A RARE TESTICULAR TUMOR; CAVERNOUS HEMANGIOMA

Background and Aim: Childhood malign testicle tumors are very rare. In a recent paper dated 2007, it has been reported that there is only 19 cavernous hemangioma cases exist in English literature. In this study we discuss a testicular mass case which diagnosed as cavernous hemangioma.

Case report: The patient, who followed for left hydronephrosis/non-functioning kidney since her birth, was admitted to our clinic with complaints of left testicular mass and pain at his 1-year of age. Patient was lost for follow-up, however he re-admitted to our clinic at his 3-year of age with persistent mass in left testicle. On physical examination, left testicle was firm and nodular with palpation, and ultrasonography measurements were as follows; right testicle 13,3x7 mm, left testicle 24,5x15,8 mm. Left testicular paranchyma was hypoechoic heterogenous and with rough granules in it. On Doppler ultrasonography, left testicle was 12x17x27 mm in size and there was a solid cystic lesion 12x13x22 mm in size which contains multiple milimetric cystic cavities. Lesion was found vascularized at its inferior part. Spectral investigation revealed high resistance blood flow. There was a lymph node 7x3 mm in size with cortical hypertrophy, and its fatty hilum was found faded. Tumor markers were in normal range. During operation, we found that there was no testicular tissue and whole testis was cavernous in nature. Histopathological investigation revealed cavernous hemagioma.

Conclusion: Cavernous hemangioma is the rarest tumor of the testicle, either in childhood or adult period. Even it has its characteristic ultrasonographic findings, radiology is far beyond to eliminate the malignancy, and final diagnosis can only be made after orchiectomy. In testicle mass cases with negative tumor markers and hypo-echogenic central part with hyper-echogenic periphery by ultrasonography, cavernous hemangioma should be kept in mind.

TESTİSİN NADİR BİR TÜMÖRÜ; KAVERNÖZ HEMANJİOM

Giriş ve Amaç; Çocukluk çağı malign testis tümörleri nadir görülür. 2007 yılına ait bir çalışmada, literatürde 19 kavernöz hemanjiom vakasının rapor edildiği bildirilmiştir. Bu çalışmada, testis kitlesi ile takip edilen olgumuz kavernöz hemanjiom tanısı alması nedeniyle tartışılmıştır.

Olgu sunumu: Doğumundan itibaren sol hidronefroz/nonfonksiyone böbrek olarak takip edilen olgu bir yaşında iken sol testiste şişlik ve ağrı şikâyeti ile 1 yaşında polikliniğimize başvurdu. Takipten çıkan hasta 3 yaşında iken sol testiste devam eden şişlik şikâyeti ile yeniden değerlendirildi. Fizik muayenede sol testis palpasyon ile sert ve nodüler yapıda olup ultrasonografi ile sol testis boyutları 24,5 x 15,8 mm sağ testis boyutları 13,3x7 mm olarak ölçüldü. Sol testis parankim ekosu hipoekoik heterojen kaba granülerdi. Doppler Ultrasonografi de sol testis boyutları 12x17x27mm ve parankim içerisinde 12x13x22 mm boyutlarında multipl milimetrik boyutlu kistik açıklıklar içeren solid kitlesel lezyonlar izlendi; ayrıca lezyonun inferior kesiminde vaskülarizasyon mevcuttu. Spektral incelemede yüksek dirençli akım saptandı. Sol inguinal bölgede 7x3 mm boyutlarında, yağlı, hilusu silinmiş, kortikal hipertrofi gösteren lenf nodu izlendi. Tümor belirteçleri normal sınırlarda idi. Operasyon sırasında sol testis dokusunun olmadığı, testis tamamen kavernöz görünümde olduğu saptandı. Orşiektomi sonrası patoloji sonucu kavernöz hemanjiom olarak rapor edildi.

Sonuç: Erişkin ve/veya çocukluk çağında, testisin kavernöz hemanjiomu en nadir görülen testis tümörüdür. Ultrasonografik olarak karakteristik görüntüleri mevcut olmasına rağmen, radyoloji maligniteyi ekarte etmekten uzaktır ve kesin tanı ancak histopatolojik olarak konulabilir. Tümör belirteçlerinin negatif olduğu ve ultrasonografik olarak ortası hipoekojen çevresi hiperekojen görülen testis tümörlerinde, kavernöz hemanjiom olasılığı daima akılda bulundurulmalıdır.
