Esophageal transplantation in rats

 

Aim: Esophageal replacement surgery have been used for treating long gap esophageal atresia, caustic esophageal stricture or esophageal avulsion. Here, we report the esophageal transplantation in the rats without vascular anastomosis as a option to esophageal replacement surgery.

Methods: Fourteen total segments of esophagus transplants were harvested from male 24-week-old Sprague-Dawley rats by applying the harvesting procedure and transplanted to male 15-week –old Sprague-Dawley rats side by side to own esophagus without vascular anastomosis by using transhiatal pull-up technique. Each ends of transplant esophagus were ostomized by cervical and abdominal esophagostomy. In group I (n = 7) only cotrimaxazole, in group II (n = 7) cyclosporine and cotrimaxazole were used for 10 days. On postoperative day 10, the rats were sacrificed, and the transplant and own esophagus were evaluated macroscopically and histopathologically.

Results: All transplantations were achieved successfully and all transplanted rats survived. No evidence of complications was observed, but mild necrosis in the cervical end of the transplant in one rat in the group II was observed. Histopathologic examination showed viable esophageal structure in all rats. Inflammation and muscular atrophy were lower in group II than group I compared with own esophagus.

Conclusions: Esophageal transplantation can be performed directly without vascular anastomosis into own place in the rat model, may be achieved in humans in the future.

Sıçanlarda özefagus transplantasyonu

 

Amaç:Özefagus replasman ameliyatları uzun aralıklı özefagus atrezisi, kostik özefagus striktürü ve özefagus avulsiyonu tedavisinde kullanılmaktadır. Burada, özefagus replasman ameliyatlarına bir seçenek olarak damarsal anastomoz yapılmaksızın sıçanlarda gerçekleştirdiğimiz özefagus transplantasyonunu sunuyoruz.

Gereç ve Yöntem:Ondört tam segment özefagus transplantı 24 haftalık erkek Sprague-Dawley sıçanlardan doku hasadı işlemi uygulanarak alındı ve erkek 15 haftalık Sprague-Dawley sıçanlara transhiatal pull-up tekniği kullanarak damarsal anastamoz yapılmaksızın kendi özefagusları yanına transplante edildi. Transplant özefagusun her iki ucu servikal ve abdominal özefagostomi ile ostomize edildi. Grup I’de (n = 7) sadece kotrimaksazol, grup II’de ise (n = 7) siklosporin ve kotrimaksazol 10 gün süre ile kullanıldı. Postoperatif 10. günde sıçanlar sakrifiye edilip transplant ve kendi özefagusları makroskobik ve histopatolojik olarak değerlendirildi.

Bulgular:Tüm transplantasyonlar başarıyla gerçekleştirildi ve tüm transplante sıçanlar yaşadı. Komplikasyon kanıtına rastlanmadı, ancak grup II'de bir sıçanda transplantın servikal ucunda nekroz saptandı. Histopatolojik incelemede tüm sıçanlarda özefagus canlı bir yapı gösteriyordu. Enflamasyon ve müsküler atrofi sıçanların kendi özefagusları ile karşılaştırıldığında grup II’de grup I’den daha düşüktü.

Sonuç:Özefagus transplantasyonu sıçan modelinde damarsal anastomoz yapılmaksızın kendi bulunduğu yere doğrudan yapılabilir, gelecekte de belki insanda başarıyla uygulanabilir.
