TREATMENT RESULTS OF CHILHOOD CONSTIPATION IN TWO DIFFERENT DEPARTMENTS

OBJECTIVE:

Constipation is one of the frequent complaints in childhood that brings the families to outpatient clinics. Treatment of constipation requires the patience of both the physician and the family. Treatment should be explained to the family in detail. Here we report the results of a prospective study comparing two treatment protocols using the same approach and two different drugs for constipation in 2 different outpatient clinics, paediatric surgery and paediatric gastroenterology.

 

METHOD:

A constipation question form was created and the same form was used in both departments. Age, gender,  the time of onset of constipation, stool frequency, staining of the underwear, diet, comorbid disease, previous medical treatments, and Bristol Stool Scale were documented. As a treatment for constipation, magnesium hydroxide was prescribed for paediatric surgery patients and lactulose was prescribed for gastroenterology patients. In addition to medical treatment, families received training in diet and behaviour. Local anaesthetic ointment and hot sitz baths were recommended for patients with anal fissure. Patients were reexamined at the second and tenth weeks after the first examination.

 RESULTS:

Of the 43 patients that were enrolled, 26 patients were treated by the paediatric surgery and 17 by the gastroenterology units. Patients' age range was 8 months-15 years in the paediatric surgery group, and 2-15 years in the paediatric gastroenterology group. According to the Bristol Scale, 60% of patients had type-2-3 stool. Fourteen patients admitted to surgical and 2 patients admitted to gastroenterology outpatient clinics had anal fissure. Two patients, one that was operated for anorectal malformation, and another with anal stenosis, underwent bougie dilation. Ten patients (39%) that were admitted to surgery, and 9 patients (53%) that were admitted to the paediatric gastroenterology unit have responded well to treatment at the second week of follow-up. Drug dose adjustment was applied and the importance of diet therapy was emphasized in 9 patients during controls. All patients in both groups have responded well to treatment regardless of medication at the end of 10 weeks.

 IMPLICATIONS:

Additional pathologies such as anal fissure, anal stenosis and anorectal malformation were more frequent in paediatric surgery patients (61%) compared to gastroenterology patients (12%). This finding may explain the slightly lower response to treatment at the end of the second week in surgical patients compared to gastroenterology patients. There were no differences in lactulose and magnesium hydroxide groups at the end of 10th week. Overall, 73% of the patients have previously received medical treatment at another centre without any progress. We think that, in addition to two different laxatives, behavioural treatment, diet adjustments and approach to additional pathologies helped us to get positive results in these patients.

ÇOCUKLUK ÇAĞI KONSTİPASYONU TEDAVİSİNDE İKİ AYRI BİLİM DALININ SONUÇLARI

AMAÇ:

 

Çocuklarda kabızlık ailelerin çocuk cerrahisi polikliniklerine sıkça başvurdukları bir şikayettir. Kabızlık, hem aile hem de hekimin sabırlı olması gereken bir tedavi süreci gerektirir. Tedavi yönteminin hekim tarafından aileye detaylı bir şekilde anlatılması gerekmektedir. Çalışmamızda çocuk cerrahisi ve pediatrik gastroenteroloji polikliniğine başvuran kabızlık hastaları aynı yaklaşım protokolü ve iki ayrı laksatif uygulanarak prospektif olarak karşılaştırılmıştır.

 

YÖNTEM:

 

Çalışmamızda kabızlık formları oluşturularak kabızlık şikayeti ile gelen hastalara sorular yöneltildi. Yaş, cinsiyet, kabızlığının ne zaman başladığı, ne sıklıkta gaita yaptığı, kilot kirlenmesi, diyeti, ek hastalığı, daha önce herhangi bir medikal tedavi alıp almadığı, Bristol Skalasına göre gaitanın hangi tipe benzediği sorgulandı. Kabızlık tedavisi olarak çocuk cerrahisi polikliniğine gelen hastalara magnezyum hidroksit, pediatrik gastroenteroloji polikliniğine gelen hastalara ise laktüloz başlandı. Medikal tedavinin yanında diyet ve davranış eğitimi verildi. Birlikte anal fissürü olan hastalara anestol pomad ve sıcak oturma banyosu önerildi. İlk muayene sonrası ikinci ve onuncu hafta kontrolleri yapıldı.

 

BULGULAR:

Çocuk cerrahisinde 26, gastroenterolojiden 17 olmak üzere toplam 43 hasta çalışmaya dahil edildi. Çocuk cerrahisine gelen kabızlık hastalarının yaş aralığı 8 ay-15 yaş idi. Pediatrik gastroenteroloji polikliniğine başvuran hastaların yaş aralığı ise 2-15 yaş idi. Hastaların %60 ının gaita tipi Bristol Skalasına göre tip-2-3 idi. Cerrahi polikliniğine başvuran 14 hastada, gastroenteroloji polikliniğine başvuran 2 hastada anal fissür mevcuttu. Opere anorektal malformasyon ve anal stenoz olan iki hastaya buji dilatasyonu uygulandı. 2. hafta kontrollerinde cerrahi polikliniğine başvuran hastalardan 10(%39) hasta, pediatrik gastroenteroloji hastalarından ise 9 (%53) hastada tedaviye yanıt alınmıştı. Kontrollerde 9 hastaya ilaç doz ayarlaması yapıldı ve diyet tedavisinin önemi vurgulandı. Her iki gruptaki hastaların tamamı 10. hafta kontrollerinde tedaviye cevap almıştı.

 

ÇIKARIMLAR:

Çocuk cerrahisi hastalarında anal fissür, stenoz, anorektal malformasyon gibi ek patolojiler hastaların %61’inde görülmekteyken, gastroenteroloji polikliniğine başvuran hastalarda bu oran %12 olarak bulundu. Bu durum, cerrahiye başvuran hastalarda ilk haftada tedaviye alınan cevabın gastroenteroloji polikliniği hastalarına göre biraz daha düşük olmasını açıklayabilir. Her iki grupta onuncu hafta sonunda laktüloz ve magnezyum hidroksit arasında sonuçlar bakımından fark bulunamadı. Hastaların tamamına bakıldığında %73’ü daha önce başka bir merkezde medikal tedavi almış, fakat sonuç alınamamıştı. Laksatiflerin yanı sıra, beslenme, davranış ve ek patolojilere yönelik yaklaşımlarımızın sonuçlarımmızı olumlu kıldığını düşünüyoruz.
