A VERY RARE LESION

INTRODUCTION: Cutaneous bronchogenic cysts (CBC) are rare lesions that originate from primitive tracheobronchial tree. These lesions locate generally intrapulmonary and the most common extrapulmonary location is mediastinum. Other extrapulmonary locations are lingua, intraabdominal and cutaneous regions. Lesions that located over scapula subcutaneously are the rarest type of CBC.

We reported a bronchogenic cyst localized on the scapular region subcutaneously and reviewed clinical properties and previous cases in this report.

CASE REPORT: A four years old boy was brought to our clinic with complaint of growing swelling during last few months at his back on his right scapula. That swelling was painless and 2x2 cm in dimensions. All the other system examinations were normal. In USG examination the swelling was reported as a cystic lesion with high dense fluid content and 20x11 mm in dimensions and 2 mm far from epidermal surface in depth. Then, patient was evaluated with CT scanning. There was a mass, 26x15 mm in dimensions, on right scapular region nearly located to bone structures subcutaneously and there were no enhancement after IV radioopaque solution injection. We operated the patient with a pre-diagnosis of cystic lesion. The swelling was excised through an transvers incision dimensions of swelling were 20x15 mm. There was a connection to spine of scapula with a fibrotic band. The patient was discharged after three-hour observation. It was seen that pseudostrafied ciliated epithelium lined cyst wall and lymphoid cell infiltration with germinal centers under the epithelium in the histopathological examination of the cyst. Thus, the swelling was reported as bronchogenic cyst. His postoperative follow-up period was uneventful.

CONCLUSION:Cutaneous bronchogenic cysts are very rare masses and mostly seen in childhood because of their congenital origin. Number of described cases is approximately 70 in literature. It could be said that, in conclusion, bronchogenic cysts should be in mind if there is a cutaneous cystic lesion, especially in children and must be excised surgically.

OLDUKÇA NADİR BİR LEZYON

GİRİŞ: Kutanöz bronkojenik kistler primitif trakeobronşiyal ağaçtan köken alan nadir lezyonlardır. Bu lezyonlar genellikle akciğer içinde yerleşirler ve en sık akciğer dışı yerleşim mediastendir. Diğer akciğer dışı yerleşim yerleri dil, karın içi ve kutanöz bölgelerdir. Bu çalışmada skapular bölgede subkutan yerleşimli bir bronkojenik kisti bildirmek ve geçmiş vakaları incelemek istedik.

VAKA SUNUMU: Dört yaşındaki bir erkek çocuk sağ skapulasının üzerinde cilt altı dokuda son birkaç ayda büyüyen şişlik şikayeti ile polikliniğimize getirildi. Şişlik 2x2 cm boyutlarında ve ağrısızdı. Diğer sitem muayeneleri normaldi. Ultrasonografik incelemede yoğun içerikli sıvı içeren 20x11 mm boyutlarında ve epidermal yüzeyin 2mm derininde yer alan kistik kitle bildirildi. Daha sonra çekilen BT görüntülerinde ise 26x15 mm ölçülerinde sağ skapular bölgede yer alan subkutanöz yerleşimli, diğer kemik yapılara yakın ve kontrast madde enjeksiyonundan sonra değişiklik göstermeyen kistik kitle tespit edildi. Hasta kistik kitle öntanısı ile ameliyata alındı. 20x15 mm ölçülerindeki kitle transvers bir insizyon ile çıkarıldı. Kitle skapulaya fibrotik bir bant ile bağlı idi. Hasta üç saatlik bir gözlemden sonra taburcu edildi. Kitlenin histopatolojik incelemesinde kist duvarında yalancı çok katlı silialı epitelyum olduğu ve epitelyum altında germinal merkezleri olan lenfoid infiltrasyonlar vardı. Bu sebeple kitle bronkojenik kist olarak rapor edildi. Ameliyat sonrası dönemde herhangi bir sorun ortaya çıkmadı.

SONUÇ:Kutanöz bronkojenik kistler oldukça nadir kitlelerdir ve konjenital olduklarından dolayı çoğunlukla çocukluk çağında görülürler. Literatürde bildirilen vaka sayısı yaklaşık 70’ tir. Özellikle çocuklarda kutanöz kistik kitle varlığında bronkojenik kistler ön planda yer almalı ve mutlaka cerrahi olarak çıkarılmalıdır.
