Treatment of resistant pulmonary atelectasis after esophageal atresia repair with rhDNase

Introduction

Atelectasis is frequent in children with respiratory diseases and often requires invasive ventilation. The standard treatment is a postural drainage and the aspiration of bronchial tree after selective intubation. Dornase alfa (rhDNase) hydrolyzes the DNA present in mucus of cystic fibrosis patients and reduces viscosity in the lungs, promoting improved clearance of secretions. We presented here the results of rhDNase treatment in two cases that developed right lung atelectasis after esophageal atresia repair and did not respond to conventional treatments. 

Case 1

A 15-day-old newborn girl was operated due to esophageal atresia with distal tracheoesophageal fistula and right lungatelectasis was developed in postoperative 11th day. Standard treatments including thoracic physiotherapy, vapor inhalation, sale irrigation and intratracheal aspiration have been ineffective. On the 20th postoperative day rigid bronchoscopy was performed. The atelectasis were partly abated but recurred. Nebulization with rhDNase was started on the 26th postoperative day. Three doses per day, each dose containing 1.25mg of rhDNase were applied for 2 days. Chest x-ray was taken at the end of three doses. The full recovery in patient’s atelectasis was observed after 2 days and no recurrences occurred. 

Case 2

A 7-day-old boy newborn that underwent esophageal atresia with distal tracheoesophageal fistula repair on 3-day-old was developed right lung atelectasis. Patient was underwent standard treatment, but atelectasis wasn’t improve. Patient was treated with rhDNase on the 9th postoperative day. Patient was treated for 2 days with rhDNase same as in the first case. The full recovery in patient’s atelectasis was observed after 2 days and no recurrences occurred. 

Conclusions

To the best of our knowledge, there is no previous literature report on the use of rhDNase treatment in patients who developed resistant atelectasis after thoracic surgery. In newborns who underwent a thoracic surgical intervention, rhDNase treatment was found to be a possible tool to cope with resistant atelectasis of postoperative origin. 

Özofagus Atrezisi Operasyonu Sonrası Gelişen Dirençli Atelektazinin rhDNase ile Tedavisi

Giriş

Atelektazi solunum yolu hastalıkları olan bebeklerde sık görülür ve genellikle mekanik ventilasyon gerektirir. Standart tedavisi postural drenaj ve selektif entübasyon ile ana bronşların aspirasyonundan ibarettir. Alfa dornaz (rhDNase) kistik fibrozisli hastalarda mukus içerisindeki DNA'yı hidrolize ederek, viskoziteyi azaltır ve akciğerlerdeki sekresyonların atılımını kolaylaştırır. Burada özofagus atrezisi operasyonu sonrasında gelişen ve konvansiyonel yöntemlerle düzelmeyen, sağ akciğer atelektazili iki olguda, rhDNase tedavi sonuçlarını sunduk.

Olgu 1

On beş günlük kız bebek özofageal atrezi ve distal trakeoözofageal fistül nedeniyle opere edildikten 11 gün sonra sağ akciğerinde atelektazi gelişti. Toraks fizyoterapisi, buhar, serum fizyolojik ile irrigasyon ve trakeal tüp içi aspirasyonu gibi standart tedavilere cevap alınamadı. Postoperatif 20. gün rijit bronkoskopi yapıldı. Atelektazi kısmen açıldı ancak tekrar etti. 26. gün gün nebulüzatörle rhDNase tedavisi başlandı. 2 gün boyunca, günde 3 kez 1.25mg dozda rhDNase uygulandı. Her 3 dozluk tedavi sonunda kontrol akciğer grafileri çekildi. İkinci gün sonunda hastanın atelektazisinin tamamen düzeldiği görüldü ve tekrar etmedi.

Olgu 2

Özofageal atrezi ve distal trakeoözofageal fistül nedeniyle 3 günlükken opere edilen, 7 günlük erkek bebekte, sağ akciğerde atelektazi gelişti. Hastaya standart atelektazi tedavileri uygulandı, fakat atelektazide düzelme olmadı.  Postoperatif 9.gün hastaya rhDNase tedavisi başlandı. Hastaya, olgu 1’de uygulandığı şekliyle, 2 gün boyunca rhDNase tedavisi uygulandı. İkinci gün sonunda hastanın atelektazisinin tamamen düzeldiği görüldü ve tekrar etmedi. 

Sonuç

Bildiğimiz kadarıyla, toraks cerrahisi sonrası dirençli atelektazi gelişen hastaların tedavisinde rhDNase kullanımı daha önce literatürde bildirilmemiştir. Toraks operasyonu yapılmış yenidoğanlarda postoperatif gelişen dirençli atelektazilerin tedavisinde rhDNase kullanımı alternatif olabilir.
