SUBLINGUAL MATURE TERATOMA

INTRODUCTION: Mature teratoma is benign and well differentiated tumor. Most common localizations are sacrocoxegeal region, over, abdomen and pelvis.

CASE: A 23 days old male presented with a sublingual mass in September 2001. CT examination revealed a submandibular lobular multicystic  mass located between submandibular gland and superior masticator region. The patient was operated . The pathologic examination of mass lesion revealed mature cystic teratoma. Thorax was found to be normal. Serum alfa fetoprotein level of the patient was measured as 79 ng/mL. The patients is followed  up nearly for 12 years and he is stil in remission.

DISCUSSION: Mature teratomas present in head and neck region in 1.6-6.9% of cases. In 0.01% of patients the tumor presents in oral cavity. Here we presented a very rare case presenting with mature teratoma located in sublingual region. Total exicion of the tumor is the effective treatment in order to diminish recurrence.

Giriş

Matür teratom benign ve iyi diferansiye bir tümördür.En sık sakrokoksigeal böge ve overde olmak üzere abdomen ,pelviste sık görülür.

Olgu

Vakamız 23 günlük erkek hasta Eylül 2001 ‘de çene altında şişlik şikayeti ile başvurdu.Preoperatif boyun BT de sağda submandibuler mesafede submandibuler bez ile SCM kası arasında yerleşmiş superiorda mastikatör mesafeye anterior ineriorda sublingual mesafeye uzanan lobule multikistik kitle lezyonu izlendi. Lezyonun ayırıcı tanısında kistik higroma ? düşünüldü. Hasta opere edildi. Patolojisi matür ksitik teratom ile uyumlu geldi. Toraks BT normal saptandı. AFP 79 ng/ml saptandı.Hasta halen Eylül 2001 den beri remisyonda izlenmektedir.

Tartışma

Matür teratomların baş ve boyunda görülme sıklığı %1.6-6.9 dur. Oral kavitede ise %0.01 den daha az sıklıkta görülür.Dilaltı lokalizasyon nadir görülür.. Kitlenin total çıkarılaması ile rekürrensler azalır.
