A Rare Congenital Anomaly: Congenital Prepubic Sinus

 

Aim:

Prepubic sinus is a rare congenital anomaly of the urinary tract, which has controversial ethiology.  It’s clinically presented as discharge from an opening overlying the syphisis pubis. The track originates from  skin over pubic symphisis, superior to the base of penis or clitoris and it extends to bladder but not communicates with bladder.Aim of this study is to present a boy with Prepubic sinus and to discuss the embryologic, diagnostic and therapeutic aspects of this rare anomaly.

Case:

 an eleven years old boy admitted to our clinic with history of intermittant purulat discharge from a midline orifice superior to penis. Results of laboratory examinations were normal. Excision of the 6 cm sinus extending from the skin overlying the pubis to the anterior surface of the bladder was carried out. Histological examination revealed that the sinus was surrounded by transitional epithelium and mononuclear inflamatory cells.There were no postoperative complications.

Conclusion:

 In Stephens’ classification three forms of dorsal duplication are described.(i) a complete or incomplete channel running paralel to normal urethra from the glans to the bladder. (ii) an epispadiac type (iii) dermoid sinus-like type. Due to it’s anatomical location and histology of epithelium our patient fits type 3 form of urethral duplications.We believe complete excision is the most definitive procedure for definate cure.

Nadir Görülen Konjenital Bir Anomali: Konjenital Prepubik Sinüs

 

Amaç:

Prepubik sinüs etyolojisi tam olarak bilinmeyen, çok nadir görülen, konjenital bir anomalidir. Kliniğini pubik bölge üzerinde bulunan bir açıklık ve bu açıklıktan kaynaklanan akıntı oluşturur.  Orta hatta Pubik simfizisin hemen üzerinde başlayan kanal mesaneye kadar uzanım gösterse de mesane ile bağlantısı bulunmaz. Burada tedavisini yaptığımız bir prepubik sinüs’lü erkek çocuğu sunmak ve nadir görülen bu anomalinin embriyoloji, tanı ve tedavisine ait yönlerini tartışmak amaçlanmıştır.

Olgu:

Onbir yaşında erkek çocuk, prepubik bölgede, orta hatta penis kökü dorsalinde 2-3 mm’lik anormal açıklık ve bu açıklıktan zaman zaman olan sarı renkli akıntı şikayeti ile getirildi. Laboratuar incelemelerinde özellik yoktu. Prepubik sinüs ön tanısı ile ameliyat edilen hastada pubis üzerindeki cilt seviyesinden başlayıp mesanenin ön yüzüne kadar uzanan, yaklaşık üç cm kateterize edilebilen sonrasında fibrotik ve kör sonlanan altı cm uzunluğundaki sinüs traktı total olarak eksize edildi. Hasta aynı gün taburcu oldu ve postoperatif izlemde sorun yaşanmadı. Histolojik incelemede transizyonel epitelle döşeli, hafif derecede mononükleer iltihabi hücrelerle infiltre sinüs traktı görüldü.

Sonuç:

Prepubik sinüsün emriyolojisi tartışmalıdır, üretra duplikasyonlarının bir çeşidi olarak da tanımlanır.  Stephens’ın dorsal üretral duplikasyon sınıflamasına bakıldığında 3 tip duplikasyon vardır. Bunlar; (I) glanstan mesaneye kadar uzanan komplet ya da inkomplet tip (II) epispadiak tip (III) dermoid sinüs şeklinde olan tip. Anatomik yerleşimi ve histolojik özellikleri dolayısıyla bizim olgumuzu tip 3 grubuna dahil edebiliriz. Bu vakalarda kör sonlanan sinüs traktının tam olarak çıkartılmasının tam bir kür sağlayacağını düşünmekteyiz.
