ELECTROPHYSILOGIC EVALUATION OF CREMASTERIC REFLEX IN EXPERIMENTAL ORCHITIS

Aim: Orchitis is the most common cause of acute scrotum in children.Preserved cremasteric reflex (CR) in one the most important clinical finding in the differential diagnosis of testicular torsion (TT). Although clinical and electrophysiologic findings of TT is well described, electrophysiologic alterations in orchitis is not known. Therefore, an experimental study was performed to evalyuate the clinical and electrophysiological fetaures of CM in orchitis.

Materials and methods:  Eighteen Wistar albina rats included into study were allacated into three groups; control (CG), sham (SG) and orcihitis (OG). In CG, after anestheization with Ketamine hydrochloride upper tigh of the rat were stimulated to obtain CR clinically and needle probe evoked latancy of CR were recorded. Electrophysiologic evaluations were performed 24 hour after injection of 0.1 ml of 106cfu/ml E.coli (0:6 strain) in 1 ml of physiologic saline in right testicle in OG, and 1 ml of saline in SG. All testicle were sampled for confirming the orchitis after electrophysiologic evaluations.    

Results: CR was obtained in all of the rats in CG and in 83.3% and 66.6% in SG and OG respectively(p >0.05). The latancy of CR was higher in SG (15.5 ±1.2 ms) and OG  (15.1 ±0.9 ms) than CG (10.5 ±0.7 ms) without any siginifant difference (p>0.05). Histopathologic confirmation of orchitis was obtained in all testicle samples in OG and number of neutrophilis and total orchitis score was significantly higher in OG than other groups (p<0.05).

Conclusion: The results obtained form this study demosntartes that electrophysiologic parameters of CR can be altered in orchitis. Although, prolonged latancy of CR in orchitis did not impede the clinical presentation of reflex, the role of CR in the differential diganosis of TT becomes contraversial.     

 

 

DENEYSEL ORŞİT MODELİNDE KREMASTER REFLEKSİNİN ELEKTOROFİZYOLOJİK İNCELENMESİ

Amaç: Orşit çocuklarda görülen en sık akut skrotum nedenidir. Orşitte kremaster refleksinin (KR) korunmuş olması testis torsiyonu (TT) ayırcı tanısında önemli bir bulgudur. TT’da KR’nin klinik ve elektrofizyolojik özellikleri bilinmekle birlikte orşitte KR’inde ortaya çıkan elektrofizyolojik değişiklikler bilinmemektedir. Bu amaçla deneysel orşit modelinde KR’nin klinik ve elektrofizyolojik özelliklerini incelemek üzere deneysel bir çalışma yapılmıştır.

Gereç ve Yöntem: Çalışmaya dahil edilen 18 Wistar albino sıçan kontrol (KG), sham (SG) ve orşit (OG) olmak üzere üç gruba ayrılmıştır. KG’da intramuskuler Ketamin ile yapılan anestezi sonrası uyluk iç yüzü uyarılarak KR alınmış ve iğne elektrod ile uyarı verilerek KR latans değerleri elde edilmiştir. Elektrofizyolojik incelme, OG’da sağ testise 0.1 ml 106cfu/ml E.coli (0:6 strain) 1 ml SF içinde injekte edilerek orşit oluşturulduktan 24 saat sonra, SG’da ise sağ testise 1 ml SF injekte edilikten 24 saat sonra yapılmıştır. E.coli injeksiyonu orşit oluşumunu histopatolojik olarak kanıtlamak amacıyla elektrofizyolojik inceleme sonrası tüm testislerden biyopsisi yapılmıştır. 

Bulgular: KR,KG’deki sıçanların hepsinde, SG ve OG’daki sıçanların ise sırasıyla %83.3 ve %66.6’ında (n:4) alınmıştır (p >0.05). KR’nin latans değerleri SG (15.5 ±1.2 ms) ve OG’da (15.1 ±0.9 ms) KG’a (10.5 ±0.7 ms) oranla uzamış ancak istatistiksel olarak anlamlı bulunmamıştır (p>0.05). Histopatolojik olarak orşit oluşumu OG’daki tüm testis örneklerinde tespit edilmiş ve PMNL sayıları ile toplam orşit skoru diğer gruplara göre OG’de anlamlı olarak artmış bulunmuştur (p<0.05).

Sonuç: Elde edilen bulgular orşitte KR’in  elektrofizyolojik parametrelerinin etkilenebildiğini göstermektedir. Orşitte ortaya çıkan KR’e ait bu latans uzaması refleksin alınmasına engel olmamakla birlikte, testis torsiyonu ayırıcı tanısında refleksin güvenirliliği tartışmalı hale getirmektedir.
