DUODENAL ATRESIA IN ASSOCIATION WITH SITUS INVERSUS TOTALIS

 

Aim:Situs inversus is a rare condition causing mirror image positioning of abdominal and thoracic viscera with an estimated frequency of about one in 10000 of normal population. It is commonly associated with other serious cardiac and splenic malformations. Duodenal atresia is estimated to appear with an incidence of about one in 4000 to one in 15000 live births and about half of them have an associated congenital anomaly of another organ system.  The association of congenital duodenal obstruction and situs inversus is extremely rare, with only21 cases reported so far in the literature.  

Case:A 3-day-old full-term male baby weighing 2.7 kg was consulted with recurrent bile-stained vomiting. The babygram revealed dextrocardia with a reverse double bubble sign and no distal gas, echocardiogram was normal. The neonate underwent laparotomy on the 4 th day of life. The stomach and spleen were found on the right side while the liver was found on the left side. Patient had an uneventful recovery and was discharged in satisfactory condition.

Conclusion:Congenital duodenal obstruction is an uncommon condition.  The diagnosis of congenital duodenal obstruction in association with situs inversus is made easily on a plain babygram when a reverse double bubble sign is seen with reversed heart shadow. Confirmation of the diagnosis can be done by a contrast meal. Echocardiography to document the presence or absence of associated congenital cardiac anomalies is an important preoperative investigation. The treatment of congenital duodenal obstruction with or without associated situs inversus is the same. 

SİTUS İNVERSUS TOTALİS VE DUODENAL ATREZİ BİRLİKTELİĞİ

 

Amaç: Situs inversus, normal popülasyonda yaklaşık 10000’de bir tahmini sıklığı ile abdominal ve torasik organların ters tarafta(ayna görüntüsü) olmasıyla görülen nadir bir durumdur.  Bu, genellikle diğer ciddi kardiyak ve dalak malformasyonlar ile ilişkilidir. Duodenal atrezi yaklaşık 15000- 4000 canlı doğumda bir sıklık ile ortaya çıkar ve bunların yaklaşık yarısında başka bir sistem ile ilişkili konjenital anomali olduğu tahmin edilir. Konjenital duodenal obstrüksiyon ve situs inversus birlikteliği oldukça nadirdir, literatürde şimdiye kadar sadece 21 olguda bildirildi.

Olgu: 2.7 kg ağırlığında 3 günlük term erkek bebek tekrarlayan safralı kusma nedeniyle danışıldı. Ayakta direkt batın grafisinde ters bir çift hava kabarcığı işareti vardı, distalde gazı yoktu ve dekstrokardi saptandı. Ekokardiyografi normaldi. Yenidoğanın yaşamının 4. gününde laparatomi yapıldı. Karaciğer sol tarafda, mide ve dalak sağ tarafta bulundu. Duodenoduodenostomi yapıldı. Ameliyat sonrası hasta sorun yaşanmadan taburcu edildi.

Sonuç: Konjenital duodenal obstrüksiyon nadir bir durumdur. Situs inversus ile birlikte konjenital duodenal obstrüksiyon tanısı ayakta direkt batın grafisinde ters bir çift hava kabarcığı ve ters kalp gölgesi ile kolayca yapılır. Tanıya doğrulamak için kontrastlı çalışma yapılabilir, önemli bir preoperatif inceleme olan ekokardiyografi ile ilişkili doğumsal kalp anomalilerinin varlığı ya da yokluğu belirlenebilir. Situs inversus olan veya olmayan konjenital duodenal obstrüksiyonun tedavisi aynıdır.
