CASE WITH SACROCCYGEAL MASS THAT DISAPPEARED BY TIME

AIM: The retrorectal cystic lesions in pediatric age group are rare and majority of them are congenital.The most congenital formations in the retrorectal region are teratomas and developmental cysts,sacrococcygealwhich are epidermoid cysts,dermoid cysts and enteric cysts.Although the diagnosis is possible with the diagnostic imaging methods,the definitive diagnosis is made by the histopathological examination.In our case,a congenital retrorectal cyst and intra-abdominal free air due to perforation was detected.The cystic mass regressed spontaneously after abdominal surgery.

CASE:A female infant,born by caesarean section at 37 weeks,weighing 3150 gr, had postnatal abdominal distension,hyperemia in the abdomen and a cystic mass of 4x3 cm the right gluteal region. The abdominal X-ray revealed right diaphragmatic free air and air-fluid levels.In the ultrasonography(USG) to moderate free fluid with an intensive content was seen intra-abdominally.The patient was operated with the diagnosis of intestinal perforation.The intestinal contents were encountered when the abdominal peritoneum was opened.No focus of perforation was found by examination from the stomach to the rectum.The oral feeding was started on the 5th post-operative day.The ultrasound and magnetic resonance imaging(MRI) of the gluteal mass revealed a smooth cystic mass of approximately 85x32x27mm in the presacral area,at the right side of the anus and the rectum and extending to the perineum,with a surrounding wall formation,without a significant solid component;and a second cystic lesion in the direct neighborhood,localized between the gluteus maximus and medius muscles on the right side,approximately 38x18x12mm in size,with a surrounding wall and incomplete septa formation,with similar characteristics as the previous cyst were observed.ß-HCG serum levels were <1.20U(Normal) and the AFP serum levels were 12,360U(Normal). During the control visit 10 days later,the USG of the gluteal region did not reveal the cystic lesions which were described in previous investigation,within the gluteal muscles and in the adjacent region.In this localization,a tubular mass of 22x8mm was visualized,with a dense content.The regular physical examination and ultrasound checks of the patient at frequent intervals revealed a regression of the cystic structure,which disappeared completely,when the patient was 3 months old.Control AFP levels were 270U. The patient is currently 10 months old without any problems,and is followed-up by imaging examinations.

DISCUSSION: In the sacrococcygeal region,partly complex,many malformations can be seen.The retrorectal developmental cysts include epidermal,dermoid,tailgut,mucus-secreting cysts,enteric duplications,neuroenteric cysts and cystic teratoma.The recommended treatment for the developmental cysts is surgical excision,in case of recurrence,malignant degeneration and chronic infection risk.The cystic mass in our patient has regressed spontaneously during follow-up,without any surgical excision.

REGRESE OLAN SAKROKOKSİGEAL KİTLE OLGUSU

Amaç: Çocukluk yaş grubunda retrorektal kistik lezyonlar nadirdir ve çoğunluğu konjenitaldir. Rektorektal bölgede yer alan konjenital oluşumların çoğu teratomlar ve gelişimsel kistlerdir; bunlar epidermoid kistler,dermoid kistler ve enterik kistlerdir. Tanı görüntüleme yöntemleri ile konabilmekle birlikte kesin tanı histopatolojik inceleme ile konulur. Olgumuzda konjenital retrorektal kisti ve bu kiste bağlı perforasyon nedeni ile intraabdominal serbest hava saptandı. Abdominal cerrahi sonrası kistik kitle spontan regrese oldu.

Olgu: Sezeryan ile 37 hafta 3150 g ağırlığında doğan kız bebek postnatal karında distansiyon ve karın cildinde hiperemi, sağ gluteal bölgede 4x3 cm boyutlarında kistik kitle mevcuttu. Direkt karın grafisinde sağ diafragma altı serbest hava ve görünümü hava sıvı seviyelenmeleri, ultrasonografide (USG) intraabdominal orta derecede, yoğun içerikli serbest sıvı görüldü intestinal perforasyonu tanısıyla opere edildi. Periton açıldığında karın içinde barsak içeriği görüldü. Mideden rektuma kadar yapılan incelenmede herhangi bir perforasyon odağı olmadığı görüldü. Ameliyat sonrası 5. gün oral beslenmeye başlandı. Gluteal kitlesine yönelik yapılan USG ve magnetik rezonans görüntülemede (MR) presakral mesfede rektum ve anüs sağ tarafında perineye de uzanan yaklaşık 85x32x27 mm boyutlarında, etrafında duvar formasyonu izlenen, belirgin solid komponenti bulunmayan, düzgün konturlu kistik kitle, yine sağda gluteus maksimus ve medius kasları arasında lokalize, yaklaşık 38x18x12 mm boyutlarında, etrafında duvar formasyonu bulunan ve inkomplet septa içeren, önceki kistle benzer özellikte ve direk komşuluk gösteren ikinci bir kistik lezyon izlendi. ß-HCG: <1.20 (N), AFP:12,360(N). 10 gün sonra kontrole gelen hastanın gluteal bölgesinde olan kitlesi için yapılan USG’de önceki tetkiklerde tarif edilen gluteal bölgede kas içerisinde ve hemen komşuluğunda tarif edilen iki adet kistik lezyon bu tetkikte görülemedi. Bu lokalizasyonda 22x8 mm boyutlarında, yoğun içerikli tübüler görünüm izlendi. Hastanın sık aralıklarla yapılan muayene ve USG kontrollerinde kistik yapının azaldığı, hasta 3 aylık olduğunda ise tamamen regrese olduğu görüldü. Kontrol AFP:270 geldi. Hasta şu anda 10 aylık olup kontrol muayeneleri ve görüntüleme yöntemleri ile herhangi bir sorunu olmadan takip edilmektedir.

Tartışma: Sakrokoksigeal bölgede bir kısmı kompleks, çok sayıda malformasyon izlenebilmektedir. Retrorektal gelişimsel kistler içerisinde epidermal, dermoid, tailgut, mukus salgılayan kistler, enterik duplikasyonlar, nöroenterik kistler ve kistik teratomları içerir. Gelişimsel kistler için önerilen tedavi nüks, malign dejenerasyon ve kronik enfeksiyon riskleri nedeni ile cerrahi eksizyondur. Hastamızda bulunan kistik kitle takipler esnasında spontan regrese olup cerrahi eksizyon yapılmamıştır.
