UNDESCENDED TESTICLE TREATMENT WITHOUT SCROTAL INCISION

Objective: We aimed to discuss whether some patients with undescended testicle could be kept away from scrotal incision with a little modification in standard inguinal orchiopexy operation. 

Materials and Methods: A total of 54 child patients (62 testes), who have undergone surgical operation for undescended testicle treatment in our clinic between 2009 and 2012 were evaluated. 

Results: In surgical operation approach, the following methods were used: Of 54 patients, 49 were performed standard inguinal orchiopexy, 5 were undergone laparoscopic surgery, 4 were applied only scrotal incision and the remaining 4 patients were operated with only inguinal incision. In four patients (7.4 %), the operation was finished via performing only inguinal incision without scrotal incision, however this method has not been planned prior to the operation. The only difference of this approach from the standard inguinal orchiopexy is that there is no scrotal incision in this method. If the surgical technique can briefly be summarized: The testes of the patients were releazed from the surrounding tissues just as in the standard orchiopexy and herniation sacs were repaired. The scrotum was passed through the inguinal incision via inguino-scrotal way and the testicles were fixated to the dartos fascia of the scrotum. After this step, the scrotum and testicles were located in normal anatomical position. In long term follow-ups of these patients with an average of two years, it has been observed that there was no problem in testicular volumes in USG. Also, there was no dislocation of the fixated testes in upper direction. Furthermore, it was very satisfying in cosmetical and appereance aspects that there was only one incision site.    

Discussion: Inguinal orchiopexy operation without scrotal incision can not be applied to all patients with undescended testicle. It can only be performed in case of the patients with large scrotum and sufficient testicular volume, in whom the testes can be easily pulled towards the scrotal location without any distention.  

SKROTAL KESİ YAPILMADAN İNMEMİŞ TESTİS ONARIMI

Amaç: Amacımız; standart inguinal orşiopeksi ameliyatında yapılacak küçük bir modifikasyonla bazı inmemiş testisli hastaların skrotal kesiden kurtarılabileceğini tartışmaktır…

Gereç ve Yöntem: Kliniğimizde 2009 ile 2012 yılları arasında inmemiş testis tanısıyla cerrahi olarak tedavi edilen toplam 54 çocuk (62 testis) değerlendirildi.

Bulgular: Olgulara cerrahi yaklaşım olarak; 49’una standart inguinal orşiopeksi, 5’ine laparoskopi, 4’üne sadece skrotal kesi ve 4’üne ise sadece inguinal kesi yapıldı. Hastalardan 4 tanesine (%7.4) skrotal kesi yapılmadan sadece inguinal kesi yapılarak ameliyat tamamlandı. Hastalara ameliyat öncesi böyle bir yöntem yapılması planlanmadan ameliyatları yapıldı. Bu hastalara uygulanan ameliyat tekniğinin standart inguinal orşiopeksiden tek farkı skrotal kesi yapılmamasıdır. Kısaca ameliyat tekniğinden söz edilecek olursa; hastaların testisleri standart inguinal orşiopeksideki gibi etraf dokulardan serbestlenip fıtık keseleri onarıldı. İnguino-skrotal yolla skrotum inguinal kesiden doğurtuldu. Testis doğurtulan skrotumun dartos fasyasına fikse edildi. Sonra dikkatli bir şekilde skrotum testisle birlikte normal anatomik pozisyonuna getirildi. Bu hastaların uzun dönem (ortalama 2 yıl) takiplerinde testis volümlerinde sonografik olarak herhangi sorun olmadığı görüldü. Testisin skrotuma tespiti ile ilgili olarak yukarı kaçma gibi bir sorun da gözlenmedi. Ayrıca tek insizyon olması kozmetik görünüm açısından oldukça tatminkardı. 

Tartışma: Skrotal kesisiz inguinal orşiopeksi her inmemiş testisli hastaya uygulanamaz. Ameliyat sırasında testis volümü yeterli olan, skrotuma gergin olmadan çok rahat bir şekilde indirilebilen ve skrotumu geniş olan hastalara uygulanabilir.
