A Case Of Gastric Duplication Presenting With Gastric Perforation

 

Aim:

Duplication anomalies of the gastrointestinal system are rare and the stomach is the least expected site of occurence. Gastric duplication can be in a tubular or cystic form and no connection is found between the duplication and the stomach lumen when it is cystic. These anomalies may remain asymptomatic over years. When symptomatic, they present with a vague abdominal pain, nasuea, vomiting, hematemesis or melena. In this report, we present a case of gastric duplication which became symptomatic with gastrointestinal bleeding and resulted with perforation.

Case:

A three month – old infant with the symptoms of hematemesis and melena was admitted to our clinics and hospitalized. Her history revealed that she had been hospitalized for gastrointestinal bleeding before and no source of bleeding had been found. Upon admission, she received a gastroprotective treatment. Gastroscopy showed a bleeding focus which was intially thought to represent an arteriovenous malformation. Scintigraphic work up was negative for any bleeding source. Her barium study was normal except for a gastroesophageal reflux finding. A computed tomography (CT) of the abdomen revealed a 25x35 mm mass inside the stomach lumen. Patient was consulted with oncology department and her tumor markers was negative. A repeated endoscopy  was  also negative except for the bleeding source.  A diagnosis of duplication cyst was made and ultrasound findings were consistent. Laparotomy revealed a closed perforation of the anterior wall of the stomach and a 2x3 cm duplication cyst just lateral to perforation site. Cyst was excised and perforation was repaired. Pathologic examination showed pancreatic tissue inside of the cyst. Patient was discharged on the 5th postoperative day.

Conclusion:

Duplication of the stomach is a rare condition and its symptoms are non-specific. In our case, initial misinterpretation of the cyst as an intraluminal mass and misinterpretation of the bleeding source found on gastroscopic examination caused a delay in diagnosis and this delay resulted in a gastric perforation. Because it was a closed perforation, findings of peritonitis were absent. There are reports on the perforation of the duplication cyst in the literature but perforation of the stomach is not reported. In our case, perforation is not thought to be asssociated with the location of the cyst on stomach wall. Because the patient’s first symptom was gastrointestinal bleeding, it is assumed to be associated with a gastric ulcer which is developed as a result of hormonal stimulation.

Mide Perforasyonu İle Seyreden Gastrik Duplikasyon Olgusu

 

Amaç:

Duplikasyonlar, gastroinetstinal sistemin en nadir görülen anomalisidir ve en az görüldüğü yer midedir.  Mide duplikasyonları tubuler veya kistik olabilir ve kistik olduklarında lümenle ilişkileri bulunmaz. Uzun yıllar sessiz kalabilecekleri gibi, müphem karın ağrısı, kitle, bulantı, kusma, hematemez veya melana şeklinde kanama belirtileri verebilir.  Burada gastroinestinal kanama ile belirti veren ve mide perforasyonu ile sonuçlanan bir gastrik duplikasyon olgusu sunulmuştur.

 Olgu:

Daha önce GİS kanaması nedeniyle yatırılıp kanama nedeni bulunamamış üç aylık kız hasta hematemez ve melena şeklindeki kanama şikayetiyle tekrar çocuk kliniğine yatırıldı. Hastaya gastroprotektif tedavi başlandı ve gastroskopide midede A-V malformasyon olarak düşünülen kanama odağı görüldü. Yapılan kanama sintigrafisinde ise özellik saptanmadı, ÖMD incelemesinde GÖR dışında patoloji yoktu, BT de mide lümeni içerisine büyümüş 25x35 mm büyüklüğünde kitle görüldü. Hasta onkolojiye danışıldı ve istenilen tümör belirteçleri normal bulundu. BT sonucuna göre tekrarlanan endoskopide de kanama odağı dışında özellik görülmedi. Bu aşamada kliniğimize danışılan hastanın duplikasyon kisti olabileceği söylendi ve yapılan USG incelemesi bu öntanıyı doğruladı. Hasta kliniğimize alındı ve laparotomide mide ön duvarında kapalı perforasyon, bunun lateralinde 2x3 cm boyutlarında duplikasyon kisti görüldü. Kist tam eksize edildi ve perforasyon primer onarıldı. Patolojik incelemede kist içerisinde pankreas dokusu görüldü. Hasta PO beşinci gün sorunsuz olarak taburcu edildi.

Sonuç:

Mide duplikasyonlarının az görülmesi ve ortaya çıkan belirtilerin özgül olmaması sebebiyle ayırıcı tanıda arka sıralarda yer almaktadır. BT incelemesinde kistin intralüminal yerleşimli kitle olarak değerlendirilmesi ve gastroskopide kanama odağının yanlış yorumlanması tanıya giden yolu uzatmıştır. Duplikasyon kisti şüphesinde noninvazif bir yöntem olan USG daha değerlidir ve öncelik verilmelidir. Olgumuzda tanıdaki gecikme gastrik perforasyon ile sonuçlanmıştır ve kapalı perforasyon nedeniyle genel peritonit bulguları oluşmamıştır. Literatür araştırmasında mide duplikasyonlarında, kistin perforasyonu bildirilmiş olmasına rağmen mide perforasyonu olgusuna rastlanmamıştır. Mide perforasyonunun,  kistin mide üzerindeki yerleşimi ile doğrudan bir ilişkisi olmadığı düşünülmüştür. Başlangıç bulgusunun GİS kanaması olması dolayısıyla perforasyonun hormonal mekanizmalarla oluşmuş bir ülser zemininden gelişmiş olabileceği düşünülmüştür.
