Acquired diaphragmatic hernias in children

Aim: Acquired diaphragmatic hernias are rare in children. Diagnosis is usually difficult. The aim of this study is to determine some clues for early diagnosis and treatment of the disease.

Materials and Methods:  Between 2005-2012, five children with  acquired diaphragmatic hernias were evaluated.  The cause of hernia, clinical and radiological findings, localization of rupture,  organs herniated to the thoracic cavity, surgical procedures and outcome of the patients were outlined retrospectively .

Results: Four of the patients were male with an age range of 9 months  to 13 years.  Initial diagnosis was blunt trauma in 3 cases, respiratory distress in 1, and evantiration of the diaphragm in 1 patient. Chest  X-ray and computed tomography (CT) revealed bilateral diaphragmatic rupture in one patient and rupture on the left in two patients with blunt thoracic trauma. In two patients with left diaphragmatic rupture,  stomach, splenic flexure and spleen were the herniated structures to the thoracic cavity. In bilateral diaphragmatic rupture, transverse colon was herniated to the thoracic cavity which was shown by contrast enema of the colon.  Additionally in these patients, there were liver laceration, multiple pelvic fracture, and left duplexkidney and laceration of ureteropelvic junction of the lower pole in one patient. In the patient with respiratory distress, there was a history of empyema, two chest tube applications and thoracoscopy two months ago. Chest X-ray was not conclusive but herniation of liver and intestines had been detected on CT.  In the patient with initial diagnosis of diaphragmatic evantration, there was a history of two surgical explorations involving the left thorax for hydatic cyst two years ago. It was hard to decide to the integrity of the diaphragm with CT. Stomach herniation was detected with upper GI series. Primary reparations were applied  to 4 cases by abdominal approach and both abdominal and thoracal approach in one patient. In one case, reoperation with graft was carried out for recurrence. Chest X-ray showed clear margins of the diaphragm one month after the operations in all patients.  One patient was followed up with gastroesophageal reflux with antireflux treatment.

Conclusion: In patients with acquired diaphragmatic hernia, detailed medical history as well as conventinal radiology with contrast and CT are greatly helpful procedures for diagnosis and treatment of the condition.      

Çocuklarda edinsel diyafragma hernileri

Amaç: Çocuklarda edinsel ve travmatik diafragma hernileri az görülmekte ve diğer ciddi yaralanmalarla birlikte olması nedeniyle tanı aşamasında ciddi zorluklarla karşılaşılmaktadır.Amacımız erken tanı ve tedaviye yönelik ipuçlarını değerlendirmektir.

Gereç ve yöntem: 2005-2012 yılları arasında edinsel diyafragma hernisi tanısı ile bölümümüzde tedavi ettiğimiz 5 olguda herninin oluş nedeni, klinik ve radyolojik bulguları, rüptürün lokalizasyonu, toraksa herniye olan organlar, yapılan cerrahi girişimler ve olguların takip sonuçlarına ilişkin bilgiler geriye dönük olarak değerlendirilmiştir.

Bulgular: Hastaların4’ü erkek, 1’i kız, yaşları 9 ay- 13 yıl arasındaydı. 3 olgu künt travma, 1 olgu solunum sıkıntısı, 1 olgu diyafragma eventrasyonu ön tanısı ile başvurdu.  Künt travma nedeniyle başvuran olgularda çekilen akciğer grafisi ve toraks bilgisayarlı tomografisi  (BT)  tetkiklerinde 2’inde sol, 1’inde bilateral diyafram rüptürü tespit edildi. Sol diyafram rüptürü olanlarda mide, splenik fleksura, dalak toraksa herniye olmuştu. Bilateral olanda ise kontraslı kolon grafisinde transvers kolonun herniye olduğu görüldü. Bu olgularda ek olarak karaciğer laserasyonu, çoklu pelvis fraktürü, 1 olguda sol böbrekte çift toplayıcı sistem ve altpol üreteropelvik bileşkede laserasyon mevcuttu. Solunum sıkıntısı ile başvuran olgunun öyküsünde 2 ay önce sağ toraksta ampiyem nedeniyle 2 kez göğüs tüpü takıldığı ve torakoskopi yapıldığı öğrenildi. Akciğer grafisinde diyafram konturu silinmişti, toraks BT’inde karaciğer ve barsak herniasyonu tespit edildi.  Diyafragma eventrasyonu ön tanısı ile başvuran olgunun öyküsünden 2 yıl önce 2 kez sol akciğer kist hidatiği nedeniyle opere olduğu öğrenildi. Çekilen toraks BT’sinde diyafram bütünlüğü net değerlendirilemedi. Üst gastrointestinal görüntülemede midenin herniye olduğu tespit edildi. Olguların 4’üne abdominal, 1’ine torakal ve abdominal yaklaşım ile primer onarım uygulandı. 1 olgu rekürrens nedeniyle tekrar opere edilerek greft ile onarım yapıldı. Postop 1. ay kontrollerinde akciğer grafilerinde diyafragma konturlarının iyi olduğu görüldü. 1 olguda gastroözefagial reflü nedeniyle antirefü tedavisi ile takibe devam edildi.

Sonuçlar: Çocuklarda az görülen edinsel diyafragma hernisi tanısında ayrıntılı öykü, direk radyografi, kontraslı tetkikler ve BT’nin yardımcı olduğu görüldü.
