A COMPARISON OF SSI PAYMENTS IN CASES OF ACUTE APPENDICITIS TREATED USING EITHER LAPAROSCOPIC OR OPEN SURGICAL TECHNIQUES

AIM

Since 2010 the costs of inpatient and outpatient health services in health care facilitiesare determined accordingto the Social Security Institution (SSI) Medical Application Declaration(MAD). According to the MAD,university hospitalsare classified as 3rdstep health institutions.Appendectomy is accepted as a Group-C Operation under MADAppendix-9. Payments for appendectomies, including all diagnostic and treatment expenses, are set at package prices of 440 TL for open technique and 748 TL for laparoscopic technique operations. Under the performance system, exceeding the amount of a package has negative financial consequences for the responsible surgeon. In our study we aimed to compare open and laparoscopic appendectomy techniques in terms of the parameters affecting the cost, such as duration of hospitalization, antibiotic use and postoperative complications.

MATERIALS AND METHODS

Patients with acute appendicitis, who underwent surgery in a university hospital with the use of either the open or the laparoscopic operational techniques, were compared retrospectively with regard to such parameters as  age, sex, duration of hospitalization, antibiotic use and postoperative complications were compared and a cost analysis was performed.

RESULTS

A group of 6 cases of acute appendicitis operated using the open technique was compared witha group of 6 cases of acute appendicitis treated using the laparoscopic technique. The mean length of stay in hospital was 2.3 days in both groups. For cases in which clinical and radiological findings indicated acute appendicitis, intravenous single antibiotic treatment was commenced, whereas for patients with suspectedperforation Intravenous tripleantibiotic treatment was initiated. After the operation patients with acute appendicitis were given intravenous single antibiotic. After this IV treatment, wide spectrum oral antibiotic was given until the 7th day postoperatively. There were no complications except early abdominal pain requiring medical attention in 2 patients in each group. Apatient with early abdominal pain in the laparoscopicsurgerygroup developedpersistent abdominal pain and tests were performed to reveal the etiology.This was the only patient causing a lossin financial terms in the laparoscopicsurgery group. The average cost of the cases operated using the open technique was 676 TL, and that for the cases in which the laparoscopic technique was used was 753 TL. Whilst a financial loss was incurred for each of the6 patients operated using the open technique, there were no financial losses in the laparoscopic technique group except for the afore-mentioned patient in who developed a non-regressing post-operative abdominal pain requiring advanced investigation.

CONCLUSIONS

Although the groups are small, in terms ofcostanalysis, postoperativecomplications and duration of hospitalization, both open and laparoscopic techniquesproduced similar results. In this casethe surgeon's medical decision, in suitable cases, to use the laparoscopic technique,for which the MAD payment is higher, will protect the management ofthe hospital and thus the responsible surgeon frompossible financial losses. However further studies are needed in terms of amortisement costs of medical devices (surgical instruments, laparoscopic instruments).

LAPAROSKOPİK VE AÇIK YÖNTEMLE YAPILAN AKUT APANDİSİTLERDE SGK ÖDEMELERİNİN KARŞILAŞTIRILMASI

AMAÇ

Sağlık kuruluşlarında ayaktan ve yatarak verilen sağlık hizmetlerinin  bedeli 2010 yılından beri Sosyal Güvenlik Kurumu(SGK) Sağlık Uygulama Tebliği (SUT)’ ne göre belirlenmektedir. SUT’a göre üniversite hastaneleri 3.basamak resmi sağlık kuruluşları olarak sınıflandırılmaktadır. SUT EK- 9  a göre appendektomiler C grubu ameliyat olarak kabul edilmektedir. Tüm tanı ve tedavi giderleri dahil olmak üzere açık teknikle yapılan appendektomi 440.00 TL paket fiyat üzerinden, laparoskopik teknikle yapılan appendektomiler ise 748.00 TL paket fiyat üzerinden ücretlendirilmektedir. Performans sisteminde paket tutarının aşılması sorumlu cerrahı maddi açıdan olumsuz olarak etkilemektedir. Çalışmamızda hastanede yatış süresi, antibiyotik kullanımı, postoperatif komplikasyonlar gibi maliyeti etkileyen parametreler açısından açık ve laparoskopik teknikle yapılan appendektomilerin karşılaştırılması amaçlanmıştır.

GEREÇ VE YÖNTEM

Üniversitesi hastanesinde açık ve laparoskopik teknikle opere edilen akut apandisitler retrospektif olarak yaş, cins, hastanede yatış süresi, antibiyotik kullanımı, postoperatif komplikasyonlar gibi parametreler açısından karşılaştırılmış ve maliyet analizleri yapılmıştır.

BULGULAR

Altı açık, 6 laparoskopik teknikle ameliyat edilen akut apandisit olgusu karşılaştırıldı. Hastanede yatış süresi her iki grupta ortalama 2.3 gündü. Klinik ve radyolojik bulgulara göre akut apandisit olduğundan emin olunan hastalarda iv tekli antibiyotik başlandı. Perforasyon şüphesi olan hastalarda ise iv 3 lü antibiyotik başlandı. Akut apandisit olgularında postoperatif IV tekli antibiyotiğe geçildi. Her iki grupta ikişer hastada postoperatif erken karın ağrısı dışında komplikasyon olmadı. Laparoskopik teknikle opere edilen grupta yer alan ve postop karın ağrısı gelişen hastanın ağrıları gerilemeyince ağrı etyolojisini ortaya koymak için tetkikler yapıldı. Bu hasta laparoskopik cerrahi yapılan grupta maliyet açısından tek zarar eden hastadır. Hastaların antibiyoterapisi IV antibiyotik kesildikten sonra oral geniş spektrumlu antibiyotikle 7 güne tamamlandı. Açık teknikle yapılan vakaların ortalama maliyetinin 676 TL olduğu, laparoskopik teknikle yapılan vakaların ortalama maliyetinin 753 TL olduğu bulundu. Açık teknikle opere edilen 6 hastanın tamamı zarar ederken, laparoskopik teknikle opere edilen hastalardan sadece postoperatif gerilemeyen karın ağrısı gelişen ve ileri araştırma yapılması gereken hasta zarar etmiştir.

 

ÇIKARIMLAR

Gruplarımız küçük olsa da, maliyet analizleri, postoperatif komplikasyonlar ve hastanede yatış süresi açısından baktığımızda açık ve laparoskopik teknikle yapılan vakaların birbirine yakın sonuçlarının olduğu görülmektedir. Her iki grupta da antibiyotik kullanımı için aynı protokol kullanılmıştır. Bu durumda cerrahın uygun vakalarda teknik olarak SUT ödemesi daha fazla olan laparoskopik tekniği kullanması hastane yönetimlerini ve dolayısıyla sorumlu cerrahı olası maddi kayıplardan koruyacaktır. Ancak, kullanılan tıbbi cihazların (cerrahi aletler, laparoskopi cihazları) amortisman maliyetleri açısından daha ileri çalışmalara gereksinim vardır.
