Laparoscopic Nissen fundoplication with U-stich Stamm gastrostomy; early period outcomes

 

Background and aim: We aimed to evaluate the outcomes of laparoscopic Nissen fundoplication with U-stich Stamm gastrostomy in children with neurologic impairment.

Patient and methods: Medical records of 20 children whom undergone to laparoscopic Nissen fundoplication with U-stich Stamm gastrostomy between February 2012 and June 2012 were retrospectively evaluated.

Results: Mean age was 38.7 (6-164) months. Mean weight was 13.25 (4.6-27) kg. Indications for operation were cerebral palsy, swallow dysfunction due to metabolic and neurologic impairment, gastroesophageal reflux, failure to thrive, and recurrent respiratory infections. Mean operation time was 140 minutes (90-200). Early complications did not encountered in the series, however a gastrocutaneous fistula due to U stich was developed in one child which was spontaneously closed after suture removal and gastrostomy leak was occurred in in two children due to improper changing the gastrostomy catheter with a button. One of these children was treated by open surgery and the other was operated by laparoscopy. Intermittent gastroesophageal reflux complaints were occurred in one of these children.

Conclusion:Laparoscopic Nissen fundoplication and gastrostomy proved its efficiency in neurologic impaired children. In catheter revision, changing with low diameter catheters can prevent gastrostomy leak risk. Also gastrostomy leaks are limited defects that could be repaired by laparoscopy.

Laparoskopik Nissen Fundoplikasyon ve U-dikiş Stamm Gastrostomi: erken dönem sonuçlar

 

Önbilgi ve Amaç:Nörolojik sekelli çocuklarda laparoskopik Nissen fundoplikasyon ve U-dikiş Stamm gastrostomi operasyonun sonuçlarını değerlendirilmesi amaçlandı.

Hastalar ve yöntem:Şubat 2012 ve haziran 2012 tarihleri arası laparoskopik Nissen fundoplikasyon ve U dikiş Stamm gastrostomi açılan 20 (12 erkek, 8 kız) olgu geriye yönelik değerlendirildi. Çocukların demografik bulguları, operasyon süreleri, erken ve geç komplikasyonlar değerlendirildi.

Bulgular:Ortalama yaş 38.7 (6-164) ay idi. Ortalama ağırlık 13.25 kg (4.6-27) idi. Çocuklarda operasyon nedenleri; serebral palsi, metabolik ve nörolojik nedenlere bağlı yutma disfonksiyonu, kusma, gelişme geriliği ve tekrarlayan akciğer enfeksiyonları idi. Ortalama işlem süresi 140 dakika (90-200) idi. Olgularda erken komplikasyon olmaz iken bir olguda U dikişe bağlı ve dikiş alındıktan sonra kendiliğinden iyileşen gastrokutanöz fistül ve 2 hastada işlemden 3 hafta sonra gastrostomi kateterinin uygun olmayan şekilde buton kateteri ile değişimi sırasında kaçak gelişti. Olgulardan biri açık yöntem ile diğeri ise yine laparoskopik olarak onarıldı. Kaçak gelişen olgulardan birinde bolus beslemede aralıklı reflü yakınması saptandı.

Sonuç:Nörolojik sekelli çocuklarda laparoskopik fundoplikasyon ve gastrostomi etkinliğini kanıtlamış bir yöntemdir. Gastrostomi katateri değişim sırasında düşük çaplı kateter kullanımı ile gastrostomi kaçak riski önlenebilir. Ayrıca gastrostomi kaçakları sınırlı seviyede olmaktadır ve laparoskopik olarak onarılabilmektedir.
