Ileal trichobezoar mimicking acut appendicitis

Introduction: Bezoars are intraluminal masses which are formed by foreign substances in the stomach or small intestine. Trichobezoars are the bezoars that caused by hair. Trichobesoar mimicking acute appendicitis is presented.

Case: 15year old girl was admitted to our emergency room with the complaints of abdominal pain, nausea, vomiting. Abdominal ultrasound study revealed edema and free fluid around the caceum. There was leukocytosis (15.000/mm3) on the blood cound and the appendix was not seen. The patient was operated with the diagnosis of appendicitis. During operation the  appendix seemed like a normal. The patient underwent appendectomy.  There were no other pathologic findings during the exploration. In the postoperative period, clinical  signs did not regress and at the 10th day after operation the second laparotomy was performed. There was a mass in the ileum and the mass was considered as a bezoar. Enterotomy was performed and trichobezoar was extracted.

Conclusion: In children,various disease, like Familial Mediterranean Fever and mesenteric lymphadenitis, can mimic the appendicitis.In our patient, trichobezoar which localised in the ileum mimicked as accute appendicitis  due to  edema and partially obstruction. In conclusion, trichobezoar must be considered in the differential diagnosis of acute appendicitis.

Akut appendisiti taklit eden ileal trikobezoar olgusu.

Giriş:Bezoarlar yutulan yabancı maddelerin mide veya ince bağırsakta oluşturduğu intraluminal kitlelerdir. Saç kıllarının neden olduğu bezoarlara trikobezoar denir. Akut apandisiti taklit eden trikobezoar olgusu sunuldu.

Olgu:15 yaşındaki kız çocuğu karın ağrısı, bulantı ve kusma şikayetleri ile başvurdu. Fizik muayenede sağ alt kadranda defans, rebound ve hassasiyet saptanan hastada ultrasonografide çekum çevresinde serbest sıvı ve ödem gözlendi. Appendiksi vizualize edilemeyen hastada kan sayımında lökositoz vardı(15.000/mm3). Hasta akut apandisit ön tanısıyla opere edildi. Ancak appendiks şüpheli normal değerlendirildi. Eksplorasyonda ek patoloji saptanmayan hastaya appendektomi uygulandı. Post-op klinik bulguları gerilemeyen hastaya 10. Günde  yapılan ikinci exploratris laparatomide ileumda lümen içinde hareket eden kitle bezoar lehine değerlendirldi. Enterotomide trikobezoar saptanarak çıkarıldı.

Sonuç: Çocuklarda ailevi akdeniz ateşi, mezenterik lenfadenit gibi birçok hastalık akut apandisiti taklit edebilir. Olgumuzda, ileumda lokalize olan trikobezoar, kısmi obstrüksiyon ve enflamasyona sebep olarak akut appendisiti taklit etmişti. Sonuç olarak akut apandisit düşünülen çocuklarda ayırıcı tanıda trikobezoar düşünülmelidir
