmaktadır. Bu çalışmamızda iktiyozis tanılı bir hastada gastrostomi çevresi bakımında yaşanan sorunlar değerlendirildi.

Olgu: 5 yaşında kız hasta, motor mental retardasyon, epilepsi ve iktiyozis vulgaris tanısı ile takipliydi. Hastaya solunum problemleri nedeniyle üç yıl önce trakeostomi açılmıştı. Gastroözefajial reflü hastalığı ve oral alamama nedeni ile laparoskopik nissen fundoplikasyonu ve gastrostomi uygulanması planlandı. Postoperatif gastrostomi giriş yerinde, ciltte ve mide mukozasında açılma oldu. Yara iyileşmesindeki problemlerin hastanın iktiyozisine bağlı olduğu düşünüldü. Intravenöz antiasit tedavisi verildi. Günlük olarak dermatoloji bölümünce önerilen majistral kremler ile pansuman yapıldı. Gastrostomi giriş yerindeki açıklık düzelene kadar hasta endoskopik olarak yerleştirilen nazojejunal tüp ile beslendi. Yara iyileşmesi yakın takip edildi. Gastrostomi giriş yerindeki açıklık bakım ve pansumanlar ile gastrostomi tüpünü saracak açıklığa gelince nazojejunal tüp çekilerek hasta gastrostomi yoluyla beslendi.

Sonuç: Gastrostomi tüpü ve çevresi bakımı özenli bir hemşirelik bakımı gerektirmektedir. Gastrostomi ile beslenen cilt hastalığı nedeni ile yara iyileşmesi sorunu olan hastalarda uygun pansuman, yakın takip ve bakım çok önemlidir.
