Percutaneous Endoscopic Gastrostomy; Experience of two clinics

Objective:Percutaneous endoscopic gastrostomy catheter (PEG) is the most common and most simple, minimally invasive method in gastrostomy methods which has been used in China. This procedure can be applied with local anesthesia and sedation in older children. So, we decided to evaluate the patients who underwent PEG at our clinics.

Material and methods: Between January 2010 - January 2012, 13 patients who underwent PEG tube in two pediatric surgery centers were retrospectively evaluated.

Results:There were 7 female (%54), 6 male (%46) patient. The mean age was 4,2 years  for girl, 2,6 years for boy and 3,5 years for all patients. The oldest patient was 13 years old girl and the smallest patient was 11 month old boy. The mean duration of hospital stay was 3 ± 1 days (3.5 ± 1 days in girls and 3 ± 1 days in boys). One boy who was followed due to intracranial mass in intensive care unit (duration of hospitalization 5 months) and one boy who was hospitalized due to esophageal atresia and other reasons (duration of hospitalization 40 days) were excluded from the analyses of hospital stays. The mean time to start feeding  was 31 ± 8 hours, the mean operation time was 45 ± 15 minutes. There was no significant difference in feeding time between girls and boys. Indication for PEG insertion was swallowing dysfunction due to neurological condition in 12 patients and swallowing dysfunction after repair of esophageal atresia in 1 patient. A patient with hypoxic ischemic encephalopathy who had aspiration pneumonia after PEG insertion and 3 patients who underwent esophageal atresia repair, had undergone antireflux surgery. A patient who underwent esophageal atresia repair had undergone esophageal dilation for esophageal anastomotic stricture and oral feeding was performed. PEG feeding was performed in other 12 patients.

Conclusion: PEG is a technically feasible method. PEG may increase the rate of gastroesophageal reflux.  We have performed gastroesophageal reflux analysis before in our  patients. PEG could be performed without invasive analysis and surgery. Invasive analysis and surgery may be done later.

Perkütan Endoskopik Gastrostomi; İki Kliniğin Deneyimi

Amaç: Perkütan endoskopik gastrostomi kateteri ( PEG ) gastrostimi yöntemleri çinde en sık ve en basit uygulanan minimal invaziv yötemdir. Büyük çocuklarda lokal anestei ve sedasyon ile de uygulanabilen bir prosedürdür. Bu nedenle kliniklerimizde PEG uygulanan olgular değerlendirildi.              

Gereç ve Yöntem:Ocak 2010 – Ocak 2012 tarihleri arasında iki çocuk cerrahisi merkezinde perkutan endoskopik gastrostomi kateteri takılan 13 hastanın geriye dönük incelenmesi.

Bulgular:Hastaların 6’sı erkek ( % 46 ), 7’si kız ( % 54 ) idi. Yaş ortalaması kızlarda 4,2  erkeklerde 2,6 yıl, tüm hastalar alındığında ise 3,5 yıl idi. En büyük hasta kız ve 13 yaşında iken en küçük hasta erkek ve 11 aylıktı. Hastaların ortalama hastane yatış süresi 3 ± 1 gündü. Yatış süreleri erkeklerde 3 ± 1 gün kızlarda 3,5 ± 1 gün idi. Yoğun bakım gereksimini olan ve kafa içi kitle sebebiyle halen yatırılarak takip edilen bir erkek hasta (yatış süresi 5 ay) ile özefagus atrezisi olan ve diğer sebeplerle 40 gün yatırılarak takip edilen erkek hasta yatış süreleri hesaplanırken grup dışı bırakıldı. Hastaların beslemeye başlama süreleri ortalama 31 ± 8 saat, operasyon süreleri ise ortalama 45 ± 15 dakika idi. Kız ve erkek hastalarda beslemeye başlama süreleri açısından fark yoktu. PEG takılma endikasyonların ise 1 hastada özefagus atrezisi onarımı sonrası yutma disfonksiyonu ve 12 hastada santral sinir sistemini patolojilerine bağlı yutma disfonksiyonuydu. Hipoksik iskemik ensefalopati tanılı, PEG sonrasında sık aspirasyon pnömonisi geçiren ve ayrıca özefagus atrezisi onarımı yapılmış 3 hastaya daha sonra antireflü ameliyatı yapıldı. Özefagus atrezisi nedeniyle opere edilmiş hastaların 1 tanesi anostomoz darlığı nedeni ile özofagus dilatasyon yapıldı ve yarar gördü ve oral beslenmektedir. Diğer 12 hasta operasyon sonrası komplikasyonsuz olarak PEG ile beslenmeye devam etmektedir.

Sonuç:PEG işlemi kolay uygulanabilir bir yöntemdir.  Olgularımızda başlangıçta imkanlar dahilinde gastroözofagial reflü araştırması yapılmıştır. Herhangi bir nedenle gastrostomi takılması gereken olgularda gastrostomi öcesi invaziv araştırma ve invaziv cerrahi yapmadan minimal invaziv yöntem olan PEG yöntemi ile gastrostomi açılıp daha sonra gerekirse detaylı inceleme ve invaziv cerrahi yapılabilir.
