A rare case report of coexistence of appendicitis and Traumatic spleen

 

 

      11years old female patient after a traffic accident in cases brought to our hospital emergency.

 

      On the examination had abdominal tenderness. Emergency input Hb: 12.5, respectively. Had a right clavicular fracture on plain radiographs.  Thoracic CT scan revealed left hemithorax had pneumothorax minimal. Abdominal CTscan revealed a laceration of the spleen. The patient was admitted for close observation and treatment.

 

      Hbin 4 hours, respectively, 12,11,10 and the latest was 9.Then we decided to laparatomy . Spleen laceration and diffuz hemorrhage  were the exploration findings. After splenectomy during exploration we observed enlarged appendix that had two pieces of fecalon plug. Appendectomy was done. Postoperative recovery was unevetful and we have confirmed pathology.

 

       During exploration after abdominal trauma If there is meccel diverticulum, normal appendix and accesory spleen  there is no need to surgery. However a rare condition that has not preoperative clinic story as in our case. If appendix is expended, enlarged and plug fecalom is present appendectomy should be done and this should be confirmed by pathology.

Travmatik dalak ve appandisit nadir birlikteliği olgu sunumu

  

      11 yaş kız hasta, araç dışı trafik kazası sonrasında hastanemiz aciline getirilen hastayı sunmayı amaçladık.

 

     FM’de batında hassasiyet vardı. Acil giriş Hb:12.5 idi. Direkt grafide sağ klavikula kırığı mevcuttu. Toraks tomografide sol hemitoraksda minimal pnomotoraks mevcuttu. Batın tomografide dalak laserasyonu mevcuttu. Hasta yakın takip ve tedavi için yatırıldı.

 

      Hb 4 saat içinde sırasıyla 12,11,10 ve  en son 9’ a düşmesi üzerine hasta operasyona alındı. Operasyonda; Dalak laserasyonu batında yaygın kan mevcuttu. Splenektomi yapılıp, batın eksplorasyonunda appendiks genişlemiş ve içersinde 2 adet fekalom tıkacı palpe edilmesi üzerine, appendektomi yapıldı. Operasyon sonrası iyileşme sorunsuzdu, patolojiyle  konfirme ettik.

 

     Travma sonrası yapılan batın eksplorasyonlarında, semptom vermeyen mekkel divertikülü, aksesuar dalak, normal görünümlü appendiks gibi oluşumlara genel kanaat onlara dokunulmaması yönündedir. Ancak olgumuzdaki gibi, eksplorasyon sırasında nadirde görülse, appendiks lümeninde genişleme ve palpasyonla içinde fekalom tıkacı tespit edilmiş vakalarda, preklinik hikaye olmamasına rağmen, appendektomi yapılmalı ve bu durum patolojiyle konfirme edilmelidir.
