A RARE CAUSE OF ABDOMINAL PAIN: MUCINOUS ADENOCARCINOMA OF COLON

Introduction: Mucinous adenocarcinoma of colon (MAC) is rare in children. Its incidence is  1.3-2: 1 000 000. In this study a case of MAC treated in our clinic is presented.

Case report: an 11-year-old boy with abdominal pain and vomiting for 15 days  was admitted to department of pediatric oncology. Ultrasound revealed massive ascite in the abdominal cavity and solid mass with heterojenous echogenicty arising from omentum. The history revealed that he had intermittent abdominal pain for 1 year.  In the computerised tomography of the abdomen there were mass lesions on liver surface in accordance with peritoneal implants, a mass of 10x4 cm in the umbilical level and a lobulated mas of 5x4 cm in the hepatic flexure. He was operated with the diagnosis of abdominal mass. A serohemorrhagic ascite of 3000 ml was found and drained. All the omentum was found to be a solid necrotic mass. A mass in the mid-transverse colon allowing luminal passage and multiple metastatic nodules on the peritoneal surface were found. Surgical intervention consisted of multiple biopsies only. Histopathologic examination showed MAC and the chemotherapy with irinotecan was started. On his 2nd postoperative month during follow-up, he was re-laparatomized for excessive abdominal distention and an  ileostomy was performed. There was no intestinal passage problem in the postoperative period and no response to chemotherapy was observed. The patient was discharged from the hospital with his parents’ wish. Later it was learned that he died 1 month after discharge from the hospital.

Discussion:early diagnosis in colorectal carcinomas increases the chance of cure. Because it is rarely  seen in childhood, the diagnosis of colorectal carcinoma in children is a late finding. Besides it is not suspected in the differential diagnosis.  Tumor biology itself has an agressive behaviour. Colorectal carcinomas in children have poor prognosis. 

NADİR BİR KARIN AĞRISI NEDENİ: KOLON MÜSİNÖZ ADENOKARSİNOMU

Giriş: Kolon müsinöz adenokarsinomu çocuklarda çok nadir görülür. İnsidansı 1,3-2: 1 000 000’dir. Bu çalışmada kliniğimizde tedavi edilen bir olgu sunulmuştur.

Olgu Sunumu: 11 yaşında erkek hasta 15 gündür artan karın ağrısı ve kusma nedeniyle yapılan ultrasonografisinde yaygın asit, omentum kaynaklı heterojen ekoda solid kitle saptanması üzerine onkoloji servisine yatırıldı. Öyküsünde bir yıldır aralıklı karın ağrısı olduğu öğrenildi. Batın tomografisinde karaciğer düzeyinde peritoneal implantla uyumlu kitlesel lezyonlar, umblikus düzeyinde 10x4 cm kitlesel lezyon, hepatik fleksura düzeyinde 5x4 cm lobüle konturlu kitle saptandı. Batında kitle ön tanısıyla operasyona alındı. 3 000 cc serohemorajik sıvı, tüm omentumun nekrotik solid kitle haline geldiği, transvers kolon orta kesiminde pasaja izin veren kitle olduğu ve tüm peritoneal yüzeyde multipl metastatik nodüller olduğu görüldü. Biyopsiler alınarak işlem sonlandırıldı. Patoloji sonucu müsinöz adenokarsinom lehine gelen olguya İrinotecan tedavisi başlandı. İzleminin ikinci ayında ileri derecede batın distansiyonu gelişmesi üzerine tekrar laparotomi uygulanarak ileostomi açıldı. Postoperatif dönemde pasaj sorunu olmayan olgu, kemoterapiye yanıt alınamaması üzerine aile isteği ile taburcu oldu. Tabucu olduktan 1hafta sonra eksitus olduğu öğrenildi.

Tartışma: Kolorektal karsinomlarda erken tanı kür şansını artırır. Çocukluk çağında nadir görülmesi, ayırıcı tanıda akla gelmemesi ve agresif tümör biyolojisi nedeniyle geç tanı alır ve kötü prognoza sahiptir.
