Nursing Care in 2 cases with Jejunostomy

İntroduction/Aim:Enterostomies are widely used in the surgical management of congenital diseases such as necrotizing enterocolitis, Hirschprung, gastrointestinal system tumors, Volvolus, Malrotation and intestinal atresia. In this study, it is aimed to share nursing care of 2 patients with jejunostomy.

Method:During the patients' postoperative (PO) follow-up process, fluid volume imbalance(oral inability-insufficient oral intake and jejunostomy due to fluid loss), risk of infection (prolonged hospitalization process), deterioration of tissue integrity (due to of jejunostomy), inadequate nutrition (due to loss of jejunostomy), anxiety:Family (due to difference in baby's health status), lack of information:Family (due to not knowing stoma care) nursing diagnoses are planned.

Findings: Case 1, 35 weeks/2790 gr male, type 3B jejunal atresia, jejunostomy was performed while 90 days old. Oral feeding was started on the 5th day of the jejunostomy PO and proximal bowel contents were given to the distal bowel extension on the 7th day of the PO. Jejunostomy was closed on the 130th day and he was discharged on the 15th day of PO.

Case 2, 33-week-old, twin of 2040 gr male, type 3B jejunal atresia, jejunostomy was performed while 43 days old. On the 7th day of jejunostomy PO, oral feeding and proximal bowel contents were started to be given to distal bowel extension. On the 93th day of the patient, his jejunostomy was closed and he was discharged on the 36th day of PO.

Conclusion:The most important problem experienced in nursing care was the introduction of proximal jejunum to distal bowel extension and preservation of skin integrity around the stoma. Nutritional management was important in terms of the bowel adaptation. Guidance on the number of defecations and stool consistency in feeding facilitated the management of the adaptation problem. Protecting the patients with jejunostomy from complications, maintaining growth and development and caring to provide rehabilitation show that teamwork is vital in pediatric surgery.

Giriş/Amaç: Enterostomiler, nekrotizan enterekolit, Hirschprung, gastro intestinal sistem tümörleri, Volvolus, Malrotasyon ve intestinal atrezi gibi konjenital hastalıkların cerrahi yönetiminde yaygın olarak kullanılmaktadır. Bu çalışmada, Jejunostomili 2 olgunun çocuk cerrahisi yoğun bakım ünitesi hemşirelik bakımının paylaşılması amaçlanmıştır.

Gereç/Yöntem: Hastaların postoperatif (PO) takip sürecinde, sıvı volüm dengesizliği (oral alamama-yetersiz oral alım ve jejunostomi sıvı kaybına bağlı), enfeksiyon riski (uzamış hastane süreci ve jejunostomi içeriğinin çevre ve cilt ile temasına bağlı), doku bütünlüğünde bozulma (jejunostomi ve jejunum içeriğinin yakıcı özellikte olmasına bağlı), beslenmede yetersizlik (jejunostomi kaybına bağlı), anksiyete: Aile (bebeğin sağlık durumundaki farklılığa bağlı), bilgi eksikliği: Aile (stoma bakımını bilmemeye bağlı) hemşirelik tanılarına yönelik bakım planlanmıştır.

Bulgular: 1. Olgu, 35 hafta /2790 gr erkek, tip 3B Jejunal atrezi 90 günlük iken, jejunostomi yapıldı. Jejunostominin PO 5. günü, oral beslenmeye, PO 7. günü de proksimal barsak içeriği distal barsak uzantısına verilmeye başlandı. Hastanın 130. gününde Jejunostomisi kapatıldı ve PO 15. gün taburculuğu yapıldı.

2. Olgu, 33 haftalık, ikiz eşi 2040 gr erkek, tip 3B jejunal atrezi, 43 günlük iken, jejunostomi yapıldı. Jejunostominin PO 7. günü oral beslenmeye ve proksimal barsak içeriği distal barsak uzantısına verilmeye başlandı. Hastanın 93. gününde jejunostomisi kapatıldı ve PO 36. gün taburculuğu yapıldı.

Sonuç: Hemşirelik bakımında yaşanılan en önemli sıkıntı, proksimal jejunum içeriğinin distal barsak uzantısına verilmesi ve stoma çevresindeki cilt bütünlüğünün korunmasıydı. Beslenme yönetimi, barsak adaptasyonu açısından önemliydi. Beslenmede, defekasyon sayısının ve gaita kıvamının rehber alınması, adaptasyon probleminin yönetimini kolaylaştırdı. Jejunostomili hastaların komplikasyonlardan korunması, büyüme - gelişmenin sürdürülmesi ve rehabilitasyonunu sağlamaya yönelik bakımı; çocuk cerrahisinde ekip çalışmasının hayati öneme sahip olduğunu göstermektedir.
