NEONATAL GASTRIC VOLVULUS: A RARE CAUSE OF NEONATAL FEEDING INTOLERANCE

 

Aim of the study:

We aimed to present two cases with neonatal gastric volvulus that have long term feeding intolerance.

 

Methods:

Retrospective analysis of two patients with neonatal gastric volvulus was performed. Diagnosis was confirmed by radioopaque studies of stomach. Data including age, reason of hospitalization, type of operation and postoperative results were reviewed.

 

Results:

Two patients (2 M) with neonatal gastric volvulus were operated in 41. and 52. days. Reasons for hospitalization were prematurity and persistent vomitting. Optimal feding couldn’t be achieved despite of long term feeding efforts. No abnormalities were seen at direct radiograms. So, rapiopaque radiological study was performed for long-term feedig intolerance. Stomach corpus was seen as be curled above to fundus. At operation, major curvature of stomach wasn’t fixed due to gastrocolic ligament degiciency and curled above to fundus. There wasn’t seen hiatal hernia. Great curvature was fixed to abdominal wall by anterior gastropexy. Postoperative first week enteral feding was begun and second week full dose feding was accomplished. Postoperative follow-up periods are 4 and 6 months and uneventfull.

 

Discussion:

Long- term feeding intolerance with normal direct radiological studies can be seen in some infants. At these babies neonatal gastric volvulus due to gastrocolic ligament deficiency must be keep in mind. Diagnosis can be done by radiopaque esophagus- stomach- duodenum graphy. Anterior gastropexy should be preferred due to low morbidity and high efficiency.

NEONATAL GASTRİK VOLVULUS: YENİDOĞAN DÖNEMİ BESLENME GÜÇLÜĞÜNÜN NADİR BİR NEDENİ

 

Amaç:

Uzun süre beslenme güçlüğü çeken yenidoğanlarda, nadir bir neden olan gastrokolik ligaman yokluğuna bağlı gastrik volvuluslu iki olgunun sunulması amaçlanmıştır.

 

Yöntem:

Neonatal gastrik volvulus saptanan iki olgunun kayıtları incelenmiştir. Tanı opaklı özofagus- mide- duodenum grafisi ile konmuştur. Hastaların operasyon yaşı, yatış sebebi, yapılan operasyon ve postoperatif sonuçlar değerlendirilmiştir.

 

Bulgular:

Neonatal gastrik volvulus saptanan iki olgunun (2 E) operasyon günleri 41. ve 52. gündür. Hastaların biri prematürite diğeri ise dirençli kusma nedeniyle kliniğimize yatırılmıştır. İzlemlerinde enteral beslenme denenmesine rağmen uzun süre yeterli beslenmeye geçilememiştir. Ayakta direkt batın grafilerinde anormallik saptanmamıştır. Uzun süre beslenememeleri nedeniyle opaklı özofagus- mide- duodenum (ÖMD) grafisi çekilmiştir. ÖMD grafisinde mide büyük kurvaturunun fundus üzerine doğru katlanmış olduğu gözlenmiştir. Cerrahi planlanan olguların abdominal eksplorasyonunda, gastrokolik ligaman yetersizliğine bağlı midenin korpusunun fiske olmadığı ve fundus üzerine katlandığı saptanmıştır. Hiatus hernisi gözlenmemiştir. Mide büyük kurvaturu anterior gastropeksi ile batın ön duvarına fiske edilmiştir. Her iki hastada operasyon sonrası 1. hafta enteral beslenme başlanmış ve postoperatif 2. haftada tam doz beslenmeye geçilmiştir. Hastaların postoperatif izlem süreleri 4 ve 6 aydır. Sorunsuz takiplerine devam edilmektedir.

         

Sonuç:

Uzun süre beslenme güçlüğü çeken ancak direkt radyolojik tetkikleri normal olan yenidoğanlarda, gastrokolik ligaman yetersizliğine bağlı neonatal gastrik volvulus akılda tutulmalıdır. Tanı özofagus- mide- duodenum grafisi ile konulabilmektedir. Cerrahi tedavi yöntemi olarak anterior gastropeksi düşük morbidite ve yüksek etkinliği nedeniyle öncelikle tercih edilmelidir.
