NEUROBLASTOMA WITH AN INTERESTING LOCALIZATION

Aim: The sympathetic neuroblastoma is a tumor, derived from primordial neural crest cells in the adrenal medulla or sympathetic ganglia,and it comprises only 8-10% of all childhood cancers.Clinical picture depends on the location of the primary tumor and the presence of metastasis. In our patient,there is a duodenal invasion of neuroblastoma,and being located between the stomach and the pancreas,the tumor had led to upper gastrointestinal (GI) bleeding.

Case: A 5-year-old female patient had admitted to the hospital with abdominal pain, where investigation had revealed a neuroblastoma.After the first two cycles of chemotherapy,she developed an upper GI bleedind.After the bleeding was under control she was given radiotherapy for a period of 2 days.The patient was evaluated for surgical excision.The physical examination revealed a cachectic abdomen, with a hard, immobile palpable mass with a smooth surface,approximately 14x13 cm in the midline.The cytological preparations were assessed by the department of pathology in our hospital and primarily a malignant round cell tumor,most probably neuroblastoma was considered.The computed tomography and the mesenteric angiography have showed a solid bi-lobulated mass containing necrotic areas, in close neighborhood with the liver,stomach and the small intestines on the left, extending over a length of 158 mm from caudal to cranial.The mass showed infiltration to the first portion of the duodenum, with a necrotic component the level of the antrum.The intrabdominal mass caused compression of the hepatic artery,had a vascular structure originating from the splenic artery, and the proximal part of the portal vein was infiltrated by the mass. Adjacent to the mass, metastatic lymph nodes were observed, size of the largest appeared to be 11x7.5mm. The patient was operated on due to persistent bleeding, atypical location, definitive pathological diagnosis, and lack of downsizing despite chemotherapy and radiotherapy.A mass was found behind the stomach, above the pancreas,and on the transverse colon.The mass was isolated and removed by sharp and blunt dissection cranially from the stomach,laterally from the splenic artery and vein inferiorly from the mesentery of the transverse colon,posteriorly from the superior mesenteric vein and splenic vein,laterally from the wall of the duodenum on the right,posteriorly from the head of the pancreas. The mass had also invaded the duodenum.Where the tumor had invaded the duodenal wall, a resection of the duodenal wall was performed with a primary repair.The patient is symptom-free at the post-discharge controls.

Discussion: Due to the variability in the biological behavior of the tumor, spontaneous regression or benign transformation (change to ganglioneuroma) of neuroblastoma can occur especially in patients under 1 year of age,whereas the course can be more aggressive in patients over 1 year of age, leading to problems in the treatment of this condition and prognosis of the disease. Neuroblastoma can be localized everywhere, however, the gastrointestinal tract invasion with upper GI bleeding is quite noteworthy.In our patient, the neuroblastoma has had an aggressive course.Depending on the duodenal invasion,the neuroblastoma has resulted in abundant upper gastrointestinal bleeding,and postoperatively the symptoms have been cured. 

İLGİNÇ YERLEŞİMLİ NÖROBLASTOM

Amaç: Nöroblastom sempatik adrenal medulla veya sempatik ganglionlarda normalde bulunan primordial nöral krest hücrelerinden köken alan ve tüm çocukluk çağı kanserlerinin %8-10’unu oluşturan bir tümördür.Klinik bulgular primer tümörün yerleşim yerine ve metastaz varlığına göre ortaya çıkar.Hastamızda nöroblastom duodenum invazyonu yapmakta ve üst gastrointestinal (GİS) kanamasına yol açan mide ile pankreas arasında yerleşim göstermektedir.

Olgu: Beş yaşında kız hasta,karın ağrısı şikayeti ile ilk başvurduğu hastanede yapılan tetkiklerinde nöroblastom saptanmış 2 kür kemoterapi sonrası üst GİS kanaması olmuş, kanama kontrolü sonrası 2 gün süre ile radyoterapi almış. Hasta cerrahi eksizyon açısından konsülte edildiğinde fizik muayenesi keşektik,karın orta hatta yaklaşık 14x13cm sert,düzgün,yüzeyli,immobil kitle ele geliyordu. Sitolojik preparatları hastanemiz patoloji bölümü tarafından değerlendirildi ve malign yuvarlak hücreli tümör, öncelikli nöroblastom lehine düşünüldü. Hastaya yapılan bilgisayarlı tomografi (BT) ve mezenterik anjiografide ‘Karaciğer,mide ve solda ince barsaklarla yakın komşuluk gösteren içerisinde nekrotik alanlar içeren solid bilobüle kitle süperiordan inferiora 158 mm'lik bir segmentte uzanım göstermekte, kitlenin duodenum 1. kısım düzeyine infiltrasyon gösterdiği, bu antrum düzeyindeki komponent nekrotik olarak izlenmekteydi. İntrabdominal kitlenin ana hepatik artere bası yaptığı, kitle içerisinde splenik arterden orijin alan vasküler yapının dallarının uzanım gösterdiği dikkati çekmekle beraber portal ven  proksimal kesimi düzeyinde kitle tarafından infiltre olarak izlenmekteydi. Kitle komşuluğunda en büyüğü 11x7.5mm boyutlarında metastatik lenf nodları görünmekteydi. Hasta kanamanın devam etmesi,yerleşimin atipik olması,patolojik olarak kesin tanı olması ve kemoterapi ve radyoterapiye rağmen küçülme olmaması nedeniyle opere edildi. Mide arkası,pankreas üzeri,transvers kolon üzerinde kitle görüldü.Kitle yukarıda mideden, lateralde splenik arter ve ven üzerinden, inferiorda transvers kolon mezosundan, posteriorda superior mezenterik ven ve splenik ven üzerinden, sağda lateralde duodenum medial duvarından, sağ posteriorda pankreas başı üzerinden keskin ve künt diseksiyonlar ile izole edildip çıkartıldı. Kitle duodenuma invazeydi,invaze kısımda duodenum duvarı ile çıkarıldı,açıklık primer onarıldı. Hastaya postoperatif 6.gün nazogastrik sondadan opak verilerek duodenumdan kaçak olmadığı görüldü. Oral beslenmesinde sıkıntısı olmayan hasta postoperatif 8.gün sorunsuz taburcu edildi. Taburculuk sonrası yapılan kontrollerinde yakınması bulunmamaktadır.

Tartışma: Tümörün biyolojik davranışındaki değişkenlik nedeni ile, nöroblastoma özellikle 1 yaş altındaki olgularda spontan regresyon veya benign transformasyon (ganglionöroma’ya değişim) gösterebilirken,1 yaş üzerindeki hastalarda daha agresif seyir gösterebilmekte, bu durum hastalığın prognozunu belirlemede ve tedavisinde sorunlara yol açmaktadır. Nöroblastom her yerde görülebilmekle birlikte alışıldık yerler dışında yerleşim gösterebilir. Hastamızda da nöroblastom agresif seyir göstermiştir, duodenum invazyonuna bağlı olarak abondan üst GİS kanamaya sebep olmuş, cerrahi sonrası ise semptomlarda kür sağlanmıştır.
