The effect on postoperative pain of pulling the rectus muscle medially during open appendectomy surgery.

 

Introduction:Laparoscopic appendectomy is generally preferred because of its many advantages such as postoperative pain comfort but open appendectomy is a safe standard surgical technique with proven efficacy in both acute and complicated appendicitis. We investigated whether pulling the rectus muscle medially during open appendectomy surgery had any effect on postoperative pain in this study.

Material and Method:This study was performed on patients 6 years and older who were admitted for acute appendicitis and operated at the Denizli State Hospital, Department of Pediatric Surgery in this prospective study. The patients were divided into two groups and open appendectomy was performed by pulling the rectus muscle medially in the first group and splitting the rectus muscle in the second group. Pain was evaluated in both groups at 12 and 24 hours after surgery by using a visual analog scale graded from 1 to 10. The statistical analysis of the study was performed with the ''t'' test on the SPSS 16.0 software.

Results:A total of 61 patients with a mean age of 10.44±2.11 (6-14) years consisting of 34 males and 27 females were operated on for acute appendicitis during 2010 and 2011. The first group consisted of 31 and the second group of 30 patients. The preoperative and 12 and 24 hour postoperative pain evaluation results were 8.25±1.03, 2.96±1.40 and 1.16±0.93 in the first group and 8.36±0.99, 3.90±1.24 and 2.03±1.06 respectively in the second group. There was no statistically significant difference between the two groups for age, gender, inpatient duration and preoperative pain scores while the 12 and 24 hour postoperative pain scores were lower in the first group than the second group.This difference was statistically significant (p<0.05).

Discussion:Performing the appendectomy by pulling the rectus muscle medially in clinics using open appendectomy will provide a more comfortable postoperative period for the children. 

Açık appendektomi ameliyatlarında rektus kasının mediale çekilmesinin operasyon sonrası ağrı üzerine etkisi

 

Giriş:Özellikle postoperatif ağrı konforu gibi bir çok avantajlarından dolayı çoğu klinikte laparoskopik appendektomi tercih edilse de, açık appendektomi halen hem akut hem de komplike appendisitlerde güvenli ve etkinliği kanıtlanmış standart cerrahi tedavidir. Bu çalışmada açık appendektomi operasyonunda rektus kasının mediale çekilmesi işleminin postoperatif ağrı üzerine etkisi olup olmadığı araştırılmıştır.

Materyal ve Metod:Bu araştırma prospektif bir çalışmada olup, Denizli Devlet Hastanesi, Çocuk Cerrahisi Kliniğine akut appendisit tanısı ile yatırılan ve opere edilen 6 yaş ve üzerindeki hastalar üzerinde yapıldı. Hastalar iki grubu ayrıldı, birinci grup da rektus kası mediale çekilerek, ikinci grup da ise rektus kası ayrılarak açık appendektomi uygulandı. Her iki grupta da preoperatif ve postoperatif 12. ile 24. saatlerinde 1’den 10’a kadar derecelendirilen görsel kıyaslama ölçeği yardımıyla ağrı değerlendirilmesi yapıldı. Araştırmanın istatistik analizinde SPSS 16.0 programı kullanılarak “t” testi kullanıldı.

Sonuçlar:2010-2011 yılları arasında yaş ortalaması 10,44±2,11 yıl (6-14) ve E/K oranı 34/27 olan 61 hasta, akut appendisit nedeniyle opere edildi. Birinci grup 31, ikinci grup ise 30 hastadan oluşmaktaydı. Preoperatif ve postoperatif 12. ile 24. saatlerdeki ağrı değerlendirmeleri sırasıyla birinci grupta,  8,25±1,03, 2,96±1,40 ve 1,16±0,93 iken ikinci grupta sırasıyla 8,36±0,99, 3,90±1,24 ve 2,03±1,06 olarak saptandı. Her iki grup arasında da yaş, cinsiyet, hastanede kalış süresi ile preoperatif ağrı skorları arasında istatistiksel olarak bir fark bulunmazken, postoperatif 12 ve 24. saat ağrı skorlarının birinci grupta,  ikinci gruptan daha düşük olduğu saptandı. Bu farklar istatistiksel olarak anlamlı idi (p<0,05).

Tartışma:Açık appendektomi uygulanan kliniklerde, appendektominin rektus kasının mediale çekilerek yapılması, çocuk hastaların daha konforlu bir postoperatif dönem geçirmelerinde önemli bir tercih olacaktır.
