Nursing Care According To Levine’s Conservation Model Of Child With Multiple Trauma Due To Abuse: Case Report

Aim:The aim of this study is to sample the use of Levine's Conservation Model (LCM) in nursing care for a child with multiple trauma due to abuse.
Method:According to LCM, humanbeing is a dynamic subject interacting with his environment.The equilibrium in which person is present is distorted in the case of illness. According to the model everything outside of the person forms its surroundings.Environment completes integrity of the individual. the aim of the model that has four basic concepts and three basic components is to support adaptation and to ensure integrity.With the prepared nursing care plan, systematic nursing care was provided to patient. In this study,the case was examined by researchers and nursing care was given according to LCM.
Case Study:A 3-year-old girl was beaten by her father and underwent laparotomy, cholecystectomy procedures in the outer center due to liver laseration and gallbladder leakage.After the operation,the leakage was continued and she was referred to our clinic.The patient is NPO monitored,there is serohemorrhagic reflux from the drains.In the patient with hypotensive and tachycardic illness,there was abdominal distension,subfebrile fever and vomiting.The patient who had respiratory distress was referred to the Child Intensive Care Unit for 15 days.After CICU she was started to fed orally and the drains drawn.When she was being monitored at the unit,she was exposed to psychological and verbal violence by her mother,and her hand was cut by her mother with a fruit knife.Then the mother burned her child's hand by pushing it in a cup of hot tea.The child who had second degree burn in her hand is taken to Pediatric Surgical Intensive Care Unit (PSICU) for protection.Psychiatry,Neurology consultations were requested and contacted with the Child Office of Police Department. In accordance with Levine's principles of conservation; pain, infection, bleeding, skin integrity deterioration,tissue integrity deterioration,delay in surgical healing,deterioration of body image,deterioration of parent-child bond,parental conflict,risky health behaviours,inability to cope nursing judgments were determined and nursing care was carried out. After spending 20 days in PSICU,18 days in clinic,16 days in child intensive care unit,delivered to the Social Services.
İstismara Bağlı Çoklu Travma Tanılı Çocuğun Levine Koruma Modeline Göre Hemşirelik Bakımı: Olgu Sunumu

Amaç: Bu çalışmanın amacı; istismara bağlı çoklu travma tanılı çocuğa yönelik hemşirelik bakımında Levine’in Koruma Modeli’nin kullanımını örneklemektir.
Yöntem: Levine’in Koruma Modeli’ne göre insan çevresiyle etkileşim halinde dinamik bir bütündür. İnsanın içinde bulunduğu denge hali hastalık durumunda bozulur. Modele göre insanın dışındaki her şey onun çevresini oluşturur. Çevre bireyin bütünlüğünü tamamlar. Dört temel kavramı ve üç temel bileşeni olan modelin amacı; adaptasyonu desteklemek bütünlük sağlamaktır. Hazırlanan hemşirelik bakım planı ile hastaya sistematik hemşirelik bakımının verilmesi sağlanmıştır. Bu çalışmada olgu; araştırmacılar tarafından incelenmiş, Levine’in Koruma Modeli’ne göre hemşirelik bakımı verilmiştir.
Olgu: 3 yaşındaki kız olgu babası tarafından darp edilmiş, karaciğer laserasyonu ve safra kaçağı nedeniyle dış merkezde laparatomi, kolesistektomi işlemleri uygulanmıştır. Ameliyat sonrası safra kaçaklarının devam etmesi üzerine kliniğimize sevk edilmiştir. NPO izlenen hastada, drenlerden serohemorajik geleni vardır. Hipotansif ve taşikardik izlenen hastada batında distansiyon, subfebril ateş ve kusma mevcuttur. Solunum sıkıntısı başlayan hasta 15 gün süre ile ÇYBÜ’ye sevk edilmiştir. ÇYBÜ dönüşü PO beslenmeye başlayan hastada; drenler çekilmiştir. Serviste izlenirken annesinin psikolojik ve sözel şiddet uyguladığı, çocuğun elini meyve bıçağı ile kestiği görülmüştür. Daha sonra anne çocuğunun elini yemek yemiyor diye sıcak çay bardağına bastırmak suretiyle yakmıştır. Elinde 2. derece yanık oluşan çocuk koruma amaçlı ÇCYBÜ’ye alınmıştır. Psikiyatri, Nöroloji konsültasyonları istenmiş ve Çocuk Şube Müdürlüğü ile iletişime geçilmiştir. Olguya, Levine’in koruma ilkeleri doğrultusunda; Ağrı, Enfeksiyon, Kanama, Deri Bütünlüğünde Bozulma, Doku Bütünlüğünde Bozulma, Cerrahi İyileşmede Gecikme, Beden İmgesinde Bozulma, Ebeveyn-Çocuk Bağında Bozulma, Ebeveynlik Rol Çatışması, Anksiyete, Riskli Sağlık Davranışları, Yalnızlık, Korku, Aile Başetmesinde Yetersizlik hemşirelik yargıları konulmuş ve hemşirelik bakımı yapılmıştır. 20 gün ÇCYBÜ’da, 18 gün serviste, 16 gün ÇYBÜ’de takip edilen hasta tedavisi tamamlanarak Sosyal Hizmet Birimi’ne teslim edilmiştir.
