TRANSANASTOMOTIC FEEDING TUBE AFTER AN OPERATION FOR DUODENAL ATRESIA OR WEB

Purpose: To evaluate the effect of transanastomotic feeding tube (TAFT) on the transition time to enteral feeding, duration of transition to full-dose feeding, total parenteral nutrition (TPN) and hospital stay after an operation for duodenal atresia or web.

Methods: The files of 35 patients who underwent an operation for duodenal atresia/web between 2000-2012 were examined retrospectively. Patients who were inserted transanastomotic feeding tube were compared with those who did not in terms of the duration of transition to enteral feeding, transition to full-dose feeding, total parenteral feeding (TPN), and hospital stay.

Results: Of the 35 patients who underwent surgery 23 had duodenal atresia and 12 had duodenal web. TAFT was inserted in 18 patients during surgery but was not inserted in 17 patients at the discretion of different surgeons. Three patients were excluded from the study due to the dislodgement of the TAFT in early postoperative period. The duration of transition to enteral feeding, transition to full-dose feeding, TPN and hospital stay were significantly shorter in patients with TAFT than without TAFT (p<0.05, Mann-Whitney U test) (Table-1).  

Conclusion: The use of a transanastomotic feeding tube after an operation for duodenal atresia or web significantly shortens the duration of transition to start enteral nutrition, transition to full-dose feeding, TPN, and hospital stay.

DUODENAL ATREZİ/WEB AMELİYATI SONRASINDA TRANS-ANASTOMOTİK BESLENME SONDASI KULLANIMI

Amaç: Duodenal atrezi/web ameliyatından sonra yerleştirilen trans-anastomotik beslenme sondasının, enteral beslenmeye başlama zamanı, tam doz beslenmeye geçiş, total parenteral nutrisyon (TPN) ve hastanede kalış sürelerine etkisini araştırmak.

Gereç ve yöntem: 2000-2012 yılları arasında duodenal atrezi/web nedeniyle ameliyat edilen 35 hastanın dosyaları geriye dönük olarak incelendi. Trans-anastomotik beslenme sondası (TABS) yerleştirilen ve yerleştirilmeyen hastaların; enteral yoldan beslenmeye geçiş, tam doz beslenmeye geçiş, total parenteral nutrisyon (TPN) verilme ve hastanede kalış süreleri karşılaştırıldı.

Bulgular: Ameliyat edilen 35 hastanın 23’ünde duodenal atrezi, 12’sinde duodenal web mevcuttu. Cerrahın tercihine göre hastaların 18’ine ameliyat esnasında TABS yerleştirilirken 17’sine yerleştirilmedi. Post-operatif erken dönemde TABS yerinden çıktığı için 3 hasta çalışmaya dahil edilmedi. Enteral yoldan beslenmeye başlama, tam doz beslenmeye geçiş, TPN kullanma ve hastanede kalış süreleri TABS yerleştirilen hastalarda TABS yerleştirilmeyenlere göre istatistiksel olarak anlamlı derecede kısa idi (p<0.05, Mann-Whitney U test) (Tablo-1).

Sonuç: Duodenal atrezi/web ameliyatı sonrasında trans-anastomotik beslenme sondası kullanılması, enteral yoldan beslenmeye geçiş, tam doz beslenmeye geçiş, TPN kullanma ve hastanede kalış sürelerini anlamlı şekilde kısaltmaktadır.
