There is a Change at Ideas for Preference of Circumcision and Circumcision Techniques

Introduction: Circumcision is a most known worldwide applied surgical operation that has been done for more than 2500 years. For many years circumcision has been done without anesthesia, by people having many different occupations. Our aim is to determine knowledge content and misunderstandings of population about circumcision which is mostly done at school aged children .

Materials and Methods:We analyzed answer given to a questionnaire, questioning preoperative concepts and postoperative follow-up, that was applied to the families of 52 circumcision patients between December 2011 and June 2012. 

Results:Fifty two patients with average age of 26,5 ± 26,7 months examined. For education status, 76,1% of mothers and %78,7 of fathers were graduated from university or have higher education status. Most of the families (%80,8) indicate that they preferred circumcision for “needed for better health status” and 17,3% for “religious considerations”.  When properties of the environment that circumcision done was asked, 46,2% of the families primarily look for ‘a place where hygienic rules are applied’.  When appropriate age for circumcision was asked, 67,3% of families preferred 2 age old or younger ages. The decision of circumcision was made by doctor at 21,6% of cases. We had 72% correct answer to the question ‘What is circumcision?’. Infection was the major fear and drawback at a rate of 52%. Postoperatively seen infection (13,7%), bleeding (15,7%), pain (80,4%), swelling (47,1), scab formation (31,4%), pain during urination (41,2%) were considered normal by the families. General anesthesia is preferred 71,4%, while local anesthesia preferred 28,6%. After the circumcision, while 5 patients (9,6%) was inactive, 3 patients (5,8%) was aggressive and uneasy, 3 patients (5,8%) was anxious when going to toilet or when changing diaper, 4 patients (7,7%) had problems with replying and 3 patients (5,8%) awake with fear, 3 patients (5,8%) was seen to more pleased. Analgesic drugs were used less than or equal to 3 days. Healing completed within 7 days at 74,9% of cases. Most of the families (61,5%) emphasized that circumcision period was easier than expected and some of the families (7,7%) emphasized that circumcision period was difficult than expected. Families mostly suggested circumcision before 2-age-old (82,7%), under general anesthesia (67,3%), at a hospital with a pediatric surgery department (98,1%). The education status of parents does not affect answers given. When postoperative problems compared with emotional changes, it was seen that patients with pain during urination was more aggressive and uneasy.

Conclusions:Our study is a descriptive study about the knowledge of the families that came to our department for circumcision. We observed that higher socio-economic level, bring out health reasons for circumcision.  So that age of circumcision was decreasing. Families are demanding circumcision at surgical centers and with anesthesia. As families become conscious and the conditions advance, circumcision which was done without anesthesia or with local anesthesia, is changing according to family preferences.  We also detected that, pain during urination causes more aggressive and uneasy children after the circumcision. 

Toplumumuzda Sünnet Gerekçesi ve Yöntemleri Konusunda Değişim Var

Amaç:Sünnet 2500 yıldan fazladır yapılan ve bütün dünyada bilinen cerrahi bir işlemdir. Uzun yıllar anestezi uygulanmadan gerçekleştirilmiştir ve değişik meslek grupları tarafından yapılmaktadır. Amacımız, ülkemizde çoğunlukla okul çağında gerçekleştirilen sünnet hakkında toplumun bilgi düzeyini ölçmek ve yanlış anlamaları tespit edilebilmektir.

Yöntem:Aralık 2011 – Mayıs 2012 arasında Başkent Üniversitesi Ankara Hastanesi Çocuk Cerrahisi Polikliniği’nce sünnet operasyonu gerçekleştirilen 52 hastanın ailelerinin sünnet ile ilgili bilgi ve görüşleri ve sünnet sonrası seyri anket ile incelenmiştir.

Sonuç:Yaş ortalaması 26,5 ± 26,7 ay olan 52 hasta incelendi. Annelerin %76,1’i, babaların %78,7’i üniversite ve üzeri eğitim düzeyine sahiptir. Ailelerin %80,8’i sünneti “sağlık açısından gerekli bulduğu” için, %17,3’u “dini sebeplerden dolayı” yaptırmaktadır. Aileler sünnet yapılacak ortamı, öncelikle %46,2 oranında “hijyen kurallarına uymalı” olarak tanımladı. Ailelere sünnet için en uygun yaş %67,3 oranında 2 yaş ve altı belirtildi. Sünnet kararını %21,6 hastanın doktoru vermiştir. Sünnet nedir sorusuna %72 oranında doğru cevap aldık. Sünnet ile ilgili korku ve çekincelerin %52’sini enfeksiyon oluşturuyordu. Ailelerin %13,7’si enfeksiyonu, %15,7’si kanamayı, %80,4’u ağrıyı, %47,1’i şişlik olmasını, %31,4’u kabuk bağlamasını, %41,2 idrar yaparken sıkıntı olmasını, sünnet sonrası görülebilir olarak değerlendirmektedir. Ailelerin %71,4’ü genel, %28,6’sı lokal anesteziyi tercih etmektedir. Sünnet sonrası hastaların 5’i (%9,6) daha durgun, 3’ü (%5,8) daha agresif ve gergin, 3’ü (%5,8) tuvalete giderken veya alt değiştirilirken huzursuz olurken, 4’ünde (%7,7) güvenme problemi, 3’ünde (%5,8) uykudan korkarak uyanma görülmüş, 3’ünün (%5,8) daha mutlu olduğu belirtilmiştir. Ağrı kesici kullanımı 42 hastada 3 gün ve altında gerekmiştir. Yara iyileşmesi %74,9 hastada 7 günde gerçekleşmiştir. Ailelerin %7,7’si sünnetin beklediğinden zor, %61,5’i sünnetin beklediğinden kolay olduğunu belirtti. Aileler iki yaşın altında (%82,7), genel anestezi altında (%67,3), çocuk cerrahisi olan bir hastanede (%98,1) sünnet yaptırılmasını önermektedir. Anne ve babanın öğrenim durumunun sonuçları etkilemediği görüldü. Sünnet sonrası idrar yaparken sıkıntısı olan hastaların daha agresif ve gergin olduğu görüldü.

Yorum:Kliniğimizde sünnet yaptıran ailelerin bilgi durumlarını tespit eden tanımlayıcı bir çalışmadır. Ailelerin sosyo-ekonomik düzeylerinin yüksek olması, sağlık gerekçesi ile sünnet yapılmasını öne çıkarmaktadır. Bu nedenle sünnet yaptırma yaşı erkene kaymaktadır. Aileler sünnetin cerrahi merkezde ve anestezi altında yapılmasını talep etmektedir. Uzun yıllar anestezisiz veya lokal anestezi altında yapılan sünnetler ailelerin bilinçlenmesi ve koşulların gelişimi ile değişmektedir. Ayrıca idrar yaparken sıkıntı yaşayan hastaların sünnet sonrası daha agresif ve gergin olduğu görülmüştür.
