Nursing Care According to Functional Health Patterns in a Child with Tracheoesophageal fistula : A Case Report

Objective: Esophageal atresia (EA) is the most common congenital anomaly of esophagus with an incidence of 1:3500 live-births approximately. The proximal segment of esophagus is blind ended and distal tracheaesophageal fistula is the most common type of EA. In half of the patients, different system anomalies associate EA. Although, EA is a surgical problem during newborn period, improvement in survival leads respiratory and gastrointestinal problems during adolescence and adulthood. It was aimed to draw attention to the effect of nursing care, which has been provided after the colon interposition of a child with esophagus atresia and tracheoesophageal atresia fistula, and its effect.

Method: The nursing practices regarding the treatment and care of a patient undergoing colon interposition in the pediatric surgery department of a university hospital are summarized. The case was assessed using Marjory Gordon’s Functional Health Patterns (FHP) Nursing Care Model and NANDA nursing diagnoses. The data were collected after informing the legal guardian of the patient and obtaining their written consents. The privacy principle and ethical principles were strictly obeyed in the case presentation.

Results: The patient, 3-year-old now, was born mature and taken to the intensive care unit because of mucus flow from the mouth and nose when the mother started lactating. Then, the patient was diagnosed with esophagus atresia and tracheoesophageal fistula. When 5-days-old, the patient was undergone gastrostomy. Hospitalized for colon interposition and assessed in parallel with Gordon’s Functional Health Patterns Nursing Care Model, the nursing diagnoses in S.B. were determined as “acute pain, excessive liquid volume, ineffective healthcare management, infection, hemorrhage risk, activity intolerance, hyperthermia, malnutrition: less than body’s need, risk of fall, risk of hemorrhage, and risk of deterioration in the skin integrity”. After implementing the nursing interventions addressing these diagnoses, the results were analyzed.

Conclusion: Gordon’s functional health patterns allow examining the necessities of a patient in 11 functional dimensions and providing extensive care. These dimensions ensure the collection of systematic and standardized information, as well as analyzing the subject from the perspective of nurse. Nursing care and patient education should be implemented in parallel with the nursing process in order to optimize the healthcare status of the patient.

Fonksiyonel Sağlık Örüntülerine Göre Trakeoözofageal Fistülü Olan Bir Çocuğun Hemşirelik Bakımı: Olgu Sunumu

Amaç: Özofagus atrezisi (ÖA) en sık rastlanan doğumsal özofagus anomalisi olup yaklaşık 3500 canlı doğumda bir görülür. Proksimal özofagusun kör sonlandığı, distal kısmın ise trakeaya fistül aracılığı ile birleştiği form en sık görülen ÖA tipidir. Olguların yarısında başka sistemlere ait eşlik eden anomali bulunmaktadır. ÖA onarımı yenidoğan döneminde cerrahi bir sorun olmanın yanı sıra, ÖA'li bebeklerin sağ kalım oranlarının artması, ergenlik ve erişkin dönemde solunumsal ve gastrointestinal sorunlarıda birlikte getirmektedir. Bu olgu sunumunda özafagus atrezisi ve trakeoözofageal atrezi fistülü olan bir çocuğun kolon interpozisyonu sonrası dönemde yapılan hemşirelik bakımına ve etkisine dikkat çekilmek istenmiştir.

Yöntem: Bir üniversite hastanesinin çocuk cerrahisi servisinde kolon interpozisyonu yapılan hastanın postoperatif dönemdeki tedavi ve bakımına yönelik hemşirelik uygulamaları özetlenmiştir. Olgu, Marjory Gordon’un Fonksiyonel Sağlık Örüntüleri (FSÖ) Hemşirelik Bakım Modeli ve NANDA hemşirelik tanıları ile değerlendirimiştir. Veriler hastanın yasal vasisine gerekli açıklamalar yapıldıktan ve yazılı onamları alındıktan sonra toplanmıştır. Olgu sunumunda gizlilik ilkesine uyulmuş ve etik ilkelere bağlı kalınmıştır.

Bulgular: Şu anda 3 yaşında olan hasta miadında doğmuş olup annesi ilk emzirmeye başladığında ağız ve burunda mukus gelenleri olması üzerine yoğun bakıma alınır ve hastaya özofagus atrezisi ve trakeoözofageal fistül tanısı konmuştur. 5 günlük iken hastaya gastrostomi açılmıştır. Kolon interpozisyonu için servise yatırılan ve postoperatif dönemde Gordon’un Fonksiyonel Sağlık Örüntüleri Hemşirelik Bakım Modeli doğrultusunda değerlendirilen S.B.’de “akut ağrı, sıvı volüm fazlalığı, etkisiz sağlık yönetimi, infeksiyon, hipertermi, aktivite intoleransı, dengesiz beslenme: beden gereksiniminden az, düşme riski, kanama riski, deri bütünlüğünde bozulma riski” hemşirelik tanıları belirlendi ve bu tanılara yönelik hemşirelik girişimleri uygulanarak sonuçlar değerlendirilmiştir.

Sonuç: Gordon’un fonksiyonel sağlık örüntüleri hastanın gereksinimlerini 11 fonksiyonel alanda ele alarak kapsamlı bakım verilmesini sağlar. Bu alanlar, sistematik ve standardize edilmiş bilgi toplamayı ve hemşirelik bakış açısı ile bilginin analiz edilmesini sağlamaktadır. Hemşirelik bakımı ve hasta eğitimi, hastanın sağlık durumunu optimal düzeye getirebilmek için hemşirelik süreci doğrultusunda uygulanmalıdır.
