TIP AND TRIKS ON ANASTOMOS TECNIQUE IN ESOPHAGEAL ATRESIA: OBLIQUE-TO-OBLIQUE ANASTOMOSIS

BACKGROUND/AIM: Anastomotic stricture is an important problem after esophageal atresia repairing. Several predisposing factors have been implicated in the pathogenesis of stricture formation; among them, anastomosis tecnique plays a very important role in the prevention of strictures. Here, a basic anastomotic tecnique as oblique-to-oblique was described..

TECNICAL INNOVATION: In this tecnique, the distal trakeoesophageal fistula is cut from trachea vertically and edge of the fistula sutures with interrupted nonabsorbable 6/0 sutures as vertically. A 4-5 mm longitudinal incision is made on medial wall (left side) of freed lower esophageal end; this incision becomes left lateral on distal esophagus when viewed supine position.

After mobilisation of the proximal pouch; a traction suture is placed at the tip of upper pouch, and it opens with a 9-10 mm longitudinal incision from the tip along the lateral side of upper esophagus. If a 4 french endotracheal tube inserts into the upper pouch before the incision, this incision would be made easily with 11 number knife. This incision becomes at right lateral on upper pouch esophagus when viewed in supine position.  A 6 French nasogastric tube pushs into stomach from nasus to the distal esophageal lumen.

Edges prepared oblique of the each esophagus as resiprocally are sutured with interrupted 5/0 or 6/0 monoflament polyglycolic absorbable sutures until right and left corner without the edges being kept clamp under the deep generally anesthesia, back wall sutures tie, and front wall sutured complete interrupted.

CONCLUSION: Trough oblique to oblique anastomosis of each esophageal ends is proposed to obtain a wide anastomosis without tension and lost of esophageal tissue from the tips. This oblique anastomosis is simple, safe, and effective tecnique in preventing stricture formation. This anastomotic tecnique could be performed through a classic right thoracotomy or thoracoscopy.

ÖZOFAGUS ATREZİSİNDE ANASTOMOZ TEKNİĞİNDE İNCELİKLER: OBLİK-TO-OBLİK ANASTOMOZ

GİRİŞ/AMAÇ: Anastomoz darlığı özofagus atrezisi onarımı sonrası önemli bir sorundur. Darlık oluşum patogenezinde, çeşitli kolaylaştırıcı faktörler suçlanmıştır; bunların arasında, anastomoz tekniği darlıkların önlenmesinde çok önemli bir rol oynamaktadır. Burada oblik-to-oblik anastomoz tekniği tarif edilmiştir.  

TEKNİK TANIMLAMA: Bu teknikte, distal trakeoesophageal fistül trakeadan dikey olarak ayırılır ve trakeal açıklık dikey olarak tek tek 6/0 prolen dikişlerle sütüre edilir. Serbest kalan alt özofagusun üst ucu medial duvarında (yani özofagusun sol tarafında) 4-5 mm’lik boyuna kesi yapılır, bu kesi yatar pozisyonda bakıldığında distal özofagus üst uç sol yanındadır.

Üst özofagus poşu mobilize edildikten sonra, en uç noktaya asıcı sütur konulur ve buradan yukarıya doğru üst poşun yan tarafında 9-10 mm boyuna kesi ile üst özofagus açılır. Kesi öncesi üst özofagus poşu içine 4 numara endotrakeal tüp yerleştirilirse, bu kesi 11 numara bistüri ile kolayca yapılır. Yatar pozisyonda bakıldığında bu kesi üst özofageal poşun sağ yanında kalır. Burun deliğinden ilerletilen 6 numara nazogastrik sonda üst esofagus ve alt esofagustan geçirilerek mideye yerleştirilir.

Oblik olarak hazırlanan alt ve üst özofagus kenarları karşılıklı olarak 5/0 veya 6/0 monoflament poliglikolik emilebilir dikişlerle sağ ve sol köşesinde kadar sütüre edilir, derinleştirilmiş genel anestezi altında iken arka duvar sütürleri bağlanır ve ön duvar anastomozu separe sütürlerle tamamalanır.  

SONUÇ: Her bir özofagus ucunun oblik-to-oblik anastomozu sayesinde gergin olmayan geniş bir anastomoz ve esofagus uçlarından doku kaybı olmaması amaçlanmıştır. Bu oblik anastomoz, darlık oluşumunu önlenmesinde basit, güvenilir ve etkin bir tekniktir.  ve komplikasyonların önlenmesi açısından, basit, güvenli ve etkili bir yöntemdir. Bu anastomoz tekniği, klasik bir sağ torakotomi veya torakoskopi ile yapılabilir.
